    Notes: Abstract A 50-year-old woman with idiopathic deep cerebral sinus and vein thrombosis (DCVT) had cerebellar disturbance prior to impaired consciousness. CT and MRI revealed haemorrhagic infarction in the cerebellum and signal changes suggesting infarction in the thalamus and basal ganglia bilaterally. The straight sinus and internal cerebral vein (ICV) were dense on CT. On angiography, the vein of Galen (VG) and straight sinus were not seen. Following clinical recovery, CT and MRI became normal, and angiography showed recanalization of the VG and ICV. The relationship between cerebellar infarction and DCVT, and signal changes on CT and MRI are discussed.

    Notes: Abstract We devised a three-dimensional method for estimation of cerebral development and myelination which measures cerebral volume using MRI. Accuracy of the system was estimated using cadaver brains. The mean percentage error in the calculated volumes compared with the real volumes was 2.33 %, range 0.00–5.33 %. We applied the method to the volume of both cerebral hemispheres (CH), basal ganglia, thalamus and internal capsule (BT), and myelinated white matter (WM) in 44 neurologically normal individuals (4 months to 28 years of age), 13 patients with spastic motor disturbances (2–25 years of age), and 9 patients with athetotic motor disturbances (2–23 years of age). In the neurologically normal cases, the volumes of CH, BT and WM increased with age; the volume of MW more slowly than that of CH. In cases with spastic motor disturbances, the volumes of CH, BT and WM were between –1.4 and 3.5 SD, –1.0 and –3.5 SD, and 0.0 and –5.2 SD respectively, of those of neurologically-normal cases. On the other hand, 7 of the 9 cases with athetotic motor disturbances were within 2 SD of the volume of CH in neurologically normal cases. Our method for direct measurement of cerebral volume based on serial MRI should be useful for the accurate assessment of brain development and quantitative analysis of delayed myelination.

    Notes: Summary  Mirroring the ambiguities noted with other endocrine tumours, histology is a poor predictor of the invasiveness of pituitary adenomas. Accordingly, the proteins p27Kip 1 (p27) and Ki-67, which respectively block cell-cycle progression and reflect proliferative activity of cells, were studied immunohistochemically in 123 specimens of pituitary adenomas including 57 clinically verified invasive cases. Only Ki-67 had a significant association with invasiveness; only p27 differed significantly between functioning and nonfunctioning adenomas, with a higher frequency of cell labelling in the former. No relationship between Ki-67 and p27 was obtained. Ki-67 detected by MIB-1 is thus an important marker related to the invasive potential of adenomas, and thereby may be helpful in planning postoperative management of patients with pituitary adenomas.

    Notes: Summary  We experienced a case of solitary trigeminal neurinoma in a 10-year-old boy without stigmata of neurofibromatosis. This boy presented with progressive cerebellar ataxia, right abducens palsy and right hemiparesis. Magnetic resonance images (MRI) demonstrated a tumour extending into both the middle and posterior fossas forming a dumbbell-shaped mass lesion with a size of 35×30×45 mm. The tumour was successfully removed free of complications by a two-stage operation composed of right subtemporal and suboccipital retromastoid approaches. It is extremely rare for a solitary neurinoma to occur at this site in a child without neurofibromatosis. Surgical approaches in this type of tumour are discussed with a review of the literature.

    Notes: Abstract. The objective of our study was to determine the correlation between gallbladder opacification by delayed CT and contrast-associated nephropathy (CAN). Delayed CT after angiography was performed in 269 patients. Gallbladder opacification was defined when the CT value in gallbladder was more than 30 HU. Positive renal dysfunction was recognized as CAN which defined as an increase in serum creatinine level 〉 0.3 mg/dl and 〉 20 % on days 1, 3, or 7. Gallbladder opacification appeared in 154 of 269 (57 %) patients and CAN in 35 (13 %) patients. Of 117 patients without gallbladder opacification, 17 CAN (14 %) were recognized, and of 152 patients with gallbladder opacification, 18 CAN (12 %) were recognized. No significant correlation existed between delayed gallbladder opacification and CAN.

    Notes: Abstract. Background: To examine the incidence and predisposing factors of gallbladder opacification in delayed computed tomography (CT) after angiography. Methods: CT examination 12–24 h after angiography was performed in 389 patients. Univariate and multivariate analyses were made. Results: Two hundred thirty-three of 389 patients (60%) revealed gallbladder opacification. Type of contrast medium (P 〈 0.01), total bilirubin (P 〈 0.01), and serum creatinine (P 〈 0.01) were significant and independent factors relating to gallbladder opacification. Ioxaglate (70%, relative odds = 2.86) and iohexol (63%, relative odds = 2.03) showed higher gallbladder opacification rates than that of iopamidol (45%, relative odds = 1.43) and diatrizoate (30%). Patients with a lower serum bilirubin level (relative odds = 1.67) and a raised serum creatinine level (relative odds = 2.01) showed higher incidence of gallbladder opacification. Conclusion: Gallbladder opacification after angiography is not an abnormal finding on delayed CT in patients with not only abnormal renal function but also normal hepatobiliary and normal renal function, especially for modern contrast media.
