Zusammenfassung Die Behandlung offener Knochenbrüche hängt in erster Linie vom Weichteilschaden ab, die stabile Osteosynthese soll der Wundheilung dienen. An der Unfallchir. Klinik der MHH wurden in $$4{\raise0.5ex\hbox{$\scriptstyle 1$}\kern-0.1em/\kern-0.15em\lower0.25ex\hbox{$\scriptstyle 2$}}$$ Jahren 232 Patienten mit 260 offenen Frakturen behandelt. $${\raise0.5ex\hbox{$\scriptstyle 1$}\kern-0.1em/\kern-0.15em\lower0.25ex\hbox{$\scriptstyle 3$}}$$ aller Brüche war 1.-gradig offen, 44% 2.-gradig und 26% 3.-gradig mit schwersten Weichteilschäden bis zur traumatischen Amputation. 88% aller Frakturen wurden operativ ver sorgt. Daraus resultierten 6,1 Infekte und 30 Pseudarthrosen. In 2,3% mußte wegen Ischämie nach Erhaltungsversuchen mit Gefäßläsionen amputiert werden.

Summary In $$4{\raise0.5ex\hbox{$\scriptstyle 1$}\kern-0.1em/\kern-0.15em\lower0.25ex\hbox{$\scriptstyle 2$}}$$ years, 232 patients with 260 compound fractures were treated at the trauma center of the Hannover Medical School. One third of all these fractures involved a first-degree soft-tissue lesion, 44 percent a second-degree lesion and 26 percent a third-degree lesion (which includes severe soft-tissue damage to the extent of traumatic amputation). Of all these fractures, 88 percent were treated by internal fixation; the results included bone infection in 6.1 percent and nonunion in 3 percent. Amputation was necessary in 2.3 percent due to ischemia following severe compounds lesions with vascular damage.
