    Description / Table of Contents: Zusammenfassung Die Hypersekretion von biliärem Cholesterin scheint der Schlüsseldefekt in der Pathogenese der Cholesteringallensteine zu sein und ist möglicherweise bedingt durch eine erhöhte Cholesterinsynthese. Um die fraktionelle Synthese von biliärem Cholesterin und Plasmacholesterin zu messen, wurden 5 männliche und 3 weibliche gesunde Probanden mit einer intakten enterohepatischen Zirkulation intravenös mit [1-13C]Acetat für 15 h infundiert. Proben duodenaler Galle und Blutproben wurden stündlich gewonnen und eine Formuladiät enteral verabreicht. Die Massenverteilung des freien Cholesterins wurde mittels Gaschromatographie mit Massenspektrometrie analysiert. Die Anwendung der Mass Isotopomer Distribution Analysis — (MIDA) — Technik erlaubte die Berechnung der fraktionellen Synthese. Nach 6stündiger Infusion erreichte die [13C]Markierung des cytosolischen Acetatpools etwa 12%. Die individuellen fraktionellen Cholesterinsynthesen im Plasma und in der Galle korrelierten signifikant miteinander (6–15 h) und betrugen 4,2 und 5,3% nach 15 h. Aus dieser Studie wurde die Schlußfolgerung gezogen, daß neu synthetisiertes Cholesterin gegenüber dem Plasma zu einem höheren Anteil in die Galle sezerniert wird.
    Notes: Summary Hypersecretion of biliary cholesterol appears to be the key defect in the pathogenesis of cholesterol gallstones, and this may be due to an enhanced synthesis of cholesterol. To measure fractional syntheses of biliary and plasma cholesterol, five male and 3 femalc healthy humans with an intact enterohepatic circulation were infused intravenously with [1-13C]acetate for 15 h. Samples of duodenal bile and blood were taken hourly and an enteral formula diet was given. Free cholesterol mass distribution was analyzed by gas chromatography mass spectrometry. The Mass Isotopomer Distribution Analysis (MIDA) technique allowed to calculate fractional synthesis. After 6 hours of infusion, the [13C]label of the cytosolic acetate pool reached a plateau of approximately 12%. Individual fractional cholesterol synthesis in plasma and bile correlated significantly (6–15 h) and amounted to 4.2% and 5.3% after 15 h, respectively. It may be concluded from this study, that newly synthesized cholesterol is secreted into bile to a higher extent than into plasma.

    Notes: Summary While several radiological findings point towards the basal ganglia as a possible anatomical site of the lesion in dystonia patients the biochemical basis of the disorder is still unknown. 5-Hydroxyindoleacetic acid (5-HIAA) and homovanillic acid (HVA) levels — the respective metabolites of serotonin and dopamine — were measured in lumbar cerebrospinal fluid (lCSF) of 15 patients with idiopathic adult-onset focal dystonia and in lCSF of 11 controls. 100 μl lCSF were analyzed for 5-HIAA and HVA by reversed-phase HPLC with electrochemical detection. 5-HIAA levels were significantly reduced in dystonia patients (11.4μg/ml) compared to controls (18.4ng/ml) (p 〈 0.02). HVA levels in dystonia patients (30.3ng/ml) were below control values (41.6ng/ml) but this finding did not reach statistical significance. Decreased lCSF levels of 5-HIAA suggest an impaired serotonin metabolism in patients with idiopathic adult-onset dystonia. This observation may provide a biochemical basis for a more specific pharmacotherapy in dystonia patients.

    Notes: The case of a 36-year-old male patient is reported who fell from 20 m, sustaining injuries to the abdomen and pelvis and fractures of both arms and the left leg as well as erect dislocation of both shoulders (luxatio erecta humeri). The injury on the right was subcoracoid, that on the left subglenoid – a compound dislocation of the humerus head through the axilla. Based on this case and the pertinent literature, the pathophysiology, diagnosis and treatment of this rare injury are discussed. After immediate closed reduction, soft tissue damage, fractures or neurovascular lesions should be operated on as soon as possible.

    Notes: In a prospective study, we examined 34 patients with shoulder instabilities and 5 patients with unclear chronic shoulder pain (4 females, 35 males; 18 – 56 years of age, median 28 years) by CT arthrography and MRT arthrography from August 1994 through December 1995. No complications were seen when gadolinium-DPTA was applied intra-articularly. Twenty-three patients were follwed up by operation and/or arthroscopy; 20 patients underwent a modified, open Bankart operation. In this paper, we present a new classification for damage of the anterior capsule and labrum. MRT arthrography showed better results in judging the anterior labrum and in determining the degree of damage to the labrum (sensitivity, specificity and accuracy 100%) in comparison with CT arthrography (sensitivity 90%, specificity 100%, accuracy 91%). Furthermore, MRT arthrography gave clearer results than CT arthrography regarding SLAP and cartilage lesions. Thus, MRT arthrography has proved to be a very exact method for diagnosing shoulder instabilities and is superior to CT arthrography in diagnostic accuracy.

    Notes: Abstract The advantages of laser welding are high process velocity, the possibility to joint alloys, which are not solderable, and the high bonding strength. During the laser welding process a high energy input in a very short time and a small area is performed, resulting in possible changes of structure and composition. Direct information of the change of the electrochemical behaviour of dental alloys after Laser welding procedure does not exist. The aim of this work was, therefore, to assess the electrochemical behaviour of laser welded joints in comparison with the bulk material using the mini-cell system. The logI vs. E curves performed on the bulk material show a complete change concerning the characteristic data, the shape and the position in comparison to such curves measured on the laser track. The characterisation of the electrochemical behaviour of laser welding tracks seems to be very easy by using the mini-cell system.
