    Notes: A central role of iron in the pathogenesis of Parkinson's disease (PD), due to its increase in substantia nigra pars compacta dopaminergic neurons and reactive microglia and its capacity to enhance production of toxic reactive oxygen radicals, has been discussed for many years. Recent transcranial ultrasound findings and the observation of the ability of iron to induce aggregation and toxicity of α-synuclein have reinforced the critical role of iron in the pathogenesis of nigrostriatal injury. Presently the mechanisms involved in the disturbances of iron metabolism in PD remain obscure. In this review we summarize evidence from recent studies suggesting disturbances of iron metabolism in PD at possibly different levels including iron uptake, storage, intracellular metabolism, release and post-transcriptional control. Moreover we outline that the interaction of iron with other molecules, especially α-synuclein, may contribute to the process of neurodegeneration. Because many neurodegenerative diseases show increased accumulation of iron at the site of neurodegeneration, it is believed that maintenance of cellular iron homeostasis is crucial for the viability of neurons.

    Notes: Summary Schwannomas are benign solitary tumours of the peripheral nerve sheaths. The occurrence of multiple schwannomas usually implies hereditary disease. The most frequent syndrome associated with multiple schwannomas is neurofibromatosis type 2 (NF2), which is defined by bilateral vestibular schwannomas. Schwannomatosis is a distinct disease characterized by multiple pathologically proven schwannomas in the absence of vestibular schwannomas. It is not currently known if the presence of multiple schwannomas confined to a limb may represent a mosaic form of NF2 or a distinct disease, because mutation analysis of these tumours is not routinely performed. We report a 31-year-old patient who presented with multiple slowly growing subcutaneous tumours on his left arm. His family history was negative for cutaneous tumours or central nervous system disease, and he did not have additional features of NF2. Magnetic resonance tomography and ophthalmological examination excluded vestibular schwannoma and eye stigmata of NF2. After resection of three tumours, histological analysis confirmed the diagnosis of benign schwannomas. Molecular genetic analysis by temperature gradient gel electrophoresis and microsatellite marker analysis demonstrated two distinct mutations of the NF2 gene (NF2) in two different schwannomas, with concomitant loss of heterozygosity in both tumours. In contrast, neither normal skin nor peripheral blood lymphocytes revealed mutations of NF2. The clinical and molecular genetic findings suggest that the diagnosis in our patient is schwannomatosis rather than segmental NF2 because the mutations found in different tumours were not identical. The possibility of a localized predisposition for the acquisition of NF2 mutations is discussed.

    Notes: Zusammenfassung Es wurde gefunden, da\ die magnetischen Kernresonanzen 1 der Kupferisotope Cu63 und Cu65 in einem Einkristall von K3[Cu(CN)4] als Folge der Wechselwirkung der elektrischen Atomkernquadrupolmomente mit dem inhomogenen elektrischen Kristallfeld in je drei Komponenten aufspalten, deren Lage von der Orientierung des Kristalles im Magnetfeld abhängig ist. Die grö\te Aufspaltung, die im Falle der Kupferisotope (KernspinI=3/2) gleich der Quadrupolkopplungskonstanten ist, beträgt für Cu63 (1,113±0,003) MHz und für Cu65 (1,030±0,003) MHz. Durch eine bis zur zweiten Näherung durchgeführte Störungsrechnung, bei der die Quadrupoleinstellungsenergie in einem inhomogenen elektrischen Feld als Störung der reinen magnetischen Energieterme des magnetischen Kernmoments in einem äu\eren Magnetfeld eingeführt ist, lä\t sich die winkelabhängige Lage der Komponenten gut wiedergeben. Das elektrische Feld am Ort der Kupferkerne erweist sich als rotationssymmetrisch um die dreizählige Symmetrieachse des Kristalles. Für das Verhältnis der Quadrupolmomente ergibt sich aus den Messungen $$\frac{{Q_{Cu^{63} } }}{{Q_{Cu^{65} } }} = 1,081 \pm 0,003.$$ Dieser Wert liefert in Verbindung mit schon bekannten optischen Ergebnissen mä\iger Genauigkeit über die Absolutwerte der Quadrupolmomente als wahrscheinlichste Werte der Quadrupolmomente der Cu-Isotope $$Q_{Cu^{63,65} } = - (1,2 \pm 0,1) \cdot 10^{ - 24} cm^2 .$$

    Notes: Die Astrozytome hohen Malignitätsgrades (Grad III, IV) sind mit etwa 30% die häufigsten und zugleich aggressivsten Hirntumoren gliösen Ursprungs und führen unbehandelt innerhalb weniger Wochen zum Tode. Neben der primär einzusetzenden Operation ist der Stellenwert der postoperativen perkutanen Strahlentherapie in der Behandlung der malignen Gliome unbestritten. Derzeit gibt es keinen gesicherten Hinweis, daß eine Chemotherapie Operation und Strahlentherapie ersetzen kann, so daß die malignen Gliome wegen ihres aggressiven Wachstums und der damit verbundenen schlechten Prognose unverändert eine Herausforderung an den Radioonkologen stellen.

    Notes: Summary The aim of this study was to evaluate whether intra-operative ultrasound (=IOUS) is a suitable tool to detect residual tumour tissue after gross total resection in supratentorial gliomas. During a period of 18 months 45 patients with supratentorial gliomas (38 high-grade and 9 low-grade, according to the WHO-grading system [42]) were operated on. A series of 78 biopsies was taken from the resection cavity under continuous sonographic control at the end of surgery. Gross total tumour resection was intended in 34 patients (=76%). The biopsy specimens were matched with the sonographic features at each biopsy site. The sonographic appearance of the resection margins were classified into 2 groups: (1) Irregular hyperechoic areas extending from the cavity into the iso-echogenic brain tissue and (2) a dense small (≤3 mm in diameter) rather regular hyperechoic rim surrounding the resection cavity. 47 out of 53 biopsies taken from hyperechoic areas (group 1) (36 high-grade/11 low-grade) revealed solid tumour tissue (=89%). 34 (=72%) of these 47 areas were microscopically assessed as inconspicuous by the surgeon. 6 samples (4 high-grade/2 low-grade) contained tumour infiltration zone. 25 biopsies (23 high-grade/2 low-grade) taken from the hyperechoic rim [group 2] were diagnosed as follows: Normal brain tissue in 11, tumour infiltration zone in 8 and solid tumour tissue in 6 cases. Of 34 cases with “gross total removal” according to the surgeon's assessement 25 showed sonographic signs of residual tumour tissue, which was confirmed histologically as solid tumour tissue in 22 of these cases. It is concluded, that IOUS following resection of supratentorial gliomas can detect residual tumour tissue with high specificity and thus improve gross total resection. However, a thin hyperechoic rim surrounding the resection cavity (less than 3 mm in diameter) is a non-specific finding, which can mask thin residual tumour layers and therefore needs further evaluation of its nature.
