Summary 1. Pituitary tumors are located in an area bordering the sphenoidal sinus. They can be treated by 3 methods: by cranial operation, by transphenoidal operation and by radiation with x-ray or radium. 2. This report deals with series of 413 patients operated by the transphenoidal method. 3. 277 patients have been operated on during the preantibiotic era, 1919 to 1937 (incl.) with a mortality of 5,4%, 73 patients have been operated during the antibiotic era with a mortality of 1,4%. 4. The follow-up revealed that 68% of a series of 140 patients who were 5 to 19 years after operation showed lasting improvement, 5. 25 patients are known maintaining or having maintained the improvement for 19 1/2 to 35 years. 6. The importance of postoperative, local radium radiation is stressed.

Résumé 1. Les tumeurs de l'hypophyse sont localisées dans la région voisine au sinus sphénoidal; elles peuvent être traitées d'après trois méthodes: l'opération chirurgicale par voie intracranienne, celle par la voie sphénoïdale et la radium- ou roentgenthérapie. 2. L'A. rapporte ses observations personnelles sur 413 cas de tumeurs hypophysaires opérés par la méthode transphénoïdale. 3. 277 malades ont été opérés avant l'ère des antibiotiques (1919 à 1937 incl.) avec une mortalité de 5,4% tandis que chez 73 cas opérés durant l'ère antibiotique ont a eu que 1,4% de décès. 4. Le 68% d'un groupe de 140 patients observés de 5 à 19 ans après l'opération révélait une amélioration persistante. 5. 25 patients révélaient, ou revèlent, une amélioration persistante durant 19 1/2 à 35 ans, signe, celui-ci, qui laisse croire qu'une amélioration durable toute la vie peut être obtenue. 6. L'A. met. en évidence l'importance du traitement local post-opératoire avec le radium. Ce traitement est facilité par 1 accessibilité de la région de l'opération.

Zusammenfassung 1. Die Hypophysentumoren sind ein Grenzgebiet. Drei Methoden stehen zu deren Behandlung zur Verfügung: die Operation auf kranialem, die Operation auf transsphenoidalem Wege und die Strahlentherapie mit Röntgen oder Radium. 2. Es wird aus eigener Erfahrung über 413 Hypophysentumoren berichtet, die nach transsphenoidalem Wege operiert worden sind. 3. 277 Patienten wurden in der vor-antibiotischen Ära 1919 bis 1937 (inkl.) mit einer Mortalität von 5,4% operiert, 73 in der antibiotischen Ära mit einer Mortalität von 1,4%. 4. Von einer Serie von 140 Patienten, die 5 bis 19 Jahre nach der Operation waren, wiesen 68% anhaltende Besserung auf. 5. 25 Patienten wiesen oder weisen eine 19 1/2- bis 35jährige anhaltende Besserung auf, ein Zeichen, daß lebenslängliche Besserung erzielt werden kann. 6. Es wird auf die Wichtigkeit der postoperativen, lokalen Radiumbestrahlung hingewiesen, die durch die bleibende Zugänglichkeit des Operationsfeldes erleichtert wird.
