    Notes: Abstract Multiple endocrine neoplasia type 1 (MEN1) is an autosomal dominant disorder characterised by tumours of the parathyroids, pancreas and anterior pituitary. The MEN1 gene has been localised to a 2-Mb region of chromosome 11q13 by meiotic mapping studies in MEN1 families. Such studies may have a limited resolution of approximately 1 cM (i.e. 1 Mb) and we have therefore investigated 96 MEN1 families (40 British, 17 French, 12 Finnish, 7 Swedish, 7 Dutch, 7 North American, 2 Australian, 1 New Zealand, 1 German, 1 Spanish and 1 Danish) for linkage disequilibrium, in order to facilitate a finer mapping resolution. We have utilised five microsatellite DNA sequence polymorphisms from the candidate region and have accurately determined their allele sizes, which ranged from 161 bp to 272 bp. The heterozygosity and number of alleles (given in brackets), respectively, at the loci were: D11S1883 (76%, 11), D11S457 (55%, 5), PYGM (94%, 18), D11S1783 (10%, 4) and D11S449 (87%, 16). Allelic association was assessed by Chi-square 2 ×n contingency tables, by Fisher exact 2 ×n contingency tables and by a likelihood-based approach. The results of haplotype analysis revealed 91 different affected haplotypes in the 96 families, an identical affected haplotype being observed in no more than two families. These results indicate the absence of an ancestral affected haplotype. Significant linkage disequilibrium (P 〈 0.005) could be established amongst the microsatellite loci but not between the loci and MEN1 in either the total population or in any of the geographical sub-populations. The absence of linkage disequilibrium between MEN1 and the polymorphic loci is probably the result of the occurrence of multiple different disease-causing mutations in MEN1.

    Description / Table of Contents: Abstract Introduction: Pain in childhood still seems to be a major problem. In this study we investigated parents’ opinion about the frequency of pain in their children and the efficacy of pain treatment. Method: Parents of 93 children (average age: 11–14 years) answered a questionnaire. The participants were asked to evaluate the frequency of pain and the success of pain treatment in their children. They were also asked about their family structure. Results: The result confirmed that perioperative pain therapy in children was not satisfactory. However, the majority of parents were satisfied with the outpatient pain therapy. Conclusion: In order to achieve adequate and successful pain therapy in childhood, better information of the parents and better medical education of doctors is mandatory.
    Notes: Zusammenfassung Fragestellung: In unserer Studie ging es um die Darstellung wesentlicher Aspekte des kindlichen Schmerzes. Zur Untersuchung wurde ein Fragebogen verwendet, den Eltern über die Schmerzproblematik ihrer Kinder ausfüllen sollten. Methode: Anhand eines Fragebogens wurden Eltern von insgesamt 93 Kindern im Durchschnittsalter von 11–14 Jahren Fragen zur Häufigkeit von kindlichen Schmerzen und zur Zufriedenheit bezüglich der Schmerztherapie gestellt. Darüber hinaus sollte die Familienstruktur, in der die Kinder integriert waren, erhoben werden. Ergebnisse: Das Ergebnis der Umfrage bestätigt, daß die Schmerztherapie bei Kindern im Rahmen operativer Eingriffe zu wenig ernst genommen wird. Mit der Schmerztherapie außerhalb des Krankenhauses waren die Eltern überwiegend zufrieden. Schlußfolgerung: Um eine Verbesserung der Schmerztherapie bei Kindern zu erzielen, ist neben einer intensiven Weiterbildung der Ärzte auch eine bessere Aufklärung der Eltern notwendig.

Despite improvements in surgical practice and postoperative care, the large vertical midline or transverse transperitoneal approaches used in abdominal aortic surgery are still associated with a relatively high perioperative morbidity and mortality rate even in patients who are considered good risks for undergoing aortic surgery. This significant perioperative morbidity is partly caused by the major surgical trauma. To decrease the surgical stress on these patients we have developed a less-extensive procedure for this type of vascular reconstruction. Technique: The abdominal aorta is explored using a special retractor through a short upper median minilaparotomy utilizing modified conventional surgical hand instruments. For an aortobifemoral graft implantation, a retroperitoneal tunnel is necessary. During the creation of this tunnel special care should be exercised to avoid troublesome hemorrhage with iliac and other vein lacerations. To overcome these difficulties, we have developed a new tunneling device, which allows us to have visual control of the tunneling procedure. This prototype device contains a semiflexible tube with an inflatable balloon and a flexible videoendoscope. It is introduced along the external iliac artery into the retroperitoneal space and creates a tunnel through step-by-step inflation of the balloon. After this, the graft is implanted in the usual manner. To date, 19 abdominal vascular reconstructions have been performed with this method.

    Notes: Abstract The physisorption of trichloroethylene and tetrachloroethylene on ZSM-5 (Si/Al = 339) zeolite was investigated at 298 K using thermogravimetry, differential microcalorimetry and X-ray diffraction. The zeolite always undergoes a monoclinic-orthorhombic structural change during the adsorption of the first molecules. An alignment of the polar trichloroethylene molecules inside all the channels of the zeolite is proposed to account for the experimental results. A phase transition of the non-polar tetrachloroethylene is suggested to produce the stepped isotherm, the steep rise in the heat curve and the high increase in the mobility of the molecules for the loading of 4 molecules per unit cell.

    Notes: Abstract By means of a new method of measurement which defines a total of eight measuring points along the median sagittal plane of the skull, methodical aspects are explained. The advantages of this method consist in its easy use and exact reproducibility of the calculated data thus obtained. On account of the fact that it permits to obtain the median sagittal plane of the skull by means of points of a cartesian system of coordinate, phylogenetic comparison, standardisable on optional points of reference respectively planes of reference is possible. The specific trigonometric construction enables extensive interpretation of single angles, distances and parts of surface, the amount of detail of which exceeds that of method in use.

    Notes: Summary Bacteroides fragilis is a rare cause of septic arthritis. Most patients with B.fragilis septic arthritis have a chronic joint disease, particularly rheumatoid arthritis, and sources of infection are lesions of the gastrointestinal tract and the skin. We report a 69-year-old male, who developed B.fragilis septic arthritis after pilonidal sinus resection. High level of suspicion of development B.fragilis septic arthritis must be present in patients with chronic joint disease in whom gastrointestinal or skin surgery was previously performed.
