    Notes: A modified method of access to the upper thoracic spine is presented based on a case report. The third and fourth thoracic vertebral bodies can be reached by partial upper sternotomy. This approach takes the local anatomic situation into account, thus avoiding the typical complications of complete sternotomy and reducing the postoperative pain. Closure was done using biodegradable sutures, achieving satisfactory stabilisation and a low rate of complications. In only 4 of 130 cases (3%) did a superficial wound infection occur. There was no case of deep infection, osteomyelitis or dehiscence of the sternum.

    Description / Table of Contents: Zusammenfassung Von den zahlreichen Methoden des Ersatzes der subfibularen Bänder wurden 4 Verfahren in einer vergleichenden biomechanischen Studie unter einheitlichen Versuchsbedingungen an 36 Präparaten überprüft. Dabei wurden eine eigene, anatomiegerechte Technik den Verfahren von Paar, Watson Jones und B. G. Weber gegenübergestellt. Wie an den Plexiglasmodellen dargestellt wurde, können bei den meisten Methoden Dehnungen und Gleitbewegungen beobachtet werden, wenn sie nicht die anatomischen Insertionspunkte berücksichtigen. Der stabilisierende Effekt ist bei allen Verfahren ausreichend. Bei den Tenodeseverfahren, besonders bei Watson Jones ist aber die Beweglichkeit, besonders in Supination-Inversion erheblich eingeschränkt.
    Notes: Summary Four methods for the replacement of subfibular ligaments were compared in a biomechanical study with 36 cadaver joints. The anatomical procedure originally described was compared to the methods of Paar, Watson Jones and B. G. Weber. As has been shown with acrylic models, most methods produced elongation and sliding movements when the anatomical insertion points were not taken into consideration. The stabilizing effect is satisfactory in all techniques, but motion, especially supination, is restricted by tenodesis methods like the Watson Jones technique. Only anatomical replacement of the ligaments enables joint clearance, stability and range of motion to be restored.
