Tibia fractures with soft tissue damage are still a challenging problem for orthopaedic surgeons. The rates of infection, osteitis or non-union are higher than for other fractures. In the last decade we have seen a new and more biological approach to these fractures. The importance of very early soft tissue care for good long-term results is now widely accepted. New kinds of osteosynthesis that take account of the blood supply of the bone have led to much better results. It is very important that a management-protocol combining the therapy of bone and soft tissue is established right from the beginning.

Infolge der geringen Weichteilbedeckung sind Unterschenkelfrakturen nach wie vor als problematische Verletzungen anzusehen. Die Rate komplizierter Heilungen bzw. posttraumatischer Osteitiden ist im Vergleich mit anderen Frakturen – insbesondere nach Hochrasanztrauma – deutlich erhöht. In den letzten Jahren ist ein Umdenken im therapeutischen Management zu beobachten: Neue “biologische Osteosyntheseverfahren” in Verbindung mit der Erkenntnis, daß der frühzeitigen Sanierung der Weichteile höchste Priorität zukommt, haben die Komplikationsrate dieser Verletzungen deutlich reduziert. Hierbei ist entscheidend, daß beide Komponenten – Sanierung der Weichteile und Stabilisierung der Fraktur – zeitgerecht aufeinander abgestimmt sind.
