Zusammenfassung In der Chirurgie der infizierten Pseudarthrosen and der offenen Frakturen III. Grades hat sich die Indikation der Fixateur externe-Osteosynthese bewährt. Fast 500 Fixateur externe-Osteosynthesen haben die Entwicklung stabilerer Montageformen vorangetrieben, so dad heute die übungsstabile Fixierung auch kleiner gelenknaher Fragmente am Unterschenkel bei gleichzeitig vorliegendem Defekt moglich ist. Biomechanische Untersuchungen haben die Leistungsfahigkeit des Raumlichen Fixateur externe untermauert. Die technische Anwendung ist anhand groBer Farbaufnahmen gezeigt worden.

Summary Stable fixation with external clamps has proved its worth in the surgical treatment of infected pseudarthroses and of grade-III compound fractures. The performance of stable fixation with external clamps in almost 500 cases has accelerated the development of more stable types of fixation, so that it is now also possible to achieve rigid fixation of smaller fracture fragments even when the defect is near a joint. Biomechanical experiments have proved the efficiency of three-dimensional fixation. Large pictures illustrate the technical use.
