    Notes: Brain injury remains a significant and potentially devastating outcome of cardiopulmonary bypass (CPB) under circulatory arrest. These outcomes caused by a microvasculature embolization are associated with increased mortality, longer hospital stays and increased use of intermediate or long term care facilities. The administration of heparin in heart surgery during deep hypothermic cardiopulmonary bypass is the basic prophylactic strategy for reducing or even preventing, microvasculature embolization. Unfortunately, an incidence of neuropsychological impairments (NPI) is found in as many as 25 % of the survivors. As it is suspected that these impairments are correlated with morphological alterations, in our study we use the GP IIb-IIIa receptor blocker Eptifibatide for the inhibition of platelet aggregation, in order to look for a reduction of tissue damage compared to the standard treatment. Two groups of 11 piglets (mean body weight of 15±5 kg) underwent 10-minute normothermic bypass, 40-minute cooling on cardiopulmonary bypass, 60-minutes deep hypothermic circulatory arrest (DHCA) at 15°C, and 40-minute rewarming to 37°C. Group 1 was treated only with unfractionated heparin (UFH), whereas Group 2 was medicated with Eptifibatide, in addition to the UFH-treatment group 1. After rewarming, all animals underwent bilateral carotid perfusion with 4% paraformaldehyde. Histological investigations of semi thin sections reveal a marked decrease of hippocampal alterations by using the GP IIb-IIIa receptor blocker in addition to standard UFH treatment. We detect a reduction of degenerative areas in perivascular (vessels with 10–30 μm in diameter) tissue. These semi-quantitative data are confirmed by ultrastructural findings.

    Notes: Bone marrow derived stromal cells are of mesenchymal origin and precursor cells for skeletal tissue components such as chondroblasts and osteoblasts. Furthermore, under experimental conditions, a differentiation potency into myogenic and neuronal cells could be demonstrated. Because of their multipotency these cells represent a population of non-haematogen stem cells that can be regarded as an alternative to human embryonic stem cells for future autologous cell replacement therapies. For a closer look at the differentiation capacity of these cells, rat and human bone marrow stromal cells were isolated from the femur bone and kept in the cell culture applying different cultivation protocols. In a cultivation medium with a serum content of 20%, the majority of these cells express a variety of neuronal markers such as ß-III Tubulin and NeuN as well as the astrocyte marker GFAP, while a minority of about 20% express the marker for neural precursor cells nestin. Cultivation in a chemically defined serum free medium results in the differentiation of a markedly higher percentage of nestin positive neural precursor-like cells. Using bFGF in combination with B27 these cells can be forced to form three dimensionally organized spheres. In order to elucidate a possible therapeutical potency of the bone marrow derived cells the synthesis of neurotrophic factors such as BDNF and NGF were analysed using the ELISA technique. Furthermore, they can be infected using a third generation adenoviral vector with high efficiency and show migratory activity in vitro. After injection of bone marrow derived mesenchymal stem cells into the lateral ventricle of adult rats they adhere to the ependymocytes and pass the ependymal barrier in order to settle in the subventricular space.

    Description / Table of Contents: Zusammenfassung Anhand von 4 Azinuszelltumoren der Ohrspeicheldrüse wird (in Erweiterung bereits mitgeteilter Befunde) die feinstrukturelle Organisation dieser Geschwülste beschrieben. Die für die Geschwulstzellen typischen intracellulären Einschlüsse stellen teils strahlendichte PAS-positive, teils strahlendurchlässige PAS-negative Inklusionen dar; beide Arten von Zelleinschlüssen kommen nebeneinander vor. Die PAS-positiven strahlendichten Einschlüsse entsprechen den typischen Proenzymgranula der menschlichen und tierischen Speicheldrüsen. Die strahlendurchlässigen PAS-negativen Einschlüsse gleichen Gebilden, die auch in der menschlichen Speicheldrüsenendstückzelle nachweisbar sind. Die Ansammlung derartiger PAS-negativer Zelleinschlüsse in Zellen des Azinuszelltumors führen zu einem histologischen Bild, das mit dem der sogenannten mukoiden Zellumwandlung, also einer besonderen Form einer Proteodyschylie der Speicheldrüsen vergleichbar ist; es ist dies offenbar Ausdruck einer gestörten Sekretbildung in stärker geschädigten oder entdifferenzierten Geschwulstzellen. Durch die beschriebenen Unterschiede im färberischen Verhalten und im feinstrukturellen Aufbau der Zellgranula der Azinuszelltumoren können die diagnostischen Schwierigkeiten bei der Abgrenzung der Azinuszelltumoren von anderen Geschwülsten der Speicheldrüsen erklärt werden.
    Notes: Summary In four cases of acinic cell carcinoma of the parotid gland the fine structure of the tumor cells shows striking similarities to the serous acinar cells from human salivary glands. A detailed investigation of the cellular inclusions in the tumor cells reveals that two groups of cell inclusions occur: One group of tumor cell inclusions consists of electron dense amylase-resistant PAS-positive granules which correspond to the proenzyme granules of the serous acinar cell in the salivary glands. A second group of cell inclusions is electron transparent and PAS-negative. These inclusions are comparable in their morphological and fine structural properties to similar inclusions which can be found in the human serous salivary acinar cell, especially in the state of so-called mucoid degeneration. The electron-transparent cellular inclusions are interpreted to be the consequence of a disturbed production of secretory precursors occurring in damaged or less differentiated cells. Their presence in the cells of the acinic cell carcinoma explains some of the difficulties in the diagnosis of these rare tumors.

    Description / Table of Contents: Zusammenfassung Im Cylindrom der Speicheldrüsen spielen myoepithelial differenzierte Zellen beim Geschwulstaufbau eine wesentliche Rolle. Im Gegensatz zu den Verhältnissen beim normalen Speicheldrüsengewebe haben die basal gelegenen myoepithelialen Zellen den Zusammenhang mit den mehr epithelial differenzierten Geschwulstzellen vielfach verloren. Sie produzieren eine basalmembranartige Intercellularsubstanz, in der außer bandartigen Verdichtungen nach Art einer Lamina densa der Basalmembran sowohl aperiodische als auch periodisch gestreifte Fibrillen vorkommen. Bindegewebszellen sind bei der Bildung dieser zylinderbildenden Substanz nicht beteiligt. In den zum Vergleich untersuchten Speicheldrüsenmischtumoren sind strukturell und funktionell gleichartige Zellen ebenfalls für die Genese der intercellulären Substanz verantwortlich. Ebenso wie beim Mischtumor ist im Cylindrom der Speicheldrüsen eine echte exokrine Sekretion bei der Bildung der Intercellularsubstanz nicht nachweisbar. Ein Vergleich mit dem Cylindrom der Haut (Spiegler-Tumor) zeigt, daß hier, wie wohl auch in allen anderen cylindromatösen Geschwülsten ebenfalls eine basale Zellabsonderung zur Zylinderbildung führt. Myoepitheliale Zellen spielen aber bei diesem Vorgang im Cylindrom der Haut offenbar keine wesentliche Rolle, auch weicht die Struktur der Intercellularsubstanz von derjenigen der Speicheldrüsencylindrome ab. — Wie bei anderen Speicheldrüsengeschwülsten, bei denen Myoepithelzellen vorkommen, ist der Ausgangspunkt der Speicheldrüsencylindrome in die Schaltstücke zu verlegen. Die hier vorkommenden indifferenten Zellen sind bei der Geschwulstbildung in der Lage, die verschiedenartigen Differenzierungsmöglichkeiten des Speicheldrüsenepithels zu verwirklichen. Dies ist besonders im Speicheldrüsenmischtumor der Fall, wo in den epithelialen Abschnitten nahezu alle epithelialen Differenzierungspotenzen realisiert werden können. Die myoepithelial differenzierten Zellen zeigen Störungen im Zellzusammenhang und eine überschießende Bildung von basalmembranartiger Intercellularsubstanz, wobei es zur Bildung der schleimartigen und knorpeligen Strukturen kommt. Im Speichelgangcarcinom ist bei geringer epithelialer Zelldifferenzierung und regelhaftem Zellzusammenhang die Funktion der Myoepithelzellen offenbar intakt: Diese Geschwülste zeigen einen organoiden Aufbau. — Im Cylindrom schließlich ist die epitheliale Zelldifferenzierung ähnlich wie im Speichelgangcarcinom nur unvollkommen verwirklicht; die strukturellen und funktionellen Störungen der myoepithelialen Zellen, nämlich ein gelockerter Zellzusammenhang bei vermehrter Produktion von Intercellularsubstanz sind für die Ausbildung der typischen Geschwulststrukturen verantwortlich.
    Notes: Summary Studies of the fine structure of the cylindroma (Adenoidcystic carcinoma) of the salivary glands revealed that myoepithelial cells are structurally important in this tumor. The cells produce large quantities of interstitial, basement membrane-like, fibrillar substance which forms the typical cylindrical structures. Connective tissue cells do not take part in producing this substance. The myoepithelial cells are in part separated from the epithelial cells, and may be present also within the interstitial material. — Comparison with a dermal cylindroma (Spiegler tumor) suggests that the process of “basal secretion” is also responsible for the formation of the cylindrical structures in cylindromas of other organs. The basal formation of intercellular substance in the adenoidcystic carcinoma is comparable to the production of “myxomatous” interstitial substance by myoepithelial cells in the so-called mixed tumor (Pleomorphic adenoma). — It can be assumed that the origin of the adenoidcystic carcinoma, as that of other tumors consisting of myoepithelial and epithelial cells can be traced to the peripheral duct system, presumably the intercalated ducts. Variations in the differentiation and function of epithelial and myoepithelial cells in the mixed tumor, the salivary duct carcinoma, and the adenoid-cystic carcinoma account for the different structure and growth pattern in these tumors.

    Description / Table of Contents: Summary The fine structure of a typical oncocytoma of the parotid gland reveals an excessive accumulation and hypertrophy of the mitochondria in the cells of this tumor. It is supposed, that this accumulation of mitochondria, which occurs also in oncocytes of other tissues is caused by a defect of the intramitochondrial metabolism.
    Notes: Zusammenfassung Die Zellen eines typischen Onkocytom der Gl. Parotis zeigen bei elektronenmikroskopischer Untersuchung eine exzessive Vermehrung und Vergrößerung der Mitochondrien. Als Ursache dieser bei Onkocyten verschiedenster Herkunft nachweisbaren Hyperplasie des Chondriom wird eine intramitochondriale Stoffwechselstörung angenommen.

The recently described positive PAS-reaction (Periodic-acid-Schiff) in the cytoplasm of erythroblasts in the bone marrow of patients with kidney diseases can be reproduced in animal experiments: Variably high numbers of PAS-positive erythroblasts are found in the bone marrow of bilaterally nephrectomized rats. The PAS-positive erythroblasts belong to the maturity stages of polychromatic and oxyphilic erythroblasts. 2. The appearance of a positive PAS-reaction in the cytoplasm of erythroblasts is connected with necrobiosis of these cells. PAS-positive erythroblasts are phagocytized by reticulum cells of the bone marrow. Moreover, as reaction of the necrobiosis of these erythroblasts a strong, diffuse plasmocytosis and eosinophilia are found in the bone marrow of nephrectomized rats. 3. The uremia does not cause the appearance of PAS-positive erythroblasts. 4. A heat-resistant factor in the kidney of rats, rabbits, and human beings — probably a lipid bound to proteins — prevents the appearance of PAS-positive erythroblasts. 5. The PAS-positive material in the cytoplasm of erythroblasts is composed mainly of glycogen and lipids, but also of glyco- or mucoproteids or neutral mucopolysaccharides too. Furthermore, small amounts of acidic mucopolysaccharides and of the amino acid tyrosine as well as greater amounts of free NH2-groups and 1,2 glycols are found.

Die früher bei nierenkranken Patienten im Knochenmark gefundene positive PAS-Reaktion (Perjodsäure-Schiff) im Cytoplasma von Erythroblasten ist im Experiment reproduzierbar. PAS-positive Erythroblasten treten im Knochenmark doppelseitig nephrektomierter Ratten in wechselnd hoher Zahl auf, wobei die Erythroblasten der Reifestufe der polychromatischen und oxyphilen Erythroblasten angehören. 2. Das Auftreten einer positiven PAS-Reaktion im Cytoplasma von Erythroblasten steht im Zusammenhang mit einer Nekrobiose dieser Zellen. PAS-positive Erythroblasten werden vermehrt von Knochenmarks-reticulumzellen phagocytiert. Außerdem besteht eine starke diffuse Plasmocytose und Eosinophilie des Markes, die als Reaktion auf die Nekrobiose der Rattenerythroblasten angesehen wird. 3. Die Urämie kann als Ursache für das Erscheinen PAS-positiver Erythroblasten ausgeschlossen werden. 4. Ein hitzestabiler Nierenfaktor, der wahrscheinlich ein an Eiweiß gebundenes Lipid darstellt und bei Ratte, Kaninchen und Menschen vorkommt, verhindert das Auftreten PAS-positiver Erythroblasten. 5. Das PAS-positive Material im Cytoplasma der Erythroblasten setzt sich vorwiegend aus Glykogen und Lipiden daneben auch aus Glyko- oder Mucoproteiden oder auch neutralen Mucopolysacchariden zusammen. Weiterhin finden sich kleine Mengen saurer Mucopolysaccharide und der Aminosäure Tyrosin sowie reichlich freie NH2-Gruppen und 1,2-Glykole.
