Zusammenfassung Unilateraler and v-förmiger Fixateur externe sind geeignete Osteosyntheseverfahren bei Unterschenkelfrakturen mit Weichteilschaden. Kettenfrakturen, Mehrfachverletzungen sowie Tibiafrakturen beim Polytrauma sind weitere Indikationen. Bleibt die knöcherne Heilung mit dem Fixateur externe in angemessener Zeit aus, kann der frühzeitige Verfahrenswechsel zur intramedullären Stabilisierung empfohlen werden. Von April 1983 bis December 1984 sind 49 Patienten mit geschlossenen bzw. erst- and zweitgradig offenen Tibiafrakturen derart behandelt worden; in 22 Fällen war ein Implantatwechsel zur intramedullaren Stabilisierung notwendig.

Summary External fixation is the method of choice in compound fractures of the lower leg. Additional indications are multiple fractures of one leg, as weel as in polytraumatized patients. Because of the rigidity of external fixation, in cases of delayed bone healing an early change of osteosynthesis towards intramedullary stabilization is recommended. From April 1983 to December 1984, external fixation was performed in 49 closed and first — or second — degree open fractures of the tibia. In 22 cases, secondary intramedullary nailing was done afterwards because of delayed bone healing.
