    Notes: The clinicopathological features of two infants with desmoplastic supratentorial neuroepithelial tumours are described. The cases were similar to the 26 cases reported previously as desmoplastic infantile ganglioglioma and superficial cerebral astrocytoma attached to dura. Neuronal differentiation was absent on routine stains, but was immunohistochemically established. We review the literature and suggest classifying this clinicopathological entity by using a general designation such as desmoplastic supratentorial neuroepithelial tumours of infancy, a term which indicates the variability in the amount and lines of differentiation.

    Notes: In vivo activated T cells could be isolated from cerebrospinal fluid (CSF) of a patient suffering from chronic meningitis of unclear origin. Although the patient's skin reactivity to purified protein derivative (PPD) was negative, and peripheral T cells did not proliferate to this antigen in vitro, the majority of T cell clones from CSF specifically recognized PPD on either autologous or allogeneic HLA class II compatible macrophages. Remarkably, peripheral blood mononuclear cells potently suppressed the PPD-specific proliferate responses of healthy donors. The selective enrichment of oligoclonal IgG in the CSF but not in the patient's serum further indicated T and B cell responses lacking systemic feedback control. Analyses of a persisting immune stimulation in the CSF provide a potent diagnostic tool and may explain neurological complications as observed in a number of autoimmune diseases and chronic infections.

    Description / Table of Contents: Zusammenfassung Kugelförmige, hyaline Mikrothromben sind durch ihre Größe zwischen 2 und 30 μm, ihre kugelförmige Gestalt sowie ihre PAS-und PTAH-Positivität gekennzeichnet. Sie gelten als morphologischer Indikator von Störungen der Mikrozirkulation und Hämostase, wie sie besonders im Schockzustand auftreten. In zwei eigenen Fällen konnten ungewöhnlich große kugelförmige, hyaline Mikrothromben beobachtet werden, die zum Teil zentrale wabige Auflockerungen und Auflösungen bis zur Ausbildung offener Hohlkugeln aufwiesen. Elektronenmikroskopisch konnten gleichartige Befunde erhoben werden, wie sie von anderen Autoren an typischen Schockkörpern beobachtet wurden. Die eigenen morphologischen Befunde lassen den Schluß zu, daß es im Zentrum der kugelförmigen, hyalinen Mikrothromben zu Auflösungserscheinungen kommt, die bis an die Oberfläche vordringen können, wo dann möglicherweise die Fibrinolyse beschleunigt abläuft. Die von anderen Autoren beschriebenen zellulären Elemente als mögliches Kondensationszentrum für Fibrinpräzipitate erweisen sich als strukturell intakte Zellen, die sich offenbar in Nischen von offenen Hohlkugeln oder im Zentrum ringförmiger, hyaliner Mikrothromben befinden. Die Fragen der diagnostischen Relevanz, der Vitalität und zeitlichen Zuordnung werden erörtert.
    Notes: Summary Globular hyaline microthrombi are characterized by their diameter (2–30 μm), globular form, and positive PAS and PTAH reaction. They are a morphologic indicator of disturbed microcirculation and hemostasis, such as that occurring in shock. In two of our cases, we observed large globular hyaline microthrombi, some with central webbed degradation progressing to the formation of closed or open hollow globules. The electron microscopic findings were similar to those observed by other investigators in typical shock bodies. On the basis of the morphologic findings, the authors conclude that degradation phenomena start at the center of globular hyaline microthrombi and can progress to the surface where accelerated fibrinolysis is possible. The cellular elements that other investigators have described as a possible condensation center for fibrin precipitate proved to be structurally intact cells that are apparently located in niches of open hollow globules or in the center of circular hyaline microthrombi. The diagnostic relevance, vitality, and temporal classification are discussed.

    Description / Table of Contents: Zusammenfassung Zur Frage denkbarer toxischer Veränderungen des Gehirns nach Parathion-Vergiftung wurden insgesamt 42 tödliche Vergiftungsfälle neuropathologisch untersucht. In 41 Fällen, in denen die Betroffenen akut an den Folgen der Vergiftung gestorben waren, konnten Sauerstoffmangelveränderungen an Nervenzellen nur vereinzelt (7%) festgestellt werden. Nahezu regelmäßig (93% der Fälle) lag eine ausgeprägte Hyperämie vor, oftmals gemeinsam mit reaktionslosen, periventrikulären Blutungen, die in 40% der Fälle auftraten. In einem Drittel der Fälle (33%) fand sich eine mäßggradige Schwellung der Oligodendroglia. Während sich histologisch in nahezu einem Drittel der Fälle (30%) Hinweise auf ein Ödem fanden, konnte durch Vergleich von Hirngewichten der eigenen Fälle mit Hirngewichten einer großen Vergleichspopulation ausschließlich in fünf Fällen (12%) ein eindeutig pathologisches Hirngewicht festgestellt werden, unter Berücksichtigung eines Vertrauensbereiches mit 95%iger Sicherheit. Allerdings lag das gemessene Hirngewicht in 18 Fallen (42%) über dem Sollgewicht, wenn der Vertrauensbereich nicht berücksichtigt wurde. Ausgeprägte Sauerstoffmangelveränderungen fanden sich ausschließlich bei einer Person, die die akute Vergiftung — nach einem Herz- und Atemstillstand — vier Wochen überlebt hatte. Demgegenüber konnten primär-toxische Veränderungen in den untersuchten Fällen nicht festgestellt werden. Dieser negative morphologische Befund steht den tierexperimentellen funktionellen Untersuchungen anderer Autoren gegenüber, die den Tod infolge Parathion-Vergiftung als Folge einer toxisch bedingten, zentralen Atemlähmung ansehen lassen mußten. Das Schrifttum wird diskutiert.
    Notes: Summary A total of 42 cases were examined neuropathologically to determine possible toxic changes occurring in the brain after parathion intoxication. Sporadic anoxic alterations were observed in 7% of 41 cases in which the cause of death was acute intoxication. Nearly all cases (93%), however, showed marked hyperemia, often coupled with small reactionsless, periventricular hemorrhages which occurred in 40% of the cases. In one third of the cases (33%) there was moderate swelling of the oligodendroglia. Whereas histological evidence of edema was found in nearly one third of the cases (30%), comparison of the brain weights in these subjects with those of a large comparative collective showed definite pathologic brain weights in only five cases (12%) with 95% confidence limits. It is true that in 18 cases (42%) the brain weight was above the normal value if the confidence limit is ignored. Pronounced anoxic alterations were observed in only one case in which the individual survived the acute intoxication for 4 weeks after initial respiratory arrest. Predominantly toxic changes, however, could not be detected in any of the cases examined. This negative morphologic finding does not agree with the physiologic alterations reported by other investigators using animal models; they considered the cause of death in cases of parathion intoxication to be the result of toxic paralysis of the respiratory center. The literature was discussed.
