Summary Typhoid fever in children is uncommon in industrialized countries with only few reports on diagnosis and management in this setting. Enteric perforation is the complication of typhoid fever associated with the highest morbiditiy and mortality. We report on a 9-year-old girl with severe abdominal symptoms based on a S. typhi infection. The purpose of this report is to highlight the diagnostic and therapeutic difficulities encountered in typhoid fever in children in an area where typhoid is not endemic. Beyond the various differential diagnosis it is essential to unmask typhoid fever, because surgical treatment is only indicated in the cases of abdominal complications.

Zusammenfassung Bei Kindern kommt Typhus abdominalis in den Industrieländern nur vereinzelt vor. Dementsprechend sind die Literaturangaben über die oft schwierige Diagnostik und Therapie nur spärlich. Intestinale Perforationen sind die Komplikationen, die für die hohe Morbititäts- und Mortalitätsrate verantwortlich sind. Wir berichten von einem 9-jährigen Mädchen, das mit schwerer abdomineller Symptomatik bei S. typhi Infektion behandelt wurde. Ziel dieser Fallbesprechung ist die diagnostischen und therapeutischen Schwierigkeiten bei einer S. typhi Infektion aufzuzeigen, mit der wir in Ländern, in denen der Typhus abdominalis nicht endemisch vorkommt, konfrontiert werden. Unter den möglichen Differentialdiagnosen ist es essentiell, den Typhus abdominalis zu erkennen, da eine Operation bei dieser Erkrankung nur bei intestinalen Komplikationen angezeigt ist.
