    Notizen: Aim : To estimate the cost-effectiveness of therapy and analyse the effect of therapy compliance in naive patients with chronic hepatitis C.Methods : A decision analysis using the Markov model was performed for four different therapeutic strategies using peginterferon alfa-2b plus ribavirin or interferon alfa-2b plus ribavirin. Clinical data were obtained from available published reports and from the Spanish health system perspective.Results : The incremental cost-effectiveness ratio of peginterferon alfa-2b plus ribavirin at a fixed dose, compared with interferon alfa-2b plus ribavirin, was 8478 euros per life year saved and 3737 euros per quality-adjusted life year gained. Good therapeutic compliance and weight-adjusted doses of ribavirin decreased the incremental cost-effectiveness ratio to 1636 euros per life year saved and 721 euros per quality-adjusted life year gained. In compliant genotype 1 patients, the incremental cost-effectiveness ratio decreased to 916 euros per life year saved and 404 euros per quality-adjusted life year gained, with an increase from 64 to 69 years in the threshold age at which therapy was cost-effective. The sensitivity analysis demonstrated that changes in the values of the most relevant parameters do not modify the study outcomes.Conclusion : From the clinical and pharmaco-economics perspective, the use of decision therapeutic analysis models suggests that the most effective therapy for chronic hepatitis C is peginterferon alfa-2b plus ribavirin adjusted to patient body weight and with good compliance, particularly in genotyped patients.

    Notizen: Background : Lamivudine therapy for chronic hepatitis B has been associated with changes in different regions of the hepatitis B virus nucleotide sequence.Aim : To study changes in the sequences of polymerase and precore/core promoter regions of hepatitis B virus, before and during 5 years of therapy with lamivudine.Methods : Eighty consecutive samples were taken from 10 chronic hepatitis B ‘e’ antigen-negative patients.Results : Nine patients carried hepatitis B virus precore mutations during the study. Before therapy, wild type was replaced by A1896 in two (20%) cases. During treatment, A1896 reverted transitory to wild type in five cases (50%) and in one case wild type was replaced by A1896. The continuous detection of precore mutations during therapy was associated with a lower response rate.YMDD mutations were observed in nine cases and both, L180M and M204V/I mutations were simultaneously detected in six cases. About 75% of the patients with M204V mutations were responders and none with M204I or mixed pattern sustained response.Conclusion : Hepatitis B ‘e’ antigen-negative patients exhibit changes in the precore regions both spontaneously and under lamivudine therapy, the transitory reversion to wild type being most frequently witnessed. Patients carrying M204V mutations are more likely to respond to therapy. If, in further studies, these results are confirmed some patients with YMDD mutations could benefit from prolonging the duration of lamivudine therapy.

    Notizen: Abstract Bacillus circulans WL-12 secretes 1,4-β-d-xylanase and 1,3-β-d- and 1,6-β-d-glucanase activities. All of them are catabolites regulated by glucose and, while xylanase needs xylan as the inducer, the two latter enzyme activities are formed once glucose is depleted. Cyclic nucleotides such as adenosine 3′,5′-monophosphate (cAMP) and guanosine 3′,5′ monophosphate (cGMP) exhibit a negative effect on enzyme synthesis if added to the culture media. Based on the fact that only cAMP is found in cells growing in glucose-rich media we propose a model for B. circulans WL-12 in which cAMP acts as a negative effector for regulating the synthesis of these enzymes. The model is not, however, extrapolated to other Bacillus species and all B. circulans strains.

    Notizen: Differences in P Wave Filtering Techniques. introduction: The advent of signal-averaged ECG (SAECG) systems for P wave analysis has made it important to determine if the use of different filtering techniques in these systems is diagnostically equivalent. Methods and Results: Three different high-pass filtering techniques and two cutoff frequency values were used: 29- and 40-Hz Butterworth bidirectional filter (BB29, BB40), 29-and 40-Hz Butterworth unidirectional filter (UB29, UB40), and 29- and 40-Hz least mean square filter (LIV1S29, LMS40). Normal healthy volunteers (n = 36) and patients with documented paroxysmal atrial fibrillation (n = 23) were analyzed. A custom-built SAECG system and standard bipolar orthogonal leads were used. Noise was reduced to 〈 0.3 μV. P wave total duration, root mean square voltage of the terminal 20, 30, and 40 msec of the filtered vector magnitude, and the area under the curve between the onset and offset of averaged unfiltered and filtered P wave vector magnitude were analyzed. Only the duration of the P wave showed statistically significant differences between groups, being longer in the PAF group for all filters and cutoff frequencies studied. A bias increment of -20 msec was detected in unidirectional and least mean square filters as compared to the bidirectional filter. Sensitivity, specificity, and predictive accuracy were 〉 70% for all filters; the BB40 filter yielded the best performance. Conclusion: The normality limits derived from one filter cannot be applied directly to recordings obtained from the other filters. Critical limits must be established individually for different software settings.

    Notizen: Introduction: Repolarization heterogeneity has been shown to constitute a substrate for malignant ventricular arrhythmias. Noninvasive measurement of abnormal repolarization through assessment of QT interval dispersion from the resting standard 12-lead ECG initially had shown promise in assessing arrhythmia risk but was challenged recently. The relative T wave residuum (TWR) has been proposed recently to reflect regional repolarization dispersion more accurately. We analyzed the role played by the dipolar and nondipolar components in determining TWR. Methods and Results: Singular value decomposition was applied to the repolarization signals obtained from isolated rabbit hearts using a 5 × 8 array multielectrode recording system during premature beats (N = 11) and after d-sotalol (N = 9) exposure. Both the dipolar and nondipolar components of the T wave increased significantly during premature stimulation and after d-sotalol exposure. The relative TWR decreased significantly during premature stimulation but did not change after d-sotalol. Changes in the dipolar and nondipolar components of the second half of the T wave were significantly greater than those corresponding to the first half during premature stimulation, and a significant correlation was observed between the nondipolar components of the second half of the T wave and the Tpeak–end interval. Conclusion: Conditions exist during which both the dipolar and nondipolar components can change simultaneously. Under these conditions, the relative TWR may not reflect regional heterogeneity of repolarization with accuracy. The nondipolar components of the second half of the T wave can be linked to assessment of the transmural dispersion of repolarization. (J Cardiovasc Electrophysiol, Vol. 15, pp. 356-363, March 2004)

    Notizen: Background : Norfloxacin decreases the incidence of spontaneous bacterial peritonitis in cirrhotics, but promotes the appearance of quinolone-resistant Escherichia coli.Aim : To define the characteristics of quinolone-resistant E. coli spontaneous bacterial peritonitis.Methods : E. coli-positive ascitic fluid cultures were identified during a 6-year period. Data on quinolone-sensitive and quinolone-resistant E. coli spontaneous bacterial peritonitis were compared.Results : One hundred and two E. coli-positive ascitic fluid cultures were detected. Cirrhotics accounted for 67 cases. Spontaneous bacterial peritonitis was found in 47 of the 67 (70%) cases [35 (74%) caused by quinolone-sensitive and 12 (26%) caused by quinolone-resistant E. coli]. Norfloxacin prophylaxis was higher in the quinolone-resistant group (92% vs. 6%, P 〈 0.001). Compared with patients with quinolone-sensitive E. coli spontaneous bacterial peritonitis, those with quinolone-resistant E. coli spontaneous bacterial peritonitis showed a higher prevalence of associated immunosuppressive factors (immunosuppressive drugs, human immunodeficiency virus infection or cancer) (92% vs. 20%, P 〈 0.001). Steroid therapy was independently associated with quinolone-resistant E. coli spontaneous bacterial peritonitis (odds ratio, 49; 95% confidence interval, 3.4–699; P = 0.004). The Child–Pugh score (P = 0.03), immunosuppression (P = 0.02) and renal failure (P = 0.01) were independent predictors of E. coli spontaneous bacterial peritonitis-related mortality.Conclusions : Associated immunosuppression is an important co-factor for the development of quinolone-resistant E. coli spontaneous bacterial peritonitis and for E. coli spontaneous bacterial peritonitis-related mortality.
