    Description / Table of Contents: Abstract After successful centralization of laboratory analyses since more than 30 years, advances in biosensors, microprocessors, measurement of undiluted whole blood and miniaturization of laboratory analyzers are leading nowadays more and more to a re-decentralization in the laboratory medicine. Point-of-care-testing (POCT), which is defined as any laboratory test performed outside central or decentralized laboratories, is becoming more and more popular. The theoretical advantages of POCT are faster turn-around-times (TAT), more rapid medical decisions, avoidance of sample identification and sample transport problems and the need of only small specimen volumes. These advantages are frequently mentioned, but are not associated with a clear clinical benefit. The disadvantages of POCT such as incorrect handling and/or maintenance of the analyzers by non-trained clinical staff, inadequate or even absent calibrations and/or quality controls, lack of cost-effectiveness because of an increased number of analyzers and more expensive reagents, insufficient documentation and difficult comparability of the obtained POCT-results with routine laboratory results, are strongly evident. According to the authors’ opinion the decision for the establishing of POCT has only to be made in a close co-operation between physicians and laboratorians in order to vouch for necessity and high quality of the analyses. Taking the local situation into consideration (24-h-central laboratory, etc.) the spectrum of parameters measured by means of POCT should be rigorously restricted to the vital functions. Such analytes should be: hemoglobin or hematocrit, activated whole blood clotting time, blood gases, sodium, potassium, ionized calcium, glucose, creatinine, ammonia and lactate.
    Notes: Zusammenfassung Nach erfolgreicher Zentralisierung der Laboranalytik seit mehr als 30 Jahren, führen Fortschritte bei Biosensoren, Mikroprozessoren, Messung in unverdünntem Vollblut und die Verkleinerung von Laboranalysen heute wieder immer mehr zu einer neuerlichen Re-Dezentralisierung in der Labormedizin. Das Point-Of-Care-Testing (POCT) wird nahe dem Patienten außerhalb von zentralen oder dezentralen Laboratorien durchgeführt. Die theoretischen Vorteile des POCT sind schnellere Turn-Around-Times (TAT), schnellere medizinische Entscheidungen, Vermeidung von Probenidentifizierungs- und Probentransportproblemen sowie die Notwendigkeit von nur kleinen Probenmengen. Diese Vorteile werden zwar oft hervorgehoben, gehen jedoch nicht mit einem klar erkennbaren klinischen Nutzen einher. Die Nachteile des POCT wie unsachgemäße Handhabung und/ oder Wartung der Analysengeräte durch das ungeschulte Stationspersonal, mangelnde oder fehlende Kalibration und/oder Qualitätskontrolle, Kosten durch Vorhaltung mehrerer Geräte und teurerer Reagenzien, insuffiziente Dokumentation und schwierige Vergleichbarkeit zwischen den POCT-Ergebnissen und den Routinelaborergebnissen, sind ebenfalls augenscheinlich. Nach Meinung der Autoren sollte die Entscheidung zur Etablierung eines POCT nur in engster Kooperation zwischen Klinikern und Labormedizinern erfolgen, um Sinnhaftigkeit und hohe Qualität der Analysen zu gewährleisten. Unter Berücksichtigung der lokalen Situation (24 h-Zentrallabor, etc.) sollte sich das Parameterspektrum des POCT streng auf vitale Funktionen beschränken. Die dafür notwendigen Analyte sollten sein: Hämoglobin oder Hämatokrit, Aktivierte Vollblut-Gerinnungszeit, Blutgase, Natrium, Kalium, ionisiertes Kalzium, Glukose, Ammoniak und Laktat.

    Notes: Abstract  In the seventh round of the Institute for Reference Materials and Measurements' International Measurement Evaluation Programme, 277 clinical laboratories from 17 countries participated. The measurands consisted of inorganic components, present at minor and trace level, in a human liquid serum. Results for calcium, chloride, copper, magnesium, sodium, selenium and zinc are reported and are compared to reference values established mainly by isotope dilution mass spectrometry and neutron-activation analysis. The overall conclusion is that routine measurements of total concentrations of the components by clinical laboratories are of high quality. The fraction of significant errors is small (0.5%) and the self-declared uncertainty values are generally fairly realistic. Results are further commented on with respect to the recent development in metrology in chemistry.

    Description / Table of Contents: Zusammenfassung Bei 50 Patienten mit Sarkoidose (39 mit aktiver, 11 mit inaktiver Sarkoidose) wurde das Angiotensin-converting enzyme (ACE) im Serum bestimmt. Die Gruppe der Kontrollpatienten umfaßte 50 Personen (34 mit chronischen Lungenerkrankungen, 9 mit Morbus Hodgkin, 7 mit chronischer Polyarthritis). Der Unterschied der Mittelwerte der ACE-Aktivität war zwischen Sarkoidosepatienten (28,6±11,4) und Kontrollpatienten (14,8±4) hochsignifikant (p〈0,001). Ebenso fand sich ein deutlicher Unterschied zwischen Patienten mit aktiver (32,8±11) und inaktiver Sarkoidose (21,9±5,1) mit einemp〈0,001. Corticosteroide senken die ACE-Aktivität bei Sarkoidosepatienten deutlich, ohne daß dies als Hinweis für eine Besserung der Sarkoidose gewertet werden kann. Die gesteigerte ACE-Aktivität bei anderen epitheloidzelligen Granulomatosen (biliäre Zirrhose, Morbus Gaucher, Lepra, Asbestose) repräsentiert möglicherweise einen mit der Sarkoidose gemeinsamen pathophysiologischen Mechanismus.
    Notes: Summary Serum angiotensin-converting enzyme (ACE) was studied in 50 patients with sarcoidosis (39 active, 11 inactive) as well as in 50 control patients (34 with chronic nonspecific lung disease, 9 with Hodgkin and 7 with rheumatoid arthritis). There was a significant difference of ACE activity between sarcoidosis patients (28.6±11.4) and controls (14.8±4), and also between active (32.8±11) and inactive (21.9±5.1) sarcoidosis (p〈0.001). Coricosteroid treatment seems to lower ACE activity in patients with sarcoidosis without offering a clue for clinical improvement. Increased ACE activity in other granulomatous disorders is being discussed. ACE activity thus proves to be a valuable test especially in differentiating active from inactive sarcoidosis.

    Description / Table of Contents: Zusammenfassung Bei 2 Fällen von Lesch-Nyhan-Syndrom mit vollständigem bzw. teilweisem Fehlen der Hypoxanthin-Guanin-Phosphoribosyltransferase-Aktivität konnte Enzymprotein mit einem gegen normales Erythrocytenenzym gerichteten Anti-Serum nachgewiesen werden. Der Defekt dieser angeborenen Stoffwechselerkrankung scheint demnach in der Synthese eines inaktiven, sich im Doppeldiffusionstest immunologisch normal verhaltenden Enzymproteins zu liegen.
    Notes: Summary In two cases of Lesch-Nyhan syndrome with complete and partial deficiency of hypoxanthine-guanine phosphoribosyl transferase activity, the presence of enzyme protein could be demonstrated using a specific antiserum against hypoxanthine-guanine phosphoribosyl transferase from normal erythrocyte. Therefore, the defect in this disease seems to be based on the synthesis of an inactive but in the double-diffusion-technique immunologically normal enzyme protein.

    Notes: Summary For some years the glycosylated plasma proteins, the so-called fructosamines, have been used for the evaluation of metabolic control in diabetic patients. We studied the usefulness of fructosamine as an intermediate parameter in the control of diabetes and we also corrected fructosamine for serum protein. Furthermore, the dependence on daily times and amounts of food intake were examined for fructosamine alone as well as for protein-corrected fructosamine. Both parameters showed a statistically significant correlation with HbA1C (r=0,7;P〈0,001). Changes in the metabolic state of the diabetic patients were reflected more rapidly by both these parameters than by HbA1C. While the correction of fructosamine for serum protein eliminated its postprandial increase in the diabetic patients, circadian variation remained unchanged.

    Notes: Summary In two field experiments, plant materials labelled with 15N were buried separately within mesh bags in soil, which was subsequently sown with barley. In the first experiment, different parts of white clover (Trifolium repens), red clover (T. pratense), subterranean clover (T. subterraneum), field bean (Vicia faba), and timothy (Phleum pratense) were used, and in the second, parts of subterranean clover of different maturity. The plant materials were analysed for their initial concentrations of total N, 15N, C, ethanol-soluble compounds, starch, hemicellulose, cellulose, lignin, and ash. After the barley had been harvested, the bags were collected and analysed for their total N and 15N. In the first experiment the release of N was highest from white clover stems + petioles (86%) and lowest from field bean roots (20%). In stepwise regression analysis, the release of N was explained best by the initial concentrations of lignin, cellulose, hemicellulose, and N (listed according to decreasing partial correlations). Although the C/N ratio of the plant materials varied widely (11–46), statistically the release of N was not significantly correlated with this variable. The results of the second experiment using subterranean clover of different maturity confirmed those of the first experiment.

    Description / Table of Contents: Zusammenfassung Bei neun Sarkoidose-Patienten und 40 Patienten mit anderen dermatologischen Krankheiten wurde die Enzymaktivität des “Angiotensin-converting Enzym” (ACE) im Serum nach der Methode von Cushman und Cheung, modifiziert nach Lieberman, bestimmt. Es fanden sich nicht nur bei Sarkoidose, sondern auch bei einer Reihe anderer dermatologischer Krankheiten deutlich erhöhte Enzymaktivitäten. Die erhöhten ACE-Aktivitäten werden als Folge bestimmter entzündlich-immunologischer Vorgänge interpretiert.
    Notes: Summary Serum activities of ACE were investigated in 40 patients with dermatological diseases and in nine patients with sarcoidosis. The method of Cushman and Cheung, modified according to Lieberman, was used. Both sarcoidosis patients and patients with dermatological diseases showed marked elevations of ACE-activity. The increase in ACE-activity might result from inflammation and immunological events.
