    Structural transitions at low temperatures in vanadium deuterides (1973)

    Notes: X-ray and neutron-diffraction techniques have been used to ascertain the structures of vanadium deuterides with compositions between V2D and V4D3 over a temperature range 5–425 °K. The β phase, V2D, has a monoclinic crystal structure at room temperature with a∼c = 4.46; b = 3.00 Å, β = 95.5°. The deuterium atoms prefer octahedral interstitial sites. Near 425 °K this phase transforms to a b.c.c. phase, and the deuterium atoms prefer tetrahedral sites. The b.c.c. phase of V4D3 has been designated α′ and transforms to α′′ at about 220 °K. This transition is an ordering of deuterium as shown by additional neutron-diffraction peaks. This α′′ structure can be indexed with a∼c∼4.46; b∼3.00 Å; β = 90°. Below 160 °K an additional neutron peak appears at 2&thgr; = 12° which indicates a doubling of the b lattice parameter. This transformation to α′′′ phase involves further ordering. The metal sublattice in α′ and α′′′ is very near b.c.c. Therefore, the neutron patterns do not allow a determination of octahedral or tetrahedral site occupancy. The exact crystal structure cannot be determined. The V–D system is not completely analogous to the V–H system and some of the differences are discussed.

    Description / Table of Contents: Zusammenfassung Während bei der reaktiven, „elefantenfußartigen” Form eine stabile Osteosynthese die Heilung zur Folge hat, sind beim reaktionslosen „atrophischen” Poeudarthrosetyp zusätzlich zur stabilen Osteosynthese eine Dekortikation und eine Spongiosaplastik notwendig. Im Film werden beide Pseudarthrosetypen an der Tibia mit einer Platte stabilisiert, und zwar einer Zuggurtungsplatte für die elefantenfußartige und einer Neutralisationsplatte für die atrophische Pseudarthrose. Im letzteren Fall erfolgt nach Dekortikation und Spongiosaplastik zusätzlich ein Längenausgleich der um 2,5 cm verkürzten Tibia. Technik der Behandlung der Pseudarthrosen sowie der Verlängerung werden im Schema und in den einzelnen Operationsphasen ausführlich dargestellt.
    Notes: Summary The “elephant foot” type of reactive pseudarthrosis heals after rigid internal fixation, but the atrophic, non-reactive type requires decortication and an autogenous cancellous bone graft in addition. The film shows stabilization of the tibia with a plate in both types; a tension band plate is used in the case of “elephant foot” type pseudarthrosis and a neutralizing plate in the atrophic case. In the latter case the tibia has shortened by 2.5 cm, and this is corrected after decortication and the autogenous bone graft are complete. The techniques to be used in the treatment of pseudarthroses and that of lengthening are illustrated in detail, both in diagrams and at various stages of the operation.

    Description / Table of Contents: Zusammenfassung Zur Beantwortung der Frage, ob es im Knorpelüberzug eines Gelenkkörpers Prädilektionsstellen der Arthrose gibt, wurden biometrische Untersuchungen an Talusrollen eines Unfallkollektivs (316 Fälle) und eines Sektionskollektivs (306 Fälle) durchgeführt. Gemessen wurde einerseits das frontale und sagittale Profil der Talusrolle und andererseits der prozentuale Arthroseanteil an der Gelenkoberfläche. Dabei wurden folgende Befunde erhoben: 1. Die Talusrolle wird sowohl in frontaler als auch in sagittaler Ebene mit zunehmendem Alter flacher. Eine Zuordnung zu potentiellen arthrosefördernden Faktoren ist nicht möglich. Eine Ausnahme bilden die Diabetiker mit wesentlich konkaverem Talusprofil (ein Pendant zum Fischwirbel?). 2. Die arthrotischen Knorpelusuren im Bereich des oberen Sprunggelenkes sind mehrheitlich spiegelbildsymmetrisch und vorwiegend im Bereich der Rollkanten lokalisiert. Die prozentuale Arthrosefläche an der Gelenkknorpeloberfläche nimmt mit zunehmendem Alter zu. Eine Zuordnung zu potentiellen arthrosefördernden Faktoren ist auch hier nicht möglich.
    Notes: Summary The age dependent profile changes and the predilection areas of arthrosis in ankle joints were investigated using a series of patients with foregoing accidental trauma (316 cases) and from 306 consecutive autopsy cases. Biometrical analysis was carried out by measuring the profile of talar trochlea in sagittal and frontal plane as well as the relative area showing arthrosis compared to total cartilage area. The following results were obtained: The talar trochlea profile flattens in frontal as well as sagittal plane with age. A correlation between the age dependent profile changes and the possible arthrosis promoting factors could not be established. Diabetics show more concave profils than others. Arthrotic areas are usually symmetrical in the ankle joint and more pronounced in lateral and medial part of the articular surface. The arthrotic area increases with age, without correlation between the degree of arthrosis and the possible arthrosis promoting factors.

    Notes: Summary The uricosuric drug benzbromarone inhibits a number of membrane-bound mitochondrial enzymes. A strong inhibition of succinate dehydrogenase, α-glycerophosphate dehydrogenase and pyridinnucleotide transhydrogenase was found (approximately 28, 48, 45 μM benzbromarone for 50% inhibition respectively). Some possible consequences of these results for the lipid metabolism in patients during benzbromarone are discussed.

Der Defekt dieser angeborenen Stoffwechselerkrankung scheint demnach in der Synthese eines inaktiven, sich im Doppeldiffusionstest immunologisch normal verhaltenden Enzymproteins zu liegen.
    Notes: Summary In two cases of Lesch-Nyhan syndrome with complete and partial deficiency of hypoxanthine-guanine phosphoribosyl transferase activity, the presence of enzyme protein could be demonstrated using a specific antiserum against hypoxanthine-guanine phosphoribosyl transferase from normal erythrocyte. Therefore, the defect in this disease seems to be based on the synthesis of an inactive but in the double-diffusion-technique immunologically normal enzyme protein.

Neben einer peinlichen Asepsis bleibt aber die Operationstechnik von ausschlaggebender Bedeutung. In zwei zusammenhängenden Filmen werden Richtlinien und Ablauf dieses Eingriffes, wie sie an der Orthopädischen Universitätsklinik in Bern anhand von über 3000 Fällen ausgearbeitet wurden, aufgezeigt. Die Filme sind so abgefaßt, daß sie auch einzeln in sich abgeschlossen und verständlich sind.
    Notes: Summary Alloplastic replacement of the diseased hip joint has become a standard orthopedic operation. The surgical technique is, however, still of decisive importance, as is meticulous asepsis. These films show the general principles of this operation and the procedure developed for it in the University Orthopedic Clinic in Berne on the basis of experience of over 3000 cases. Each of the films is complete in itself.
