    Description / Table of Contents: Zusammenfassung In einer prospektiven randomisierten Studie wurde die Wirksamkeit der kontinuierlichen intraarteriellen Cytostatica-Therapie (FUDR) über eine implantierbare Pumpe (n =19) mit der Bolusinjektion (n= 17) verglichen bei isolierten nicht resektablen Lebermetastasen. Es fand sich folgendes Ergebnis: Ansprechrate bei den Pumpen 58% vs. 41% bei den Ports. Progression bei kontinuierlicher Infusion 10,5% vs. 29,5% bei Bolusinjektion. Die Toxicität war bei kontinuierlicher Cytostatica-Applikation mit 58% gegenüber 23,5% deutlich erhöht.
    Notes: Summary In a prospective randomized study, the effectiveness of continuous intra-arterial cytostatics therapy (FUDR) via an implantable pump (n=19) on isolated, nonresectable liver metastases was compared with bolus injection (n=17). The following results were found: there was a 58% response rate in the pump group vs 41% in the port group. Progression with continuous infusion was 10.5% vs 29.5% with bolus injection. The toxicity of continuous cytostatics application was distinctly higher (58% vs 23.5%).

    Description / Table of Contents: Zusammenfassung Die isolierte Leberperfusion bzw. intraarterielle Chemotherapie kann zwar bei Lebermetastasen colorectaler Carcinome beachtliche Erfolge erzielen; jedoch treten in his zu 60% der Fälle Lokalrezidive auf. Unser Ziel war es, ein Modell zur isolierten hochdosierten Cytostatica-Perfusion des Gastrointestinaltraktes zu erarbeiten. Die bisher an 58 Schweinen durchgeführten Untersuchungen zeigen die Durchführbarkeit der isolierten Perfusion des GI-Trakts mittels extracorporalem Kreislauf bei einem Leck von weniger als 4,5%. Die Aufnahme des Cytostaticums FUDR erfolgt in der Leber and im Intestinum. Die Maximumdosis ist 8 mg/kg KG.
    Notes: Summary Isolated liver perfusion and intra-arterial chemotherapy of liver metastases of colorectal carcinomas are considerably successful; however, local recurrences occur in up to 60% of all cases. Our aim was to work out a model of isolated perfusion of the gastrointestinal tract with high-dosage cytostatics. To date, research has been carried out on 58 pigs; the results demonstrate the feasibility of isolated perfusion of the GI tract by means of extracorporeal circulation. The leakage rate is than 4.5 %. The cytostatic agent FUDR is absorbed in. the liver and intestine (maximum dose 8 mg/kg body weight).
