    Notes: Even though conventional X-ray polarizers with multiple-reflection channel-cut structures perform well, they suffer from the disadvantage that the polarized beam is non-stationary as the energy is scanned. In this paper the possibility of using a thin Bragg reflector as a polarizing filter is investigated, so that the transmitted beam (rather than the Bragg-reflected beam) is used. Consequently, the position and direction of the transmitted beam are unchanged as the energy is changed, or even when the polarization direction to be filtered out is changed. Theoretical considerations as well as experimental results on the performance characteristics of the proposed transmission-type X-ray linear polarizer are presented. A polarization ratio, defined as IH/IV, higher than 105 was obtained.

    Notes: The possibility of a transmission-type X-ray linear polarizer is investigated using a thin Bragg reflector as a polarizing filter. In this device, the transmitted beam (rather than the Bragg-reflected beam) is the useful output of the device. Consequently, the position and the direction of the transmitted beam are unchanged as the energy is changed, or even when the polarization direction to be filtered out is changed. Theoretical considerations as well as preliminary transmissivity measurements are presented. The use of perfect crystals of silicon and diamond is examined. A polarization ratio, defined as I_H/I_V, higher than 105 was observed in experiments to measure the performance characteristics of the proposed X-ray polarizing reflector. The transmission-type X-ray linear polarizer is well suited for spectroscopic measurements with polarized X-rays.

    Notes: Abstract A total of 20 patients with hormone-refractory prostate carcinoma entered a pilot study of combination chemotherapy based on the EAP (etoposide, Adriamycin and cisplatin) regimen, in which Adriamycin was replaced by pirarubicin, a less cardiotoxic derivative of Adriamycin. The response was assessed by criteria modified from those of the National Prostatic Cancer Project: prostate-specific antigen was employed instead of acid phosphatase. Of 18 evaluable patients, 6 achieved a partial response, 5 had stable disease, and in 7 the disease had progressed during therapy; thus, the overall response rate was 33.3% [95% confidence interval (CI) 11.5–55.1%]. Significant pain alleviation and performance status improvement were obtained in 5 of 12 patients (41.7%; CI 13.8–69.6%) and 3 of 13 patients (23.1%; CI 0.2–46.0%), respectively. Although myelosuppression was moderate to severe, no chemotherapy-related deaths or bacteriologically documented sepsis occurred; nor was there any clinical cardiotoxicity. All the responding patients received maintenance chemotherapy with etoposide thereafter. At present, the median duration of response is 33 weeks (range: 23–91 weeks) and the median survival period for all patients is 42 weeks (range: 27+ −136 weeks), with 12 deaths. In spite of the small number of patients treated, these results suggest that this chemotherapy regimen is active in advanced hormone-refractory prostate carcinoma.

    Notes: Abstract We used a recombinant retrovirus as one of the potential vectors for human gene therapy to transfer a drug sensitivity gene into human lung cancer cells. The gene encoding the thymidine kinase (TK) of herpes simplex virus type 1 (HSV1) was used as the drug sensitivity gene. The antiherpes drugs acyclovir (ACV) and ganciclovir (GCV) were chosen to test the HSV1-TK activity transferred into the human lung cancer cell lines. The rationale for this approach was that ACV and GCV are nucleoside analogs specifically converted by HSV1-TK to a toxic form capable of inhibiting DNA synthesis or disrupting cellular DNA replication. The results obtained from our experiments demonstrate that the retroviral vector-mediated HSV1-TK gene transfer leads to ACV- and GCV-dependent cytotoxicity in human lung cancer cell lines, including both small-cell carcinoma and nonsmall-cell carcinoma. Although the gene transfer of HSV1-TK gene into tumor cells would be one model for gene therapy to control lung cancer, further investigations are necessary for the proper choice of the therapeutic gene and vector targeting such as tumor cell specific delivery of the gene or tumor cell specific expression of the transduced gene.

    Notes: Abstract The contact pressure on 112 normal hip joints and 66 acetabular dysplastic hip joints was analysed using a three-dimensional rigid body spring model. Geometric models were made from conventional anteroposterior radiographs, with the assumption that the acetabular surface was spherical. In normal hips, the distribution of contact pressure was almost even over the joint surface, and the maximum contact pressure was relatively low. However, in a dysplastic hip joint, the pressure was concentrated on the anterolateral edge of the acetabulum and increased enormously not only with the reduction in lateral coverage but also with the reduction in the anterior coverage of the acetabulum. This result indicates that a three-dimensional analysis is indispensable for estimating the mechanical effect on acetabular cartilage.

    Notes: Abstract Recombinant human erythropoietin (rHuEPO) is effective in allowing autologous blood donation in patients unable to donate because of anemia. As adverse effects of rHuEPO might include activation of coagulation and possibly fibrinolysis, we investigated these possibilities in the context of autologous blood donation preceding hip surgery. Thirty-seven patients who donated 800 ml of autologous blood for elective hip surgery were randomly assigned to either a group of 20 receiving preoperative treatment with rHuEPO (erythropoietin β), 6000 U i.v. twice weekly for 3 weeks, or an untreated control group of 17. A significant increase in platelet count was associated with autologous blood donation and intraoperative blood loss with or without rHuEPO. Coagulation and fibrinolysis were increased significantly by intraoperative blood loss in both groups, but not by rHuEPO. Coagulation and fibrinolysis were not activated by rHuEPO for elective hip surgery.

    Notes: Abstract We investigated the results of 31 hips in 26 patients with nontraumtic (n = 20) and steroid-induced (n = 6) avascular necrosis of the femoral head (ANFH) treated with vascularized iliac pedicle bone graft (PBG). The average age at operation was 38.3 years. Three were women and 23 men. The average follow-up was 8.0 years. The Harris hip score prior to operation and at latest follow-up improved from 62 to 83; one hip collapsed and was revised with a bipolar endoprosthesis. At the final follow-up, 19 hips (63%) were clinically rated as good to excellent, 4 fair, and 7 poor. At the final follow-up, 15 of 27 hips (56%) of stage II before operation showed progressive collapse after bone grafting. In steroid-induced ANFH, in three women, 2 of 4 hips showed poor results. These results are only slightly better than those of core decompression and no better than those obtained after decompression and simple nonvascularized grafts to provide support for the subchondral bone. We concluded that vascularized PBG is sometimes indicated for ANFH in an early stage before collapse of the femoral head.

    Notes: Abstract We reviewed 48 hips in 43 patients 3–7.1 years (average 4.6 years) after Sugioka transtrochanteric rotational osteotomy for osteonecrosis of the femoral head. The average age at operation was 41 years. Thirty-four patients were men and 9 women. Overall results at the final follow-up were satisfactory in 30 hips (62%). Kaplan-Meier's survivorship was 62% at 3 years and 60% at 5 years postoperatively. Six hips for which the ratio of the intact area of the articular surface on the preoperative lateral radiograph was less than 30% showed further collapse. Five hips were converted to bipolar hemiarthroplasties or total hip arthroplasties. Complications, such as varus deformity, subtrochanteric fracture, and ectopic bone formation, occurred in eight hips. Five of them were operated on in the first 2 years of this series. Three of these five operations had unsatisfactory results. We conclude that satisfactory results can be achieved using this osteotomy by maintaining exact surgical technique and by limiting the surgical indications to hips with an intact area of more than one-third of the entire articular surface on the lateral radiograph of the femoral head.

    Notes: Abstract We performed magnetic resonance imaging (MRI) in osteoarthrosis (OA) of 35 dysplastic hips in 28 patients using a 1.5-Tesla superconductive magnet. MR images were compared with conventional radiographs and arthrograms. MRI demonstrated cartilage irregularity and joint space narrowing in 17 hips earlier than did radiographs. Acetabular labrum tears were observed in 24 hips by MRI and in 23 hips by arthrography, suggesting that the former may be as useful as the latter. Osteophytes, joint effusion and synovial proliferation can be identified better by MRI than by conventional radiography. An MR grading system was established according to the thickness of the cartilage and sclerosis of the acetabulum or femoral head. This system can be a powerful tool to recognize the early stage of OA. MRI can demonstrate directly the severity of OA, including early degeneration of articular cartilage and acetabular labrum tear.
