Summary 1. By constant infusion of 9,10-H3-palmitic acid as an albumin complex and by separation of the main lipid fractions by means of thin layer chromatography the turnover rates of free fatty acids (FFA) and triglycerides (TG) in plasma, the plasma TG clearence and the plasma TG turnover time were estimated in 9 normal persons, 5 diabetics, and 6 patients with essential hyperlipemia. 2. A positive correlation was found between the concentration of FFA in plasma and TG influx (p〈0,01). This was found both in normal persons and in patients with symptomatic as well as essential hyperlipemia. In patients with essential hyperlipemia the concentration of FFA was elevated (p〈0,02) and the incorporation of FFA into plasma TG was increased (p〈0,05), but did not differ from diabetics with the same elevation of plasma FFA. The results show, that in patients with essential hyperlipemia the increased influx of TG cannot be responsible for their extremely elevated TG level. 3. In normals and patients with diabetes a positive correlation was found between TG influx and the log of TG concentration in plasma. This does not apply, however, to patients with essential hyperlipemia. In these patients TG clearence is reduced to about one fifth and plasma TG turnover time is prolonged about tenfold. It is suggested that an impaired efflux of plasma TG is responsible for the high TG level in patients with carbohydrate induced hyperlipemia.

Zusammenfassung 1. Durch konstante Infusion von 9,10-H3-Palmitinsäure und Auftrennung der Hauptlipidfraktionen durch Dünnschichtchromatographie wurden Umsatzraten der freien Fettsäuren und der Plasmatriglyceride sowie Plasmatriglycerid-Clearence und Plasmatriglycerid-Umsatzzeit bei Stoffwechselgesunden und Patienten mit essentieller Hyperlipämie untersucht. 2. Bei Patienten mit essentieller Hyperlipämie ist die Einbaurate der freien Fettsäuren in Plasmatriglyceride im nüchternen Zustand im Mittel gegenüber Stoffwechselgesunden leicht erhöht. Diese Erhöhung wird vermutlich durch die erhöhte Konzentration an freien Fettsäuren im Plasma dieser Patienten verursacht. Die Triglycerid-Clearence ist bei diesen Patienten aber auf das Fünffache erniedrigt und die Triglycerid-Umsatzzeit entsprechend auf das Zehnfache erhöht. 3. Auf Grund dieser Ergebnisse ist eine Störung im Abtransport der Plasmatriglyceride als Ursache des extrem erhöhten Triglyceridspiegels bei Patienten mit essentieller Hyperlipämie anzusehen.
