    Notes: Respiratory syncytial virus (RSV) is the most important microbiological cause of lower respiratory tract infection (LRTI) in infants. Mannan-binding lectin (MBL) is believed to play a major protective role in the vulnerable period in infancy where the maternal antibodies have been catabolized, and the adaptive immune system has not yet matured. Mutations in the promoter region and in exon 1 of the gene-encoding MBL result in low serum levels of MBL. MBL deficiency is the most common immunodeficiency on the African Continent with frequencies of the variant alleles up to 0.29. We investigated whether MBL deficiency has an impact on the hospitalization for LRTI caused by RSV in infants from Soweto, South Africa. The cases were ethnic black Africans identified through surveillance for RSV-LRTI at Chris Hani Baragwanath Hospital, Soweto, and the controls were sampled from four immunization clinics in the area. Fifty-five cases and 113 age- and sex-matched controls were identified. Seventy-six per cent were under 6 months of age, and 42% (n = 23) were under 3 months of age. No association was found between low levels of MBL or carriage of variant alleles and LRTI caused by RSV, odds ratio (OR) 1.00 (CI 0.99–1.03) and OR 1.24 (0.73–2.12). We did not find support for the hypothesis that MBL deficiency leads to the hospitalization for LRTI caused by RSV.

    Notes: To investigate whether mannan-binding lectin (MBL) in umbilical cord blood (UCB) is associated with the incidence rate of hospitalizations for infections during early childhood. A follow-up study from birth to 31 months of age, with endpoint data from the Danish National Hospital Discharge Registry. The concentration of MBL and immunoglobulin A (IgA) in UCB was measured in a time-resolved immunofluorometric assay. Information on possible confounding factors was obtained from questionnaires completed by mothers during their pregnancy, and characteristics of the child at birth were registered by a midwife on a structured coding sheet. A total of 2104 infants born in the period between 1 February 1990 and 25 May 1991 to mothers living in the municipality of Aarhus. Concentration of MBL and IgA in UCB, gestational age, birth weight, number of siblings less than 3 years old, mother's smoking habits, alcohol and coffee consumption, educational and marital status and previous spontaneous abortions and stillbirths. Of 2104 children, 626 were hospitalized at least once, and 346 of these were hospitalized with infection. The hazard ratio (HR) of hospitalization with infections in children with low levels of MBL (〈120 ng/ml) was 1.4 (95% confidence interval (CI), 1.0–1.8), which was similar to our findings on hospitalization because of other diseases. The HR of hospitalization for viral infections was 2.8 (CI, 1.3–5.9). Low levels of MBL in UCB were associated with hospitalization in general and particularly with hospitalization for viral infections.

    Notes: The recent identification of a novel clinical entity, the severe acute respiratory syndrome (SARS), the rapid subsequent spread and case fatality rates of 14–15% have prompted a massive international collaborative investigation facilitated by a network of laboratories established by the World Health Organization (WHO). As SARS has the potential of becoming the first pandemic of the new millennium, a global warning by the WHO was issued on 12 March 2003. The disease, which is believed to have its origin in the Chinese Guangdong province, spread from Hong Kong via international airports to its current worldwide distribution. The concerted efforts of a globally united scientific community have led to the independent isolation and identification of a novel coronavirus from SARS patients by several groups. The extraordinarily rapid isolation of a causative agent of this newly emerged infectious disease constitutes an unprecedented scientific achievement. The main scope of the article is to provide the clinician with an overview of the natural history, epidemiology and clinical characteristics of SARS. On the basis of the recently published viral genome and structural features common to the members of the coronavirus family, a model for host cell–virus interaction and possible targets for antiviral drugs are presented. The epidemiological consequences of introducing a novel pathogen in a previously unexposed population and the origin and evolution of a new and more pathogenic strain of coronavirus are discussed.

    Notes: Allergic contact dermatitis is a delayed type hypersensitivity reaction mediated by allergen specific T-lymphocytes. Allergen exposure leads to the activation of specific T-lymphocytes, which subsequently start to produce and release a vast array of cytokines and chemokines. However, the list of relevant genes taking part in the elicitation phase of contact dermatitis is not complete. In this study, we evaluate the use of the high-density microarray technology, which enable us to assess the global gene expression in allergen-stimulated peripheral blood mononuclear cells (PBMC). We included 3 chromium-allergic patients and 3 non-allergic controls in the study. Cultures of PBMC were established from each participant and stimulated with 100 ug/ul CrCl3 or media alone. The cell cultures were grown for 24 hours and the gene expression was analysed using an Affymetrix GeneChip(R) Array. Of the genes that exhibited differences of expression (p 〈 0.01) in allergen-activated PBMC from patients compared to controls, 54%(159/294) displayed increased activity and 46%(136/294) displayed decreased activity. Of the 159 up-regulated genes, 41 genes had a fold change above 1.50 and 30 genes among the 136 down-regulated genes had a fold change below -1.5. A significant number of the genes that showed differential expression in the cell cultures established from the allergic patients are known to be involved in immune responses and inflammation. The data indicates that the method of microarrays is a valuable tool for investigating the gene expression profile in our model system for allergic contact dermatitis.

    Notes: Background:  The need for prevention to reduce the number of occupational hand eczema is high. Occupational hand eczema is the most frequently recognised work-related disease in Denmark. Previous findings have shown that almost half of all cases develop a chronic condition with persistent dermatitis, and the annual cost to society is immense.Aims:  The aim of this study was to survey the trends and development of occupational hand eczema in Denmark and thereby help to ensure future successful prevention of chronic disabling occupational hand eczema.Methods:  758 patients with recognised occupational hand eczema were included prospectively in the period October 2001- November 2002. Data on diagnoses, disease duration, severity, absence from work and occupation was obtained from The Danish National Board of Industrial Injuries and an additional questionnaire was administered by mail.Results:  621 patients answered the questionnaire (response rate 82%). Irritant contact dermatitis was the most frequent diagnosis and the female/male ratio was 2:1. High prevalence was found in particularly wet occupations. 19 per cent had sick leave more than 5 weeks per year and the mean disease duration was 4.8 years (median 2.1 years). 68.2% had chronic changes.Conclusion:  The results showed a marked gender difference in the pattern of diagnosis and occupation. The impact of occupational hand eczema is still high with prolonged absence from work and a high percentage of chronic disease. The results of the study give important suggestions for future preventive strategies for health authorities.

    Notes: We studied the human teratogenic risk of sulfasalazine because this drug interferes with folate metabolism.〈section xml:id="abs1-2"〉〈title type="main"〉Methods:Case control study within the Hungarian Case Control Surveillance of Congenital Abnormalities, 1980–1996; based on 22 865 new-born infants or foetuses with congenital abnormalities, and 38 151 babies without any detected congenital abnormalities (control group).〈section xml:id="abs1-3"〉〈title type="main"〉Results:Seventeen pregnant women (0.07%) were treated with sulfasalazine in the case group, and 26 (0.07%) in the control group. The overall adjusted adds ratio of congenital abnormalities after sulfasalazine treatment was odds ratio=1.2 (95% confidence interval: 0.6–2.1). None of the analyses indicated any significant increased prevalence of selected congenital abnormalities among the exposed compared with the not exposed.〈section xml:id="abs1-4"〉〈title type="main"〉Conclusions:We found no significant increased prevalence of selected congenital abnormalities in the children of women treated with sulfasalazine during pregnancy. However, the amount of information is limited and additional data are needed to rule out a teratogenic effect.

    Notes: ReO3 has been studied at pressures up to 52 GPa by X-ray powder diffraction. The previously observed cubic Im3¯ high-pressure phase was shown to transform to a monoclinic MnF3-related phase at about 3 GPa. All patterns recorded above 12 GPa could be indexed on rhombohedral cells. The compressibility was observed to decrease abruptly at 38 GPa. It is therefore proposed that the oxygen ions are hexagonally close packed above this pressure, giving rise to two rhombohedral phases labelled I and II. The zero-pressure bulk moduli Bo of the observed phases were determined and the rhombohedral phase II was found to have an extremely large value of 617 (10) GPa. It was found that ReO3 transforms back to the Pm3¯m phase found at ambient pressure.

    Notes: Abstract  The objective of the study was to examine the human teratogenic potential of oral penicillin V: phenoxymethylpenicillin treatment during pregnancy in the large population-based dataset of the Hungarian Case-Control Surveillance of Congenital Abnormalities, 1980–1996. The dataset included 22,865 pregnant women who had fetuses or newborns with congenital abnormalities and 38,151 matched pregnant women who had newborn infants without any congenital abnormality (population control group). Of 22,865 case pregnant women, 173 (0.8%) had phenoxymethylpenicillin treatments, while of 38,151 population controls, 218 (0.6) were treated by this penicillin V (crude OR 1.3 with 95% CI: 1.1–1.6). This difference was explained mainly by recall bias and confounders because adjusted OR for medically documented phenoxymethylpenicillin treatments did not show difference during the second-third months of gestation, i.e. in the critical period for most major congenital abnormalities in case-matched control pairs. Thus, treatment with oral phenoxymethylpenicillin during pregnancy presents very little if any teratogenic risk to the fetus.
