Primare Letalität 4,2%. — Bei den Komplikationen, nämlich Perforation, divertikulitischer Tumor mit Stenose, Striktur oder Ileus, Fistelbildung, Blutung und Kombination mit einem Cacinom, stehen als operative Maßnahmen zur Diskussion: Die Übernähung, Vorlagerung, Drainage mit oder ohne entlastendem Transversumanus, das ein- oder mehrzeitige resecierende Vorgehen. — Das operativ-taktische Vorgehen bei den einzelnen Komplikationen wird erörtert.
    Notes: Summary 73 patients with sigmoid diverticulitis and complications of this condition have been treated by operation during the past five years. Mortality was 4.2%. — The complications we observed were: perforation, localized abscess, obstruction, bleeding, fistulation, and carcinoma, and they were treated by: local repair, drainage, proximal colostomy, exteriorization of involved segment, primary or secondary resection. The treatment methods for the different complications are discussed.

    Description / Table of Contents: Zusammenfassung 190 an der Chirurgischen Universitätsklinik Köln durch Anastomosennachresektion und Vagotomie behandelte Kranke mit peptischen Rezidivulcera wiesen eine Komplikationsrate von 18,9% and eine Operationsletalität von 8,4% auf. Infolge besonderer Situationen wurde bei 27 Patienten mit zum Teil großen, penetrierenden Geschwüren auf die Nachresektion verzichtet and lediglich eine selektive Vagotomie vorgenommen. Sämtliche belassenen Ulcera heilten innerhalb von 2 Wochen ab und die Salzsäureproduktion wurde für die BAO um 78,3%, die PAO um 66,2% reduziert. Ernsthafte Komplikationen, Todesfälle oder erneute Rezidive warden 6 Monate bis 5 Jahre postoperativ nicht beobachtet.
    Notes: Summary 190 patients suffering from recurrent ulcers after gastrectomy were treated in the surgical department of the University of Cologne by vagotomy and resection of the anastomosis; complications occurred in 18.9% and lethality was 8.4%. Due to particular circumstances, 27 patients, some of whom had large, penetrating ulcers, received only selective vagotomy without resection of the anastomosis. All ulcers healed within 2 weeks and the gastric acid output was reduced by 78.3% for BAO and 66.2% for PAO. There were no severe complications and no fatalities. Over a follow-up period of 6 months to 5 years no further recurrence of ulcers has been seen.

    Description / Table of Contents: Zusammenfassung Die in den letzten Jahren rasch ansteigende Zahl implantierter Hüfttotalendoprothesen führt zahlenmäßig immer häufiger zu primären und sekundären Komplikationen. Bei der Beurteilung kommt dem Röntgenbild eine besondere Bedeutung zu. An Hand von Beispielen werden die verschiedenen Komplikationen: mechanisch bedingte Lockerung, der bakterielle Infekt, die sterile Spätentzündung und Weichteilossifikationen, analysiert und gegeneinander abgegrenzt. Die differenzierte Beurteilung der Röntgenbefunde wird unter Berücksichtigung biomechanischer Verhältnisse in den klinischen Zusammenhang gestellt.
    Notes: Summary In recent years the implantation of total hip protheses has become more and more popular and has led to an increasing number of primary and secondary complications. X-ray examination is very important for definition of the complications, which are analyzed and differentiated on the basis of several examples. We discuss primary and secondary malpositioning, mechanical loosening, infections, so-called sterilized late inflammation, and ossification around the protheses. A specialized analysis of the X-ray pictures is evaluated in connection with the clinical observations with reference to the biomechanics of total hip protheses.

    Description / Table of Contents: Zusammenfassung 1. Der Simultaneingriff setzt die gemeinsame Abwägung von Dringlichkeit des Befundes und Operationstoleranz des Kranken voraus. 2. Erstere wird durch die beiden Befundgruppen der kurativen Indikation sowie der präventiven und der diagnostischen Indikation bestimmt. Hier hat sich die Berücksichtigung einer pragmatischen Abstufung bewährt. 3. Das zweite Kriterium ist die Operationstoleranz des Operierten. Sie wird letztlich bestimmt durch die Schwere der Erst- als auch des simultanen Zweiteingriffs; in erster Linie aber der Dauer des Eingriffs, des Indikationsgrades und der Lokalisation der Operationsfelder und schließlich nicht weniger vom Alter und den Begleitleiden des Operierten. 4. Die Eingriffsschwere läßt sich am modifizierten Schema der Schockklassifikation von Moore [15] ablesen. 5. Der Abschätzung der allgemeinen Disposition des Kranken dient die Einteilung der Operationsgrade entsprechend der von der Internationalen Gesellschaft für Anästhesiologie angegebenen Abstufung. 6. Engpässe der Indikation gibt es beim Zusammentreffen belastender Erst- und Zweitoperationen gleicher Dringlichkeit mit schlechter Ausgangssituation des Kranken. 7. Vermeidbar sind solche Notsituationen durch eine auf die unspezifische Symptomatik der Baucherkrankung rücksichtnehmende, breit angelegte präoperative Diagnostik. Für polytraumatische Abdominalbefunde sind Präventivempfehlungen nicht möglich. 8. Oberste Leitregel für den simultanen Zweiteingriff ist es, daß er nur angeraten und gerechtfertigt erscheint, wenn er unter den gleichen Sicherheitskriterien ausgeführt wird, wie sie für den Ersteingriff galten.
    Notes: Summary From the point of view of indication as well as operative technic the combined surgical procedure is defined as a separate secondary surgical operation. This analysis, based on 4128 patients (own cases and reports from the literature) concerns the special problems which accompany this procedure. Since there will be a rise of the risk factors in operations with an extended shock-stress (stomach and large intestine) one will have to take the following into consideration: 1. Assessment of the complications of both surgical procedures. 2. The separate risks of infection and operative trauma. 3. The duration of the operation. 4. The situation before the procedure (general health, age, accompanying illness). The main rule to be followed in combined surgical procedures is that one only undertakes such risks if the criteria are the same as for the single operation.

    Notes: Summary In isolated human fat cells of the greater omentum and the mammary gland, the effect of glucose, fructose, and/or insulin was tested on the re-esterification rate of FFA measured by the balance method. It could be shown that in the absence of glucose no re-esterification activity was demonstrable. Glucose alone or fructose alone stimulated the re-esterification of FFA dose-dependently in isolated fat cells of the greater omentum, and to a minor degree in fat cells of the mammary gland. Insulin had no effect on the re-esterification rate of FFA in the presence or absence of glucose or fructose, whereas it significantly stimulated the incorporation of glucose-C into CO2 and lipids. It is concluded that the re-esterification of FFA in human adipose tissue, at least in vitro, is mainly controlled by glucose without need for insulin.

    Notes: Summary Intestinal absorption of glyoeryl tri-[1-14C]-oleate and [1-14C]-oleic acid, measured by serial determinations of blood radioactivity after oral administration of the compounds in peanut oil, was significantly greater in non-ketotic diabetic Chinese hamsters than in nondiabetic controls. Incorporation of [1-14C]-oleic acid into tissue lipids by jejunal slices in vitro was equal in both groups on a unit tissue weight basis. The data suggest that intraluminal hydrolysis does not differ between diabetic and non-diabetic animals. Overall uptake of free fatty acids may be enhanced in diabetics as a consequence of increased small intestinal mass.

    Notes: Summary The levels of insulin antagonism exhibited by human plasma albumin samples extracted by the alcoholic-trichloroacetic acid (TCA) method were found to vary over a wide range. Similar variations were found in the chlorine contents of these albumin samples, thus indicating a variation in TCA content. Chlorine content correlated with the levels of antagnonism exhibited by the various samples. Both chlorine levels and antagonistic activity were reduced appreciably by chromatography with Dowex 50W ion exchange resin. When Dowex treated (nonantagonistic) albumin was dissolved in water and re-extracted by the TCA-ethanol method, the resulting albumin preparations were high in chlorine content and highly antagonistic toward insulin. Nonantagonistic albumin was rendered antagonistic by the addition of TCA. The levels of antagonism exhibited by the TCA treated albumin preparations correlated with their chlorine contents. It is concluded that at least a portion of the insulin inhibitory effects exhibited by albumin preparations isolated by the TCA-ethanol method is due to TCA which is bound to the protein.
