Zusammenfassung Die Kreuzbandplastik ist durch die räumliche Enge der Intercondylärgrube erschwert. Nicht immer gelingt die exakte Nachahmung des anatomischen Bandverlaufs durch die Plastik. Nach experimenteller Feststellung der Kreuzbandwinkel wurde ein geradlininger Bohrkanal gefunden, welcher den Bandersatz wesentlich vereinfacht. Dazu ist ein Zielgerät erforderlich. Mit den verschiedenen kannelierten Bohrern kann über einen Kirschner-Draht gearbeitet werden. Damit eignet sich das neue Gerät sowohl für das endoskopische Vorgehen wie am offenen Gelenk oder mit Durchleuchtungskontrolle. Die Technik ist an kein bestimmtes Transplantat gebunden.

Summary Replacement of the cruciate ligaments is complicated by the narrowness of the intercondylar region. Exact imitation of the anatomical function of the ligament is not always achieved. In experiments with cadavre knees using X-ray goniometry the angles of the cruciate ligaments were determined. A straight drill hole was used to simplify the replacement procedure. A new drill guide has been constructed. Various cannelated drills are used after insertion of a guide wire. The new instrument is suited to an endoscopic procedure and to operations on the open joint. Radiographic control of the guide wire allows exact location of the transplant. This technique is not restricted to any specific type of transplant.
