    Notes: Penicillamine-induced cutaneous elastosis perforans serpiginosa associated with a large air-cyst in the right lung is described in a 29-year-old female patient with Wilson disease. Identical light and electron-microscopic changes were present in both dermal and pulmonary elastic tissue, suggesting a disseminated drug-induced cutaneo-visceral elastosis. Lung cysts have not been previously reported in association with long term penicillamine treatment. The electron-microscopic morphology of the elastic fibers was found to be “specific” enough to allow separation of penicillamine-induced elastosis perforans serpiginosa from other forms of this disease.

    Notes: A skin biopsy from an 8-year-old boy with generalized lichen nitidus revealed a transepidermal perforating lesion never observed before in this disorder. A disturbance in dermo-epidermal interaction with alterations of epidermal cell kinetics could explain this finding in a disease which, curiously, shows many histologic features conducive to transepidermal perforation. A clear distinction should be made between primary and secondary perforating dermatoses, since perforation per sc is a non-specific cutaneous reaction pattern occurring in the course of many unrelated disorders.

    Description / Table of Contents: Zusammenfassung Bei einem 18jährigen Patienten wurden acneforme Veränderungen innerhalb eines Becker-Naevus beobachtet. Das histologische Substrat bestand in einer granulomatösen Follikulitis mit teilweiser Destruktion der epithelialen Wurzelscheide und Follikelycysten mit fokaler penetration von Keratin in das Corium. Das Vorliegen einer Acne vulgaris konnte histologisch ausgeschlossen werden. Der Becker-Naevus repräsentiert einen komplexen ektodermalen, neuroektodermalen und mesenchymalen Prozeß auf naevogener Basis, bei dem auch eine funktionelle Störung im Bereich der pilären Komplexe vorliegt.
    Notes: Summary Acneform lesions strictly confined to a Becker's nevus were observed in a 18-year-old man. Histologically, a granulomatous folliculitis with partial destruction of the epithelial root sheath and follicular cysts with focal penetration of keratin into the surrounding dermis were present, ruling out acne vulgaris. The concept of Becker's nevus as a complex nevogenic process, affecting ectodermal, neuroectodermal, and mesenchymal structures, also in terms of dysfunction of the pilary complex, is suggested.

    Description / Table of Contents: Zusammenfassung Es wurde eine Studie zur Bioverfügbarkeit von 8-MOP an 38-Psoriasispatienten durchgeführt. Zur Anwendung kamen ein handelsübliches 8-MOP-Präparat, das den Wirkstoff in Form eines mikronisierten Pulvers in Hartgelatinekapseln enthielt und ein 8-MOP-Präparat mit neuer Galenik in gelöster Form, abgefüllt in Weichgelatinekapseln. Mit der neuen galenischen Zubereitung konnten bereits nach einer halben Stunde um den Faktor 3,2 (P≤0.01, Wilcoxon Test) höhere und stabilere 8-MOP-Serumspiegel erzielt werden, wofür in erster Linie die verbesserten Resorptionsbedingungen verantwortlich sind.
    Notes: Summary A bioequivalence study was performed with two different 8-MOP preparations. Thirty-eight patients participated in the investigation. We compared a product already marketed containing a micronized powder in hard gelatine capsules with a new product in soft gelatine capsules containing the 8-MOP in a solution. With the new galenic preparation the serum level obtained was 3.2 (P≤0.01) times higher than with the old product. Thus, the improved resorption out of the solution was mainly responsible for these results.

Pinkus 1954 formulierte biologische Konzept der epidermalen Elimination wurde in der Folge von Mehregan als grundlegendes Prinzip bei einer Reihe von heterogenen Hautkrankheiten erkannt. Die Bedutung dieses epidermalen Reaktionsmusters wird an Hand von fünf “perforierenden” Dermatosen demonstriert und sowohl die aktive Beteiligung des Koriums an diesem Mechanismus, sowie die Rolle epitheliomesenchymaler Wechselbeziehungen hervorgehoben. Es wird die histologische Differentialdiagnose und unterschiedliche Histogenese dieser Krankheitsgruppe diskutiert.
    Notes: Summary The histologic concept of epidermal elimination of particulate matter was formulated by Pinkus in 1954. Mehregan enlarged on this by recognizing its fundamental role in a variety of skin diseases. The importance of this epidermal reaction pattern in five “perforating” dermatoses is stressed. Also, it is shown that active participation of the corium and epithelio-mesenchymal interaction are required for the initiation of this mechanism. The histological differentiation and histogenesis of the presented dermatoses are discussed.

    Description / Table of Contents: Zusammenfassung Das biologische Phänomen des Epidermotropismus hat sein physiologisches Korrelat im dynamischen Gleichgewicht heterogener Zelltypen innerhalb der Epidermis. Im Rahmen pathologischer Prozesse kann es einerseits zu Verschiebungen in diesem Gleichgewichtszustand kommen, andererseits neigen gewisse ortsfremde epitheliale, neuroektodermale und mesenchymale Zellen zur bevorzugten Besiedelung der Epidermis sowie anderer epithelialer Strukturen. Die Bedeutung des Konzeptes der epidermalen Symbiose bei diesen Vorgängen sowie daraus abzuleitende Gemeinsamkeiten im klinischen Bild und der biologischen Verhaltensweise werden diskutiert.
    Notes: Summary The biologic phenomenon of epidermotropism is mirrored in the dynamic equilibrium between different cell types within the epidermis. In various pathological conditions, this balance may be upset, or foreign epithelial, neuroectodermal and mesenchymal tissue may selectively infiltrate epidermal or other epithelial structures. The importance of the concept of epidermal symbiosis in these conditions and resulting similarities in the clinical picture and biologic behavior are pointed out.

    Notes: Conclusion Until recently, the effectivity of BP in the treatment of acne vulgaris was considered to be primarily a function of its bacteriostatic action. However, in vitro investigation using pure colonies of P.a. showed that the areas of inhibition produced by a 5% and 10% commercial BP preparation and the isolated vehicle were of about equal size.

    Description / Table of Contents: Zusammenfassung Bei neun Sarkoidose-Patienten und 40 Patienten mit anderen dermatologischen Krankheiten wurde die Enzymaktivität des “Angiotensin-converting Enzym” (ACE) im Serum nach der Methode von Cushman und Cheung, modifiziert nach Lieberman, bestimmt. Es fanden sich nicht nur bei Sarkoidose, sondern auch bei einer Reihe anderer dermatologischer Krankheiten deutlich erhöhte Enzymaktivitäten. Die erhöhten ACE-Aktivitäten werden als Folge bestimmter entzündlich-immunologischer Vorgänge interpretiert.
    Notes: Summary Serum activities of ACE were investigated in 40 patients with dermatological diseases and in nine patients with sarcoidosis. The method of Cushman and Cheung, modified according to Lieberman, was used. Both sarcoidosis patients and patients with dermatological diseases showed marked elevations of ACE-activity. The increase in ACE-activity might result from inflammation and immunological events.
