    Notes: Abstract Rupture of tumors is usually a critical and life-threatening condition. We demonstrate a wide variety of ruptured tumors with their imaging characteristics including gastric lymphoma, gastric leiomyosarcoma, leiomyosarcoma of the ileum, hepatocellular carcinoma, pancreatic pseudocyst, renal angiomyolipoma, renal cell carcinoma, ovarian endometrial cyst, ovarian corpus luteum cyst, and ovarian teratoma. Their imaging features are illustrated with an emphasis on clues to an accurate diagnosis.

    Notes: Abstract We investigated metabolic changes in brain tumours following treatment, using proton magnetic resonance spectroscopy. In meningiomas, effective therapeutic embolisation led to an acute increase in lactate. In radiosensitive tumours such as malignant lymphoma, a decrease in lactate and in increase inN-acetyl-aspartate occurred after radiotherapy, which preceded changes observed on magnetic resonance imaging. On the other hand, no significant changes in spectral patterns were observed in malignant gliomas resistant to therapy. Tissue characterisation of brain tumours by spectral patterns on proton magnetic resonance spectroscopy remains controversial. However, we have shown it to be sensitive to metabolic changes following treatment, which may reflect the efficacy of the therapy.

    Notes: Abstract The details of brain metabolism in chronic cerebral infarcts have not been clarified. Using proton MR spectroscopy (1H-MRS) at 1.5 T, we measured biochemical changes in 16 patients with large infarcts involving the motor cortex in the chronic phase (median 293.9 days) and related the findings to clinical data. Localised spectra were obtained using point-resolved spectroscopy, with an echo time of 270 ms. Regions of interest were placed on the frontal lobe, including the precentral gyrus and central sulcus. Motor function was assessed by the manual muscle power test at the time of the 1H-MRS study. Only three patients with severe paresis had no signal in the lesions and a lactate signal was obtained in 13 cases. N -acetyl aspartate (NAA) was observed in 4 cases with recanalisation of an occluded vessel. Motor function correlated strongly with the NAA/choline-containing compounds (Cho) ratio (P 〈 0.01) and lactate/Cho ratio (P 〈 0.01). We found various metabolic patterns, reflecting residual neurological function.

    Notes: Abstract We retrospectively reviewed the MRI findings in 28 patients with an intramedullary spinal cord ependymoma who underwent surgical treatment. There were 26 tumours in the cervical and two in the thoracic spine. T1- and T2-weighted and contrast-enhanced images at 1.5 T were obtained in all cases. T1-weighted imaging showed solid tumour as isointense in 13 patients, high-signal in ten and low signal in five. In contrast, T2-weighted imaging showed all tumours as high signal. Contrast enhancement was heterogeneous 13 patients, homogeneous 10, heterogeneous with cyst wall enhancement in three, and a nodule on a cyst wall was seen in two. Cases with these latter patterns require careful differential diagnosis from astrocytoma or haemangioblastoma.

    Notes: Summary The clinical features of 31 cases of syringomyelic syndrome, confirmed by metrizamide CT myelography (CTM) or magnetic resonance imaging (MRI), were analysed. Bilateral suspended sensory disturbance, previously regarded as a characteristic feature, was encountered in only 2 cases (6.5%). Thirteen patients (41.9%) showed no motor deficits of the upper limbs and 7 patients (22.6%) did not have weakness or atrophy in any of the four extremities. Cervical segmental signs were absent in 7 cases (22.6%) and in 2 (6.5%) signs were only present in the lower limbs. Scoliosis and foot deformity, such as pes cavus or equinovarus, were seen in 17 (54.8%) and 5 cases (16.1%) respectively. These skeletal abnormalities were initial symptoms in some childhood cases. In syringomyelic syndrome, clinical features are variable with many atypical clinical manifestations, apart from the previously recognized characteristic clinical features.
