Zusammenfassung Es wird über einen 17jährigen Jungen mit megaloblastärer Anämie und benigner Proteinurie seit frühester Kindheit berichtet. Bei normalem Magensaft mit normalem Gehalt an Intrinsic Faktor wurde als Ursache der Anämie eine selektive Malabsorption für Vitamin B 12 nachgewiesen. Es handelt sich um den 66. der Weltliteratur und den 3. aus Deutschland berichteten Fall eines sog. Imerslund-Gräsbeck-Syndroms. Die Besonderheit besteht in einem bisher in dieser Kombination nicht beobachteten Antikörpermangelsyndrom (IgG und IgA), welches auch bei der Mutter und bei den beiden Schwestern nachweisbar war.

Summary The case of a 17-year old boy with megaloblastic anaemia and benign proteinuria since early childhood is reported. Normal gastric juice with normal content of intrinsic factor was found. A selective malabsorption for vitamin B12 was detected as the cause of the anaemia. It is the 66th case in the world literature and the 3rd case recorded from Germany of the so-called Imerslund-Gräsbeck syndrome. The special features of this case were a hitherto not observed combination with a defect in antibody formation (IgG and IgA). The same defect was detected in the mother and the two sisters of the patient.
