    Notes: Sixteen children completed a randomized placebo-controlled double-blind study comparing the efficacy of pre-treatment with (1) 20 mg sodium cromoglycate (SCG) powder delivered by Spinhaler, (2) 2 mg SCG delivered by pressurized aerosol and (3) placebo. The drug was administered 15 min before the 6 min of treadmill-running performed at the same time of day on 3 consecutive days in a climate chamber with a temperature of 23°C and a relative humidity of 40%,. All the patients, none of whom were taking systemic steroids, had shown a post-exercise fall in peak expiratory flow (PEF) of more than 15% from baseline when tested one day prior to the study. On the day with no pre-treatment the average post-exercise fall in PEF was 46% (±4) (mean ± s.e.mean). After pre-treatment with SCG powder the average fall in PEF was 21%, (±3), after pre-treatment with SCG aerosol the fall was 22% (±3) and after placebo a post-exercise fall of 31% (±5) was seen. Both forms of SCG were significantly better than pre-treatment with placebo. Comparing no pre-treatment with pre-treatment with placebo revealed a statistically significant difference (P 〈 0.001) indicating a placebo effect. It is concluded that this study design has a marked placebo effect. Both pre-treatment with 2 mg SCG delivered by pressurized aerosol and 20 mg delivered by Spinhaler is superior to placebo in conferring protection against exercise-induced asthma (EIA). No bronchodilating effect was seen in the first 15 min after administration of the drug.

    Notes: Abstract Greatly improved second-harmonic emission has been obtained from dye monolayers adsorbed at quartz etalons. Two different types of interferometers have been studied in terms of the SH signal from dye coverage versus harmonic background emission. The phasematching-like conditions for constructive interferometric enhancement and the concomitant spatial and spectral emission characteristics are discussed in detail.

    Notes: Diskussion Publikationen über die Anwendung der epiduralen Morphinapplikation in der Geburstshilfe liegen bislang von englischen Arbeitsgruppen vor. So berichten Husemeier et al. über keine suffiziente Analgesie nach Injektion von 2 mg Morphin verdünnt in 8 ml Kochsalzlösung, während Perris et al. in 50% der Fälle eine sehr gute und in weiteren 43% eine mäßige Beeinflussung des Wehenschmerzes beobachten konnten. Das kardiozirkulatorische sowie respiratorische System blieben dabei ebenso unbeeinflußt wie die sympathische Aktivität der Schwangeren. Neben diesen durchaus positiven Untersuchungsergebnissen sind aber auch Mitteilungen publiziert worden, welche eine nach Naloxon reversible Depression der Respiration beschreiben, wobei der analgetische Effekt durch das applizierte Naloxon unbeeinflußt blieb. Unsere eigenen Erfahrungen mit der morphininduzierten Epiduralanalgesie ließen bislang keinerlei Nebenwirkungen am maternalen wie fötalen Organismus erkennen.

Bei 70 Psoriasis-Patienten konnte eine mit der Ausdehnung der Hauterscheinungen, der Akuität und Behandlung signifikant korrelierte Erhöhung des Serumspiegels and Wachstumshormonen gegenüber 40 Kontrollpersonen festgestellt werden. Die ätiologische Bedeutung der hereditären Produktionspersistenz des menschlichen Wachtumshormons wird diskutiert.
    Notes: Summary Pathological alterations of capillaries, accelerated epidermopoesis, increased rate and extent of metabolism in the psoriatic lesion and first manifestation of psoriasis rarely in the first 20 years of life made us decide to investigate the human growth hormone (STH) in blood serum. In 70 patients suffering from psoriasis we found that the extention of skin lesions was correlated to the STH levels, which were determined every 2 h all over the day. The average STH level in psoriasis was three times as high as average value in the group of 40 controls. Thus, one can conclude that psoriasis is caused by a persistence of STH with individual penetrance.

    Notes: Summary Ten new cases of accessory bisatellited marker chromosomes examined in different laboratories are reported. As a basis for genetic counselling in the context of prenatal diagnosis a cytogenetic categorization of such marker chromosomes is proposed and an estimation of the genetic risk associated with each category is carried out. The results are as follows: 1. There is no increased risk for offspring with abnormal phenotype born to a healthy carrier of an accessory bisatellited marker chromosome with either a single or two closely adjacent C-bands (Category AI or AII). 2. The unbiased sample of cases with de novo accessory bisatellited marker chromosomes of categories AI and AII is too small to allow a satisfactory estimation of the actual risk that, in case of such a prenatal finding, the foetus may not show a normal phenotype as a consequence of the marker chromosome. There is, however, evidence that this risk may be lower than 10%. 3. Accessory bisatellited marker chromosomes showing a discrete pattern of G- or R-bands situated between two distant C-bands (Category AIII) usually indicate a chromosomal imbalance giving rise to an abnormal phenotype. Mosaic carriers of such dicentric marker chromosomes may, however, present a normal phenotype.

    Notes: Summary Basal-lateral plasma membrane vesicles were isolated from rat duodenum and jejunum by a Percoll gradient centrifugation technique. Ca-uptake into and Ca-release from the vesicles was studied by a rapid filtration method. In the absence of Na (K-medium) at a Ca concentration of 0.05 mmol/liter and pH 7.4, addition of 5mm MgATP stimulated Ca-uptake up to 10-fold as compared to a control without ATP. Since the Ca-ionophore A23187 (2 μg/ml) prevented the accumulation of Ca above the equilibrium uptake and rapidly released Ca accumulated by the vesicles in the presence of ATP, it is concluded that the ATP-dependent uptake of Ca involves accumulation of Ca inside the vesicles. The ATP-driven Ca-transport comigrates with the (Na+K)-ATPase and dissociates from the marker enzymes for mitochondrial inner membrane, endoplasmic reticulum and brush border membrane. It is not inhibited by 1 μg/ml oligomicin or 0.1 mmol/liter ruthenium red. Replacing K by Na inhibits ATP-dependent Ca-uptake by 60%. Efflux of Ca from passively preloaded vesicles is strongly temperature sensitive and enhanced by A23187. An inwardly directed Na-gradient stimulates Ca-efflux as compared to a K-gradient. Addition of gramicidin reduces the Na-stimulation of Ca-efflux, indicating direct coupling of Na and Ca fluxes across basal-lateral membranes. The results suggest that basal-lateral membranes possess two distinct mechanisms for Ca-transport:a) ATP-driven Ca-transport andb) Na/Ca-exchange.
