Zusammenfassung Chirurgische Standardtherapie des Magencarcinoms ist heute Gastrektomie und systematische Lymphknoten-Dissektion. Trotz nachweisbarer Verbesserung des Langzeitergebnisses hinsichtlich Tumorlokalisation, Infiltrationstiefe, LK-Status und histologischer Tumortyp nach Lauren bleibt das Stadium der Erkrankung übergeordnete Determinante des Spätergebnisses. Verbindliche Empfehlungen für die Behandlung des Kardiacarcinoms sind z. Z. noch nicht möglich. Trotz immunologischer und klinischer Vorteile der Milzerhaltung für das Frühergebnis ist die Bedeutung für das Spätergebnis unklar.

Summary Procedure of choice in the treatment of gastric cancer is total gastrectomy and radical lymphnodectomy. Despite an improvement of long-term results concerning localization, depth of infiltration, lymphnodemetastasis and histological type of tumor according to Lauren the main determinant of late results is the stage of disease. Suggestions for treatment of cancers on the oesophagogastric junction cannot yet be given. Despite possible advantages of preservation of the spleen for the early result, the impact on late results remains unclear.
