    Notes: Summary Fifty-one patients with medulloblastoma have been treated at the Children's Memorial Hospital during the past 11 years. The ratio between males and females was 2∶1. The ages ranged from 4 months to 12 years; 7 patients were less than 12 months old at the time of diagnosis. After 1969, prior to definitive surgery, all patients were given a ventriculo-peritoneal shunt to decompress the hydrocephalus which was invariably present. There were no cases of systemic metastasis related to these shunts. Subsequently, posterior fossa craniotomy was performed, with total or radical resection of tumour in 13 cases, subtotal resection in 20 cases, and partial resection or biopsy in 14 cases. After completion of surgical treatment and radiotherapy, the three-year survival rate was 45.6 per cent and the five-year survival 34.5 per cent. The survival of children given total resection of the tumour was significantly longer, and females survived longer than males. Local recurrence and spinal cord or systemic metastases are discussed. Reexploration of the posterior fossa for recurrences failed to improve the prognosis. Recurrent or metastatic medulloblastoma should be treated by radiotherapy or chemotherapy, or both.

    Notes: Abstract Experiments with heterografts of human ovarian carcinoma (10 patients) in diffusion chambers revealed considerable individual differences in their sensitivity to chemotherapeutic agents. In four of five cases in which it was possible to compare the experimental results with the results of treatment of the patients with the same agents, correlation was found between the experimental and clinical findings.

    Notes: Summary A combination of MFC (Mitomycin-C, 5-FU and Cytosine arabinoside) was administered to 32 gastric cancer patients and study was made of the therapeutic efficacy in relation to the histological pattern. In evaluation based on the degree of cancer cell infiltration (INF), chemotherapy was more effective in cases with INF α and β than those with INF γ. As for the grade of follicular hyperplasia in regional lymph nodes of gastric cancer, the higher was the grade, the more effective was the chemotherapy. However, a significant relationship could not be observed between histological pattern of stromal reaction of the gastric cancer and clinical effects of chemotherapy. The grade of lymphocyte infiltration or connective tissue reaction in the stroma was not related with the therapeutic effects. No relationship was also obtained between the grade of sinus histiocytosis of lymphoid reaction in the lymph nodes and therapeutic effects.

    Notes: We have synthesized several potential inhibitors and/or modifiers of the carbohydrate portion of plasma membrane glycoconjugates. These include fluorinated and actylated analogs of D-glucosamine, D-galactosamine, and D-mannosamine. These compounds have been tested to determine their effects on both [14C] glucosamine and [3H] leucine incorporation into glycoconjugate and on cell growth and viability using P-288 murine lymphoma cells maintained in tissue culture. The most cytotoxic agent tested was 2-acetamido-2-deoxy-1,3,4,6-tetra-O-acetyl-β-D-glucopyranose or simply β-pentaacetylglucosamine which prevented cell growth at 10-4-10-3 M. β-Pentaacetylglucosamine cytotoxicity was correlated with its high lipid solubility, having an octanol/water partition coefficient of 0.424 as compared with 0.278 for the β-anomer and 0.017 for N-acetylglucosamine. In vitro metabolism studies with [14C]-and/or [3H]-labeled pentaacetylglucosamine have indicated intracellular de-O-acetylation leading to the biosynthesis of UDP-N-acetylglucosamine, followed by the incorporation of this sugar into cellular glycoprotein. Concomitant with the formation of increased amounts of this nucleotide sugar, intracellular UTP and CTP pools fell to one third normal within 3 h after the administration of 1 mM pentaacetylglucosamine. At present it is unclear whether the cytotoxicity of β-pentaacetylglucosamine or other similar agents is due to alterations in nucleotide and nucleotide-sugar pools causing a decrease in energy charge and polynucleotide biosynthesis or is due to a direct effect on membrane glycoconjugate biosynthesis.

    Notes: Abstract Experiments with explants of human melanomas cultivated in diffusion chambers in the peritoneal cavity of mice (10 cases) and explants into the retrobuccal pouches of golden hamsters (5 cases) revealed marked individual differences in the sensitivity of these tumors to combined treatment with three therapeutic preparations: vincristine (or vinblastine), nitrosomethylurea, and actinomycin D. Comparison of the results obtained in these two series of investigations showed agreement in three of five cases. In two cases the positive result obtained in the experiments with diffusion chambers corresponded to a negative result in the experiments on hamsters.

    Notes: Abstract A new operative technique for esophagocardial cancer was reported with resection and re-establishment of alimentary continuity at cervical esophagus which obviates thoracotomy but still retains adequate removal of the lesion. Ten cases of esophagocardial cancer thus treated were presented and the clinical data were analyzed. The distance between esophageal stump and esophagocardial junction measured on the resected specimen ranged from 3.0 to 6.7 cm with an average of 5.1 cm. No remaining tumor cells were detected microscopically on the oral cut margin of the specimen in all cases. To compensate for the inadequated lymph node dissection intensive postoperative adjuvant irradiation and/or chemotherapy was advocated.
