Zusammenfassung In einer prospektiven konsekutiven Studie bei 80 Patienten mit analer Fistelerkrankung war feststellbar, daß das morphologische Bild dieser Erkrankung dem Bau- und Verteilungsmuster der Proktoanaldrüsen entspricht. In 22,5 % war die chronische Analfissur die auslösende Ursache eines Fistelleidens. Bei einer primären Heilungsrate von 95% nach kompletter Gangspaltung zeigte sich eine Kontinenzschwäche, wenn der innere Sphincter mehr als 2,5 cm und der äußere Sphincter mehr als 1.5 cm Tiefe durchtrennt werden mußte.

Summary In a prospective consecutive study in 80 patients with anal fistula disease it was established that the morphological picture corresponded to the structure and distribution of anal glands. In 22.5% chronic anal fissure was the leading cause of the anal fistula. By completely dividing up the fistulous tracts we were able to achieve a primary healing rate of 95%. Anal continence was reduced if the internal sphincter was cut more than 2.5 cm and the external sphincter more than 1.5 cm.
