Zusammenfassung Zur Prüfung der Wertigkeit von Akutphasenproteinen und Komplementfaktoren in der Frühdiagnose der intraabdominalen Infektion, wurden bei 60 Patienten nach abdominalchirurgischen Eingriffen die Antiproteasen alpha-l-Antitrypsin, alpha-2-Macroglobin, Transferrin und Antithrombin III, sowie der Komplementfaktoren C3 und C4 postoperativ bestimmt. Patienten, die eine intraabdominelle Infektion entwickelten, hatten bereits am 2. postoperativen Tag signifikant erhöhte alpha-1-Antitrypsin Plasmakonzentrationen im Vergleich zu Patienten mit unkompliziertem postoperativen Verlauf. Mittels der linearen Diskriminanzanalyse war es bereits am 2. postoperativen Tag möglich, eine kommende intraabdominele Infektion in 84% der Fälle richtig vorauszusagen.

Summary Sixty surgical patients admitted to the intensive care unit were monitored by serial measurement of plasma protein concentrations. The following proteins were evaluated: α-l-antitrypsin, α-2-macroglobulin, transferrin, antithrombin III, and complement factors C3 and C4. Patients, who developed postoperative intra-abdominal infection had significantly higher plasma levels of α-l-antitrypsin as early as the 2nd postoperative day, compared to patients without infections. The plasma concentration of α-2-macroglobulin was lowered in patients with infections. By means of multiple-step discrimination analysis, the value of α-l-antitrypsin and α-2-macroglobulin levels for early assessment of intra-abdominal sepsis was examined. As early as the 2nd postoperative day it was possible to predict the need for relaparotomy in 84% of cases.
