Notes: Forty-nine cases of cutaneous malignant lymphoma were reviewed in order to analyze the clinicopathological features of these neoplasms. Excluding 13 cases of mycosis fungoides and 4 cases of cutaneous involvement of proven adult T-cell lymphoma/leukemia, the remaining 32 cases were further classified according to their pathological and clinical features. There were 12 primary cutaneous lymphomas, 15 cases of secondary cutaneous involvement of systemic lymphoma, and 5 cases of concurrent skin and lymph node involvement. Histologically, large cell lymphoma predominated in both primary and secondary cutaneous lymphomas. Immunohistochemical study using monoclonal antibodies reactive with B- and T-cells in paraffin sections revealed the cellular lineage in 30 cases. Nineteen cases were of T-cell origin and 11 cases were of B-cell derivation. The prognosis of these patients was rather poor; 25 patients died within 5 years. The predominance of T-cell lymphoma contrasts with a higher frequency of cutaneous B-cell lymphoma in Western countries. As the clinicopathological features of cutaneous lymphomas are diverse, it is suggested that cutaneous lymphomas should be classified and studied in a similar way to their nodal counterparts.

Notes: summary  At the oral stage of swallowing, the tongue plays a major role and proper tongue performance is necessary to form the bolus and transfer it to the pharynx. For the present study we built a prototype device for safe and handy objective estimation of tongue motion and coordination with laryngeal elevation at swallowing. The device records tongue pressure by means of two strain gauge pressure transducers aligned 20 mm apart on a brass strap placed along the palatal midline. Laryngeal vibration is recorded with piezo-electric acceleration transducers. Time differences between pressure onset at the anterior and posterior transducers and the first spike from laryngeal vibration are measured. Ten healthy subjects were asked five times to swallow 5 mL of water. Pressure onset at the anterior transducer preceded posterior pressure by 294 ± 164 ms. Given the distance between the transducers, the tongue contracted (squeezed) at a speed of 93 ± 60 mm s−1. Laryngeal vibration occurred 671 ± 175 ms after the onset of anterior pressure. There was considerable variation in these parameters between subjects. Though the data is limited, the device successfully and easily revealed certain aspects of tongue motion and coordination with laryngeal elevation.

Notes: summary  The purpose of this study was to develop a new three-dimensional (3D) mimic model of an osseointegrated implant for finite element analysis (FEA) and to evaluate stress distributions in comparison with a model commonly used in most studies as a control. Based on the 3D computer graphic data obtained by serial in vivo bucco-lingual peri-implant bone structure at 75 μm interval in monkey, a mimic FEA model with trabecular structure and a control model with uniform cancellous bone were constructed. A vertical load of 143 N was applied at the top of the implant and induced stress was evaluated at the peri-implant bone. In the mimic model, stress was distributed at both cortical and cancellous bones (1–5 MPa) in bucco-lingual central planes, but concentrated at the cortical crest (3–7 MPa) in the mesio-distal central plane. In contrast, the control model presented stress concentration at the cortical crest around the implant (5–14 MPa), with less stress (0–1 MPa) at the peri-implant cancellous bone in both planes. The findings, that stress distribution at the peri-implant bone were quite different between the mimic and control models, suggest the need to carefully interpret stress distribution in previous studies with models of uniform cancellous peri-implant bone.

Notes: Aim:  To examine the pathological changes of hepatic arteries in idiopathic portal hypertension (IPH) which is characterized by the obliteration of the intrahepatic portal vein branches and presinusoidal portal hypertension. Methods and results:  Liver specimens (biopsied or surgically resected) from 20 patients with IPH, 20 patients with alcoholic fibrosis/cirrhosis (AF/C) and 20 histologically normal livers were used. The vascular lumina of arterial and venous vessels in portal tracts were morphometrically evaluated by an image analysis system. The ratio of portal venous luminal area to portal tract area (portal venous index) of IPH and that of AF/C were significantly reduced compared with normal liver. The portal venous index for IPH was significantly lower than that for AF/C. The ratio of hepatic arterial luminal area to portal tract area for AF/C was significantly higher than that in normal liver; however, that for IPH was similar to normal. The peribiliary vascular plexus was increased in AF/C but not in IPH. In AF/C, the number of mast cells and macrophages known to be the source of angiogenic substances was significantly increased in the portal tract compared with normal liver, while in IPH it was not increased. Conclusions:  In AF/C, a reduction in portal venous lumen was associated with an increase of hepatic arterial lumen and of angiogenesis-related cells in portal tracts. However, such compensatory arterial changes were not evident in IPH, and this compensatory failure may be a feature of IPH.

Notes: Abstract Necrotizing enterocolitis (NEC) is associated with considerable morbidity and mortality in infants. The diagnosis relies heavily upon radiographic and clinical features. Failure to accurately diagnose NEC is associated with a risk of complications and death, however overdiagnosis also causes both morbidity and mortality as well as excessive medical costs. This report documents the use of barium enema to evaluate suspected clinical or radiographic NEC in 31 premature infants with ambiguous clinical and radiographic signs. The enema was normal in 26 infants and no treatment for NEC was given. Only one of these infants developed signs of NEC subsequent to the examination. Five infants had radiographic evidence of colitis including small ulcerations, spasm, intramural extravasation of barium and mucosal irregularity. Two of the five positive cases are pathologically documented. The barium enema can represent a significant improvement in the specificity of the diagnosis of NEC. Its greatest value is in the exclusion of NEC in ambiguous cases.

Notes: Summary Plucked scalp hairs and hair roots of pili annulati were examined to understand their pathogenesis. Stereoscopic examinations of hairs in transmitted light and/or reflected light and light microscopic surveys of the cross-sections of hairs confirmed that the cortical empty spaces appeared to be responsible to the unique dotted shiny appearance of the hairs seen by the unaided eyes under a refracted light. By transmission electron microscope, small vacuoles and dense bodies were observed in the cytoplasm of the differentiating cortical cells; subsequently, with increasing number of tonofilaments, an uneven distribution of free ribosomes occurred and abnormal spaces containing fine granular substances were formed in the cytoplasm of the cortical cells. Occasionally, extremely large cortical trichohyaline granules were found. In the keratinized hair, irregular empty spaces were present in the cortex of the abnormal hair segments. Histochemically, the keratinized cortex of the affected hairs always had more residual SH groups than the controls. Pili annulati may be a disorder of protein metabolism involving a partial dysfunction of cytoplasmic ribosomes, resulting in a lack of cortical keratin formation.

Notes: Abstract Sixty-eight cases of neuroblastoma detected during mass screening being performed by 11 local self-governing bodies of Japan were studied concerning age, clinical stage, and survival rate. Their average ages at the first screening, at the beginning of thorough examination, and at the start of therapy were 215.7, 245.6, and 264.7 days, respectively. This screening, which is aimed at the 6-month-old infants is considered to be acceptable, on the grounds that the majority of the cases (95.6%) were asymptomatic at the time of thorough examination, that for most of the cases (98.5%) the initiation of therapy was at under 1 year of age, and that their 60-month survival rate was 87.5%. However, there seems to be room for discussion of the best age at which subjects should be screened, because the 68 cases included some patients already with advanced stages at the time of thorough examination, and because “false negative cases” were identified besides the 68 positive cases.

Notes: Abstract Attempts were made to analyze the toxin composition of the toxic “hiogi” scallop Chlamys nobilis. The toxins were partially purified from the digestive glands by column chromatography using Bio-Gel P-2 and Bio-Rex 70 (H+ form), resulting in separation into protogonyautoxin (PX), gonyautoxin (GTX) and saxitoxin (STX) fractions. Their total potencies were scored to be 〈100 mouse units (MU), 3200 MU and 3700 MU, respectively. A 5-min hydrolysis with 0.1 N HCl enhanced the potencies of PX and GTX fractions to 450 MU and 12000 MU respectively, whereas no enhancement occurred in the STX fraction at all. Electrophoretic, thin-layer chromatographic and high performance liquid chromatographic analyses demonstrated that the PX fraction consisted mainly of GTX8 and its epimer, the GTX fraction of GTX5, GTX6, along with two unidentified toxins, and the STX fraction exclusively of two unidentified toxins. This rather unique composition suggested a complex metabolism of PSP in this species.
