    Notes: Abstract A 16-year-old boy is described who had a relapse of a phaeochromocytoma 6 years after an initially successful tumour resection. The relapse was suspected after routine testing of urinary catecholamine excretion and was confirmed by scintigraphy with 123I-meta-iodobenzylguanidine, computed tomography and magnetic resonance imaging. The plasma norepinephrine level was 3082 pg/ml (normal 〈500 pg/ml); the plasma epinephrine level was in the normal range. Surprisingly, our patient had no symptoms, including hypertension. The density of the α- and β-adrenoceptors on circulating blood cells was decreased. Postoperatively the plasma catecholamine levels were in the normal range. Three months after surgery the adrenoceptor density was almost normal. We conclude that the absence of clinical symptoms was probably due to desensitization of the adrenoceptors. After a successful operation to treat phaeochromocytoma, long-term monitoring of catecholamines is necessary to rule out an asymptomatic relapse.

    Notes: Abstract Clinical observations have shown that some asthma patients develop tachyphylaxis to β-sympathomimetic drugs. As down-regulation of the number of β-adrenoceptors in different human tissues after exposure to catecholamines and β-adrenergic drugs is well known, we investigated whether a interrelation exists between β-adrenoceptor down-regulation and clinically detectable β-adrenergic subsensitivity during β-sympathomimetic treatment. The following results were obtained: 1. β2-Sympathomimetic inhalation treatment with salbutamol in therapeutic doses led to a significant downregulation of β2-adrenoceptors and consecutive cyclic adenosine monophosphate response to isoprenaline. This effect was already detectable after short-term treatment of 3–7 days in 9 asthmatic children. 2. In the long-term study over 6 months, salbutamol inhalation in 12 asthmatic children led to a significant down-regulation of β-adrenoceptor binding sites on mononuclear blood cells (MNC) from 1539±91 to 1115±99 after 14 days, remaining in this range thereafter. 3. The mean airway resistance (Raw) of these 12 patients decreased significantly within 14 days from 8.1±0.8 to 5.7±0.5 cm H2O/l/s to remain stable throughout the 6 months of salbutamol treatment. The differences in Raw before and immediately after inhalation of 0.2 mg salbutamol (2 puffs) were unchanged during the study period. It is concluded, that long-term inhalative treatment with salbutamol over a period of 6 months does not result in refractoriness to β-adrenergic drugs in the airways of asthmatic children, even though a significant down-regulation of β2-receptors on peripheral MNC occurs.

    Notes: Summary Experiments were performed on isolated electrically driven and spontaneously beating atria from guinea pigs and rabbits in order to characterize the cardiostimulatory effect of phenylephrine as either β- or α-sympathomimetic. 1. In electrically driven left atria (1 and 2 Hz) of guinea pigs the positive inotropic effect of phenylephrine was competitively antagonized by the β-adrenolytic drug pindolol but not by the α-adrenolytic agent phentolamine. The same was true for electrically driven (2 Hz) atria of rabbits. The intrinsic activity for phenylephrine amounted to only 0.3 in the guinea-pig atrium and 0.46 in the rabbit atrium. Also in preparations from reserpine-pretreated guinea pigs the positive inotropic effect of phenylephrine was blocked by pindolol and remained uninfluenced by phentolamine. Pretreatment with reserpine did not change the intrinsic activity of phenylephrine. Three other α-sympathomimetic drugs, oxymetazoline methoxamine and naphazoline did not cause any positive inotropic effect. 2. In spontaneously beating atria of either guinea pigs or rabbits the positive chronotropic effect of phenylephrine was competitively inhibited by pindolol. Phentolamine proved to be without any effect. The intrinsic activity for the positive chronotropic effect with 0.52 in guinea-pig atria and 0.47 in rabbit atria was less than that of isoprenaline (1.0). 3. The results presented here show that at least on atria only β-adrenoceptors are of functional importance.
