Summary Right atrial thrombi are rare, but potentially life-threatening complications of ventriculoatrial shunts. We report on a 4.6 year old girl, presenting with signs of subacute valve malfunction; in her cerebrospinal fluid koagulase-negative staphylococcus could be cultured, and the CF protein was elevated. Bidimensional echocardiography demonstrated a large thrombus on the distal part of the shunt. The thrombus protruded into the right ventricle during diastole. The child underwent cardiotomy and thrombectomy. Discussion: The reported case emphasizes the necessarity of echocardiographic follow-up of all patients with ventriculoatrial shunts.

Zusammenfassung Thromben im rechten Vorhof sind eine seltene, aber potentiell lebensbedrohliche Komplikation ventrikulo-atrialer Shuntsysteme. Wir berichten über ein 4,6 Jahre altes Mädchen, das mit den Zeichen einer subakuten Ventildysfunktion zur stationären Aufnahme kam. Im Liquor wurden Koagulase-negative Staphylokokken und ein erhöhtes Liquoreiweiß nachgewiesen. Vor der geplanten Ventilrevision wurde eine Echokardiographie durchgeführt. Es zeigte sich ein großer, an der atrialen Ableitungsspitze adhärenter Thrombus, der bei jeder Herzaktion durch die Trikuspidalklappe prolabierte. Der Thrombus mußte kardiochirurgisch entfernt werden. Diskussion: Der vorgestellte Fall unterstreicht die Notwendigkeit regelmäßiger, echokardiographischer Kontrollen bei Patienten mit ventrikuloatrialen Shuntsystemen.
