    Notes: Summary  We report two cases of glossopharyngeal neuralgia associated with a vascular loop of the postero-inferior cerebellar artery, diagnosed by magnetic resonance imaging. Reviewing the literature, we found this to be the first report of a magnetic resonance-validated vascular abnormality related to such a condition. One patient was cured by surgical decompression, confirming the role of the abnormal vessel in the pain. As with trigeminal neuralgia, a possible vascular aetiology should be considered in glossopharyngeal neuralgia.

    Notes: Summary ¶ Background. We present the results of 100 consecutive magnetic resonance (MR)-guided biopsies in cases where computerised tomography (CT) guiding was considered dangerous or impossible. Method. MR guiding was preferred to CT guiding for cases where lesions were located in the central area, or were not clearly visible on CT scan, or where the visualization of vessels was considered necessary. For most of the patients, calculation of target co-ordinates was performed using dedicated software enabling trajectory previsualization. There were 62 cases of contrast enhanced lesions, 32 cases of lesions without contrast enhancement, and 6 cases of very small lesions appearing hyperintense on T2-weighted images. Findings. Biopsies allowed a histological diagnosis in 92 cases. In 8 cases, the biopsy was negative (necrosis, gliosis or normal brain tissue). Three patients had a transient worsening of their neurological disturbances. Two patient had a non-regressive loss of motor function. No patient died. Interpretation. MR guiding for stereotactic biopsies was effective for CT-invisible or ill-defined lesions, lesions located in functional or densely vascularized areas and in the brain stem. The rate of postoperative complications was equivalent to or less than that reported in series of CT-guided biopsies.

    Notes: Summary Ten patients with clinical tuberous sclerosis were examined with CT and MR imaging, before and after IV contrast in order to determine the role of Gd-DTPA. Gd-DTPA enhancement occured in eleven subependymal nodules which did not enhance on CT after IV contrast. As illustrated by previous CT and pathologic observations and related to the histologic similarity of the subependymal nodules and giant-cell astrocytomas, these hyperintense nodules could represent active lesions with the potential to evolve. Four giant-cell astrocytomas were detected both with CT and Gd-DTPA-enhanced MRI; tumor conspicuity and size assessment were improved by postcontrast MRI in two cases. No cortical tuber or heterotopic cluster enhanced; T2-weighted sequences therefore remain necessary for their detection. If pre and post-Gd-DTPA T1-and T2-weighted imaging is negative, CT is clearly the most sensitive modality in the detection of the small calcified subependymal nodules.

    Notes: Abstract In a 23-year-old woman CT demonstrated a patent giant intracranial aneurysm. MRI, CT and angiography one month later demonstrated complete spontaneous thrombosis of the aneurysm. The diagnosis was confirmed surgically and pathologically. This case demonstrates that a normal angiogram can be associated with a thrombosed giant aneurysm, and that this diagnosis should be considered in cases of angiographically-occult cerebral vascular malformations.

    Notes: Abstract We report a retrospective analysis of MRI in 206 patients with intractable seizures and describe the findings in bilateral mesial temporal sclerosis (MTS) on fast spin-echo (FSE) and fast fluid-attenuated inversion-recovery (fFLAIR) sequences. Criteria for MTS were atrophy, signal change and loss of the digitations of the head of the hippocampus. In patients with bilateral MRI signs of MTS, correlation with clinical electro, volumetric MRI data and neuropsychological tests, when available, was performed. Bilateral MTS was observed in seven patients. Bilateral loss of the digitations and signal change on fFLAIR was seen in all seven. In three, bilateral atrophy was obvious. In two patients, mild bilateral atrophy was observed and in two others, the hippocampi were: asymmetrical, with obvious atrophy on only one side. Volumetric data confirmed bilateral symmetrical atrophy in five patients, and volumes were at the lowest of the normal range in the other two. The EEG showed temporal abnormalities in all patients, unilateral in five and bilateral in two. All patients had memory impairment and neuropsychological data confirmed visual and verbal memory deficits; two patients failed the Wada test on both sides. High-resolution T2-weighted FSE and fFLAIR sequences allow diagnosis of bilateral MTS, which has important therapeutic and prognostic implications.

    Notes: Abstract We evaluated the feasibility and use of diffusion-weighted and fluid-attenuated inversion-recovery pulse sequences performed as an emergency for patients with acute ischaemic stroke. A 5-min MRI session was designed as an emergency diagnostic procedure for patients admitted with suspected acute ischaemic stroke. We reviewed routine clinical implementation of the procedure, and its sensitivity and specificity for acute ischaemic stroke over the first 8 months. We imaged 91 patients (80 min to 48 h following the onset of stroke). Clinical deficit had resolved in less than 3 h in 15 patients, and the remaining 76 were classified as stroke (59) or stroke-like (17) after hospital discharge. Sensitivity of MRI for acute ischaemic stroke was 98 %, specificity 100 %. MRI provided an immediate and accurate picture of the number, site, size and age of ischaemic lesions in stroke and simplified diagnosis in stroke-like episodes. The feasibility and high diagnostic accuracy of emergency MRI in acute stroke strongly support its routine use in a stroke centre.
