    Description / Table of Contents: Summary Clear cell acanthoma is a rare, slow-growing benign epidermal tumour of adulthood. The predilection site of this usually solitary lesion is the distal part of the leg. Multiple clear cell acanthomas are very rare and are also found in other locations besides the typical site of predilection. In this paper we report on a 52-year-old man who – over a period of more than 20 years – had developed more than 100 of these tumours in all parts of his body. Although the histological diagnosis is easy, clinical recognition often proves difficult, especially in cases of multiple lesions. Different kinds of therapy for solitary and multiple lesions are discussed.
    Notes: Zusammenfassung Das Klarzellakanthom ist ein seltener, langsam wachsender, gutartiger, epidermaler Tumor des Erwachsenenalters. Prädilektionsstelle des in der Regel solitär auftretenden Tumors ist der Unterschenkel. Multiple Klarzellakanthome sind sehr selten und finden sich dann auch außerhalb der typischen Prädilektionsstelle. Wir berichten über einen 52jährigen Patienten, der in einer Zeit von über 20 Jahren weit über hundert dieser Tumoren am gesamten Integument entwickelte. Während die histologische Diagnosestellung relativ einfach ist, wird der Tumor klinisch häufig nicht erkannt, insbesondere wenn multiple Läsionen vorliegen. Die Therapiemöglichkeiten bei solitären und multiplen Läsionen werden diskutiert.

    Notes: Abstract Treatment of the mustard (Sinapis alba L.) seedling with the herbicide SAN 9789 inhibits synthesis of colored carotenoids and interferes with the formation of plastid membrane lipids without affecting growth and morphogenesis significantly. In farred light, which is hardly absorbed by chlorophyll, development of plastid ultrastructure, synthesis of ribulosebisphosphate carboxylase and synthesis of chlorophyll are not affected by SAN 9789. It is concluded that normal phytochrome actions on plastid structural development, protein and chlorophyll syntheses are not affected by the absence of carotenoids provided that there is no significant light absorption in chlorophyll. The findings show that the inhibition of synthesis of one set of plastid membrane components (the carotenoids) does not stop synthesis of other components such as chlorophyll and does not halt membrane assembly. Supplementary experiments with the closely related compound SAN 9785, which affects the amount and composition of plastid lipids but not carotenoid and chlorophyll syntheses, suggest that the effect of the herbicide SAN 9789 is due exclusively to its inhibition of synthesis of colored carotenoids. In the presence of SAN 9789 white or red light at high fluence rate causes photodestruction of chlorophyll and ribulosebisphosphate carboxylase and photodecomposition of thylakoids. These effects are interpreted as resulting exclusively from the self-photooxidation and photosensitizing action of chlorophyll once the protection by carotenoids of chlorophyll against self- and sensitized photooxidation is lost.

    Notes: Abstract Carotenoid accumulation in the cotyledons of the mustard seedling (Sinapis alba L.) is controlled by light. Besides the stimulatory function of phytochrome in carotenogenesis the experiments reveal the significance of chlorophyll accumulation for the accumulation of larger amounts of acrotenoids. A specific blue light effect was not found. The data suggest that light exerts its control over carotenoid biogenesis through two separate mechanisms: A phytochrome regulation of enzyme levels before a postulated pool of free carotenoids, and a regulation by chlorophyll draining the pool by complex-formation.

    Description / Table of Contents: Abstract Background. In paediatrics, numerous diseases present with the leading symptome of muscular, articular, or bone-related pain. The pain as such is seldom diagnostic with regard to pain etiology. Regularly, the significance of inflammatory as well as non-inflammatory pain is underestimated. Classification of musculosceletal diseases. We present 4 case reports, illustrating the classification of musculosceletal diseases into 4 main groups, being the basis for the modelling of disease, and pain treatment. Therapy. Pain therapy is either symptomatic or based on specific pathophysiology. Pain therapy consists of the moduls analgesics, antiphlogistics, physiotherapy, psychosocial support, and complementary therapies. We give advice on differential therapy. A transparent team-oriented concept comprising physicians, physiotherapists, ergotherapists, psychologists, and social workers is the basis for any successful long-term therapy. Regular outpatient visits in consent with the family doctor are mandatory as are education and treatment periods on ward. We encourage our patients to join formal self-supportive patient groups.
    Notes: Zusammenfassung Hintergrund. Eine Vielfalt an Grunderkrankungen präsentiert sich im Kindesalter mit Schmerzen des Bewegungsapparats als Leit- oder Begleitsymptom. Die Schmerzäußerung allein lässt nur selten eine sichere ätiologische Differenzierung zu. Entzündliche sowie nichtentzündliche muskuloskelettale Beschwerden werden regelmäßig in ihrer Bedeutung unterschätzt. Hauptgruppen muskuloskelettaler Erkrankungen. Anhand von Fallbeispielen unterschiedlicher Schmerzcharakteristik und Begleitsymptomatik stellen wir 4 Hauptgruppen muskuloskelettaler Erkrankungen vor. Diese Einteilung ist Grundlage der Entwicklung von Krankheits- und Behandlungsmodellen des Schmerzes. Therapie. Die Schmerztherapie ist teils symptomatisch, teils pathophysiologisch begründet, und besteht aus den Therapiemodulen Analgetika, antiinflammatorische Therapie, Krankengymnastik, psychosoziale Begleitung und komplementäre Therapien. Hinweise für die Differenzialtherapie werden angegeben. Ein für alle Beteiligten transparentes teamorientiertes Konzept mit der Einbindung von Ärzten, Krankengymnasten, Ergotherapeuten, Psychologen und Sozialarbeitern hat sich als Grundlage einer erfolgreichen Langzeitbetreuung erwiesen. Wichtig sind regelmäßige ambulante Kontakte in Absprache mit dem Hausarzt, ggf. Schulungen und stationäre Aufenthalte. Die Zusammenarbeit mit Selbsthilfegruppen hat sich innerhalb eines solchen Therapieplans erfolgreich etabliert.

    Description / Table of Contents: Zusammenfassung Unverträglichkeit von Operationshandschuhen kann zur Berufsaufgabe führen. Zehn Patienten mit einer Kontaktallergie auf Gummihandschuhe wurden mit einer Serie von Handschuhen verschiedener chemischer Zusammensetzung epicutan getestet. Es zeigten sich große Unterschiede zwischen den einzelnen Produkten. Als Hauptsensibilisatoren konnten die Gummiacceleratoren der Thiuram- und Carbamatgruppe identifiziert werden. Alternativen für diese Patienten sind Operationshandschuhe aus reinem Latex, künstlichem Gummi (Styrol-Butadien) oder Gummihandschuhe mit interner Kunststoffbechichtung.
    Notes: Summary Intolerance of surgical gloves can lead to professional disability. Ten patients with contact allergy to rubber gloves were tested with a series of commercial gloves of different chemical compositions. Great differences were observed. Rubber accelerators of the thiram and carbamate group were identified as the main sensitizers. Alternatives for patients sensitive to these materials are gloves made from pure latex, artificial rubber (styrene-butadiene) or rubber gloves with an internal polyurethane coating.
