Zusammenfassung Von 1977–84 wurden 134 Patienten mit nekrotisierender Pankreatitis im Rahmen einer prospektiven klinischen Studie operiert. Präoperativ auftretende pulmonale oder renale Organinsuffizienz oder Schock oder Sepsis und die intraoperativ erfaßten morphologischen Morbiditätsfaktoren: Parenchymnekroseausmaß über 30%, ausgedehnte peripankreatische Nekrosen und Ascites sowie insbesondere die bakterielle Kontamination der Nekrosen (bei 41% der Patienten) korrelierten direkt mit Letalität und Prognose. Ein Nachweis dieser genannten Morbiditätsfaktoren im Krankheitsverlauf eines Patienten mit nekrotisierender Pankreatitis signalisiert demnach eine dringliche Operationsindikation.

Summary From 1977 to 1984 data from 134 operations on patients with necrotizing pancreatitis were recorded in a prospective trial. Preoperative organ insufficiency (pulmonary or renal), the presence of shock or sepsis and the following intraoperatively documented morphological morbidity factors: parenchymal necrosis 〉 30%, peripancreatic necrosis, ascites, and (in particular) bacterial contamination of pancreatic necroses (present in 41% of patients) were directly correlated with prognosis and mortality. The occurrence of these morbidity factors in a patient with necrotizing pancreatitis consequently signifies an urgent indication for operation.
