    Description / Table of Contents: Zusammenfassung Eine besondere Bedeutung der Thymusdrüse für die Myasthenie ist schon vor 70 Jahren vermutet worden (Weigert, 1901); bereits 1911 wurde die erste Thymektomie bei einer Myasthenie-Patientin durchgeführt (Sauerbruch). Inzwischen weiß man, daß bei der Mehrzahl der Myastheniker Thymusveränderungen vorliegen. Die im letzten Jahrzehnt aufgedeckten Zusammenhänge zwischen myasthenischer Reaktion und immunpathologischen Veränderungen hat die Thymusausschaltung in neuem Licht erscheinen lassen, aber die Therapie auch in anderer Hinsicht beeinflußt. Aus dem englischen Sprachraum ist wiederholt über größere Serien von Thymektomie bei myasthenischer Reaktion berichtet worden. Es hat sich dabei in einem großen Teil der Fälle eine günstige Wirkung gezeigt, so daß die Thymektomie vielfach zur Standardbehandlung gehört. Nur beim Thymom (histologische Diagnose!) ändert sich der Verlauf offenbar nicht wesentlich. Bei den 15 Myasthenie-Patienten, die die Autoren von 1967 bis 1970 mit Thymektomie behandelten, sind ernsthafte Komplikationen postoperativ nicht aufgetreten. Auch an diesem Krankengut bestätigte sich, daß der Verlauf der myasthenischen Reaktion beim Thymom durch die Thymektomie weniger gut beeinflußt werden kann als bei der hyperplastischen und normalen Thymusdrüse. Die günstigen Ergebnisse bei Kindern und Jugendlichen zeigten den Einfluß des Lebensalters auf den Operationserfolg. Die Notwendigkeit der prä- und postoperativen Betreuung auf Intensivstationen wird betont. Schließlich gehen die Autoren auf die weiteren Behandlungsmöglichkeiten ein; gerade die pragmatische Kombination auch mit ACTH, Steroiden und neuerdings Immunsuppressiva wird als vorteilhaft hervorgehoben. An Hand eines großen Krankengutes wird gezeigt, daß die Prognose der myasthenischen Reaktion sich insgesamt bei Anwendung aller verfügbaren Möglichkeiten zur Behandlung entscheidend gebessert hat.
    Notes: Summary 70 years ago it was already suspected the thymus gland might be of particular significance in myasthenia gravis (Weigert, 1901), and in 1911 the first thymectomy was carried out in a female myasthenia patient (Sauerbruch). We now know that alterations of the thymus gland occur in the majority of myasthenia gravis patients. Relationships between the myasthenic reaction and immunopathologic changes, which were discovered in the last decade, have made the removal of the thymus appear in a new light as they have also influenced the therapeutic approach in another respect. In the English speaking countries it has been repeatedly reported on large series of thymectomies in cases of myasthenic reaction. In many of them surgery had a beneficial effect so that thymectomy has frequently become part of the standard treatment. Only in the case of thymoma (histological diagnosis!) does the course not evidently change. In the 15 cases of myasthenia gravis of the authors, which have undergone thymectomy between 1967 and 1970, no serious postoperative complications occurred. It was also confirmed by these cases that thymectomy may influence the course of the myasthenic reaction in thymoma less beneficially than in cases of a hyperplastic and normal thymus. The favourable results in children and young adults demonstrate the influence of age on the success of the operation. The necessity of the pre- and postoperative care to be taken over by intensive treatment units is stressed. Finally the authors consider further therapeutic possibilities; the pragmatic combination of ACTH, steroids and recently of immunosuppressive agents is suggested to be of advantage. The survey of a large number of cases shows that the prognosis of myasthenic reaction has altogether decisively improved with the use of all available therapeutic measures.

    Notes: Summary Fifty-seven thymomas, defined as neoplasms of the epithelial-reticular framework cells of the thymus, were assessed in respect to histologic type, inclusive of there ultrastructural aspects. The median age of the 57 patients was 40.4 years, with a range of 2 1/2–72 years. All neoplasms were located in the anterior mediastinum. The tumours in 40 cases were encapsulated and without invasion of adjacent tissue or implants (= non-invasive thymomas). The tumors in 17 cases were invasive of adjacent tissue, particularly mediastinal pleura, pericard and trachea. Six of 57 patients (= 10.5%) with thymomas have had a thoracic and supraclavicular lymph node metastasis, and osteolytic metastases in the vertebrae and sternum. Thirty-seven (=64.9%) were so-called lympho-epithelial, 7 (=12.3%) pure epithelial, 4 (= 7.0%) atypical (or anaplastic) with granulomatous focuses, 3 (= 5.3%) carcinoid and one (=1.8%) seminomatous tumors. One patient have had a thymic cyst as a tumor-like conditions of the thymus, and four patients (= 7.0%) have had a thymo-lipoma. The histologie type of thymoma had no proof value in predicting prognosis with the exception of the so-called atypical or anaplastic thymoma. The fine structural aspects of thymomas and the fine structural differential diagnosis of anterior mediastinal tumors are discussed.

    Notes: Summary The most important prognostic determinants of the thymomas are the gross findings at operation (= the presence or absence of gross invasion of adjacent tissue) and the presence or absence of the thymoma-associated systemic disease, particulary myasthenia gravis. The histologic type of thymoma had no proof value in predicting prognosis with the exception of the so-called atypical thymomas. Thirty-four of 57 patients with thymomas were females and 23 males. The tumors in 40 cases were non-invasive thymomas, and in 17 cases the tumour were invasive of adjacent tissue. 35.1 percent of patients were asymptomatic, the tumours being discovered on roentgenograms done on a routine basis or for an unrelated porpose. 40.3 percent of patients have had a thymomaassociated systemic disease. The most common presenting symptoms were related to myasthenia gravis (26.3%). The 5-year survival rate was 90 percent for non-invasive thymomas without myastenia gravis and 50 percent for invasive thymomas. The 5-year survival rate for patients with myasthenia gravis and encapsulated (non-invasive) thymomas was approximately 60 percent, whereas that for invasive thymomas with myasthenia gravis was 40 percent. The primary form of therapy for all thymomas is the surgical excision. In cases with invasive and/or metastasizing thymomas, postoperative radiation and adjuvanted chemotherapy is indicated, but in most series, the longterm results of this form of therapy are discouraging.

    Notes: The kinetics of RNA synthesis have been studied under various growth conditions using synchronized Tetrahymena pyriformis GL. The curves obtained are approximately linear in growth-supporting media but vary under other circumstances. The presence of amino acids in the medium stimulates RNA synthesis in Tetrahymena as it does in bacteria. Since these induced variations do not influence the essential features of the pre-cytokinetic period it is inferred that fluctuations in the rate of RNA synthesis described by others are probably not essential features of temperature-induced synchrony. In addition, the translational stability of division-related templates at synchronizing temperatures has been investigated. The synthesis of proteins necessary for cytokinesis is shown to be greatly reduced under conditions simulating a heat-shock. Inhibitor studies using cycloheximide indicate that protein synthesis is required longer into the cytokinetic phase than had previously been thought. Collectively the data are all compatible with the hypothesis that synthesis of division-related proteins is drastically reduced in heat-shocked Tetrahymena and that the basis for this reduction is hydrolysis of template RNA without concurrent translation.

    Notes: Intracellular ribonuclease from the ciliate Tetrahymena pyriformis GL was purified 10-fold. After preheating for 20 minutes at 100°C of the ribonuclease preparation 80% of its activity was lost. Preheating under the same conditions, however, in the presence of RNA, did not affect the enzyme activity. Between 0°C and 41°C the apparent activation energy was 15,600 cal per deg. per mole.
