    Notes: Summary Plasma motilin concentrations were measured in eight controls and 11 patients with radiolucent gallstones over a period of 3–4 h following an overnight fast. In five of these patients the interdigestive motor complex (IDMC) was studied simultaneously. The results indicate: a) cyclical changes of plasma motilin concentrations (mean cycle duration 110±9.9 and 102±8.4 min, $$\bar x$$ ±SEM) and a significant correlation between the basal intercyclical motilin concentrations and the age (p〈0.02) in the controls and the gallstone patients studied, and b) a significant correlation between the maximal motilin increase and the age in the controls (p〈0.01) but not in the gallstone patients. The results therefore suggest a different behaviour of the interdigestive motilin patterns in controls and gallstone patients. As the motilin cycles were accompanied in 90% with an IDMC in the five patients studied this conclusion might include an abnormal interdigestive motor activity within the intestinal tract in gallstone patients.

    Description / Table of Contents: Summary Spontaneous or traumatic arteriovenous fistulae between vertebral artery and the surrounding venous plexus may cause vertebrobasilar hypoperfusion by steal effects. We report on a 71-year-old man presenting with vertigo. Duplex sonography revealed a vertebral arteriovenous fistula at the C4/5 level with the typical perivascular color Doppler artifact and hyperperfusion in the supplying arteries and draining veins. Angiography confirmed the findings; the consequently performed endovascular embolization using platin coils and silicon balloon removed the symptoms immediately. Ultrasonographic follow-up examinations within 5 months demonstrated the success of therapy showing only low-flow fistula yet. This case demonstrates that early detection of a vertebral arteriovenous fistula by duplex sonography is highly beneficial because efficient treatment modalities are available.
    Notes: Zusammenfassung Spontane oder traumatische arteriovenöse Fisteln zwischen der A. vertebralis und dem umgebenden Venenplexus können durch Steal-Effekte Schwindel und andere Symptome einer vertebrobasilären Mangeldurchblutung hervorrufen. Sie werden sehr selten diagnostiziert. Wir berichten über einen 71jährigen Patienten, der wegen Schwindels stationär untersucht wurde. Die Duplexsonographie ergab den Befund einer vertebralen arteriovenösen Fistel in Höhe C4/5 rechts mit dem typischen perivaskulären Farb-Doppler-Artefakt sowie Nachweis einer Mehrdurchströmung in den zuführenden Arterien und drainierenden Venen. Der Befund wurde angiographisch bestätigt. Die daraufhin durchgeführte endovaskuläre Embolisation mit Platin-Coils und Siliconballon beseitigte die Symptome schlagartig. Sonographische Kontrollen in den folgenden 5 Monaten bestätigten den Erfolg der Behandlung: die Fistel war zwar weiterhin nachweisbar, sie wies aber nur noch ein geringes Shuntvolumen auf. Der Fall zeigt den Wert der frühzeitigen sonographischen Diagnosestellung einer vertebralen arteriovenösen Fistel, da effektive Behandlungsmöglichkeiten zur Verfügung stehen.

    Notes: Abstract Cells of the yeast strain Debaryomyces vanrijiae SBUG 770, grown with glucose, converted biphenyl to 4-hydroxybiphenyl as the major metabolite. In addition, 2-hydroxybiphenyl was formed in minor amounts. No further degradation of these substances was detected. However, these monohydroxylated derivatives were oxidised by alkane-grown cells in the presence of the co-metabolic substrate, tetradecane. Under these conditions 2-hydroxybiphenyl was oxidised to 2,5-dihydroxybiphenyl, and 4-hydroxybiphenyl was rapidly metabolised by formation of two major metabolites. One was identified as 3,4-dihydroxybiphenyl. Characterisation of the second product as 4-phenylmuconolactone points to a further metabolism of the initially formed dihydroxylated biphenyl via ortho-ring fission.

    Notes: Abstract Penicillium canescens SBUG-M 1139 was shown to be able to grow using phenoxybutyric acid as the sole carbon source. The rapid conversion of the phenoxyalkanoic acid resulted in the formation of phenol, which was metabolized completely. These reactions were accompanied by an accumulation of the methyl ketone phenoxypropan-2-one. Furthermore, during the metabolism of phenoxybutyric acid, 4-phenoxy-2,3-dehydrobutyric acid, 4-phenoxy-3-hydroxybutyric acid, phenoxyacetic acid, and phenoxypropan-2-ol accumulated in minor amounts. Clearly, fungi can metabolize phenoxyalkanoic acids to produce methyl ketones in a manner analogous to that used for the conversion of short- or medium-chain fatty acids by fungi.

    Notes: Abstract The signal source impedance Z of implanted pacemaker electrodes plays an important role for adequate sensing of electrogram signals and has so far been little explored. A frequency-domain method for thein vivo calculation of Z is described. Electrogram signals picked up between two electrodes were recorded unloaded, and loaded with resistors and capacitors, then amplified, and stored on magnetic tape. After time sampling and a.d. conversion, the signals were read into a digital computer as discrete time vectors with 2P samples. Each vector was subjected to a fast Fourier transform (f.f.t.), and the ratio H(jωi) between the transformed loaded and unloaded vectors was calculated. For the electrode impedance Z a linear model was chosen that consisted of the tissue and electrode resistance RT, in series with a parallel coupling between Faraday resistance RF, and Helmholtz capacitance CH. By nonlinear regression analysis the parameters (∧) in this model were estimated from the ratios H(jjωi) obtained in patients with permanent pacemaker electrodes, and in patients with temporary multicore wire electrodes after open heart surgery. As an estimate for RT was also used the voltage/current ratio R′T, 90 μs into the stimulation pulse from an external variable parameter pulse generator. The 3-component impedance model described the essential features of the electrode impedance in the frequency range up to 100 Hz. It was found that ĈH, decreased slightly with increasing frequency, and that ŘT in general was larger than R′T. A modified model with frequency dependent parameters was found to obtain a better approach to the experimental data.
