Abstract Erdheim-Chester disease is a distinctive pathological radiographic entity, most commonly characterised by a typical skeletal involvement resulting from histiocytic infiltration of the bone marrow. Although lung involvement is often absent, we report a case of diffuse infiltrative lung disease characterised by a wide-spread reticular pattern on high-resolution CT scans associated with cystic airspaces and mild honeycombing observed throughout both lungs. Interstitial infiltration with lipid-laden histiocytes was histologically confirmed by means of transbronchial biopsy.
