    Description / Table of Contents: Summary Experimental studies with the intracutaneous pyrogentest of a series of cases of posttraumatic osteomyelitis are reported. Regarding the local inflammatory reaction of the skin, noted in this cases, it corresponded with that introduced byHeilmeyer under the term “Allophlogisty”. In one group of patients eliciting severe signs of infection, the findings with the pyrogentest were those of a hypophlogistic reaction. It is concluded that in these cases the defense reaction of the organism against the microbial agents was inadaequate also.
    Notes: Zusammenfassung Durch den Pyrogenhauttest werden an Patienten mit chronischer posttraumatischer Osteomyelitis Befunde erhoben, die sich in die Vorstellung über die Allophlogistie nachHeilmeyer einordnen lassen. Bei einer Gruppe von Patienten mit schwerer klinischer Verlaufsform der Erkrankung wurde ein Entzündungsablauf im Pyrogenhauttest beobachtet, der als Nachweis einer Hypophlogistie gewertet werden kann. In diesen Fällen ist demnach auch mit einer herabgesetzten unspezifischen Entzündungsbereitschaft gegenüber den pyogenen Erregern zu rechnen.

Durch eine systematische klinische Untersuchung werden der Stabilitätsverlust und damit die verletzten Strukturen aufgedeckt, wobei insbesondere auf die Rotationsinstabilität beim komplexen Stabilitätsverlust hingewiesen wird. An Hand von Skizzen und Tabellen wird der Untersuchungsablauf für die 3 topographischen Bereiche — mediales System, laterales System und Kreuzbänder — aufgezeigt.
    Notes: Summary The importance of capsular ligaments, in addition to the collateral ligaments and crucial ligaments, for stability of knee joint is accented. By systematic clinical examination the loB of stability and thereby the injured structures are determined with particular reference to rotatory instability by complex loB of stability. By sketches and tables, the examination procedure for the three topographic fields-medial system, lateral system, and crucial ligaments—is shown.

    Description / Table of Contents: Zusammenfassung 75 Patienten wurden nach Arthrodese des oberen Sprunggelenkes im Mittel 11 Jahren nach Operation erfaßt. Funktionsaufnahmen des jeweilig betroffenen Fußes zeigten eine überwiegend im Talo-Naviculargelenk lokalisierte Kompensationsbeweglichkeit zwischen 7° und 34°. Entsprechend der veränderten Mechanik wurden im Talo-Kalkanear- und Talo-Naviculargelenk in ca. $${\raise0.7ex\hbox{$1$} \!\mathord{\left/ {\vphantom {1 3}}\right.\kern-\nulldelimiterspace}\!\lower0.7ex\hbox{$3$}}$$ der Fälle röntgenologische Anschlußarthrosezeichen gefunden, die jedoch klinisch nicht immer in Erscheinung traten. Die routinemäßige Mitversteifung des Talo-Naviculargelenkes bei Arthrodesen des oberen Sprunggelenkes ist somit nicht zu empfehlen.
    Notes: Summary 75 patients in whom a fusion of the ankle joint was performed, were checked up 11 years after the operation on average. Functional X-rays exposed a mobility of the foot in sagittal direction of about 20° which was localized mainly in the talonavicular joint. According to the alterations of mechanical stress in the region of the talocalcaneal — and talonavicular joint, posttraumatic arthritis was found there in about $${\raise0.7ex\hbox{$1$} \!\mathord{\left/ {\vphantom {1 3}}\right.\kern-\nulldelimiterspace}\!\lower0.7ex\hbox{$3$}}$$ of patients. We advise against routine fusion of the talonavicular joint when performing fusion of the ankle joint.

    Description / Table of Contents: Zusammenfassung Zur Frage der klinischen Bedeutung einer in menschlichen Geweben nachgewiesenen Dextranase wurden bei Pat. mit latentem und manifestem Diabetes mellitus Rheomacrodex- und Macrodexinfusionen durchgeführt. Gegenüber den Normalbedingungen konnten unter den Dextraninfusionen Änderungen der Blutzuckertageskurven und der Glucoseausscheidung im 24-Std-Urin nicht festgestellt werden. Zusammengefaßt sprechen die Ergebnisse dafür, daß auch beim Vorliegen einer herabgesetzten Glucosetoleranz der hydrolytische Abbau der Dextrane im menschlichen Organismus quantitativ zu vernachlässigen ist. Den Befunden entsprechend tritt auch eine klinisch bedeutsame Aktivierung oder Induktion der Dextranase unter wiederholter Dextraninfusion bis zu 10 Tagen nicht ein. Aus den Untersuchungen ergibt sich keine Einschränkung für die Verabreichung von Dextranen bei Pat. mit einem Diabetes mellitus.
    Notes: Summary In an attempt to investigate the clinical importance of a dextranase present in human tissues, a number of patient suffering from latent or manifest Diabetes mellitus were subjected to infusions with Rheomacrodex and Macrodex. These infusions did not cause any variations in the diurnal blood sugar curve and in the excretion of glucose in the urine (collected over 24 hours), compared to normal controls. The results seemed to indicate that even in cases of a reduced tolerance for glucose the hydrolytic metabolism of dextrans is of little quantitative importance in the human organism. The results also showed that even repeated infusions of dextran do not induce a clinically significant activation or induction of the dextranase within 10 days. The investigation showed that there is no reason to restrict the administration of dextrans in patients with Diabetes mellitus.

Die Aufschlüsselung des eigenen Krankengutes der letzten 1 1/2 Jahre zeigt, an welchen Bereichen das Behandlungsproblem klinisch besonders relevant wird. Bei gelenknahen Frakturen ist die Osteosynthese als Behandlungsmaßnahme zu fordern, Übungsstabilität ist anzustreben. Es werden die typischen operativen Behandlungsverfahren für den jeweiligen Bereich mit Strichzeichnungen dargestellt und ergänzend klinische Beispiele mit Diapositiven von Röntgenbildern gezeigt.
    Notes: Summary The author reports the typical mechanisms resulting in dislocations in juxta-articular fractures. Analysis of his own patients over 18 months shows, in which spheres the problem of treatment is especially relevant. Internal fixation is necessary for the treatment of fractures near joints, and stability during movement should be attained. The typical methods of operative treatment are shown with line drawings and X-ray slides of clinical cases.

Biomechanische Untersuchungen haben die Leistungsfahigkeit des Raumlichen Fixateur externe untermauert. Die technische Anwendung ist anhand groBer Farbaufnahmen gezeigt worden.
    Notes: Summary Stable fixation with external clamps has proved its worth in the surgical treatment of infected pseudarthroses and of grade-III compound fractures. The performance of stable fixation with external clamps in almost 500 cases has accelerated the development of more stable types of fixation, so that it is now also possible to achieve rigid fixation of smaller fracture fragments even when the defect is near a joint. Biomechanical experiments have proved the efficiency of three-dimensional fixation. Large pictures illustrate the technical use.

Anschlie\end wird auf einige wichtige technische und biomechanische Gesichtspunkte der Fixateur-externe-Osteosynthese eingegangen. Auf Grund klinisch experimenteller Untersuchungen ist eine räumliche Anordnung des Fixateur externe zu empfehlen, mit der auch bei knöchernen Defekten und beim Vorliegen kurzer metaphysärer Bruchstücke Stabilität erreicht werden kann. Die Ergebnisse nach der Behandlung von aseptischen und infizierten Tibiapseudarthrosen werden tabellarisch dargestellt.
    Notes: Summary The indications for external fixation with fixateurs externes are discussed and some important technical and biomechanical aspects are reviewed. The three-dimensional fixateur externe is recommended on the basis of clinical experimental research. Stability can be obtained with this even when bone defects and short metaphyseal fragments are present. The results observed after the treatment of aseptic and infected pseudarthroses of the tibia are shown in tabular form.
