Notes: Summary To investigate the safety of the topical application of glutaraldehyde to the ankle and heel of man, the in vitro penetration of glutaraldehyde in a 10% aqueous solution through isolated human thin stratum corneum (chest and abdomen), isolated human epidermis (abdominal), and human thick stratum corneum (blister tops from the sole) was determined 1 h after application. Under these conditions, glutaraldehyde did not penetrate thick stratum corneum, while 2.8%–4.4% of the applied dose penetrated the isolated epdermis, and 3.3%–13.8% of the applied dose penetrated thin stratum corneum. An 8-week irritancy test was conducted by applying a 10% aqueous solution of glutaraldehyde to the ankle and heel area of 12 volunteers. Irritation and one case of sensitization resulted from glutaraldehyde application to areas of thin stratum corneum (anterior ankle) but not from applications to thick stratum corneum (medial, posterior, and lateral heel and posterior ankle), which may be a privileged site with respect to glutaraldehyde sensitivity.

Notes: Abstract We report the case of 5-week-old male infant who presented as a ‘near miss cot death’. He had the immunodeficient syndrome of defective neutrophil mobility and delayed umbilical cord separation. He was shown to have staphylococcal endocarditis with a large vegetation on the mitral valve, and acute obstruction of the mitral valve flow may have accounted for the suddenness of his presentation. Death resulted from overwhelming sepsis with widely disseminated micro-abscesses.

Notes: Abstract  Eleven patients (mean age 54 years, range 46–67 years) with osteoarthritis limited to the trapeziometacarpal-1 joint underwent a distal trapezium resection. In nine cases the palmaris longus tendon and in two cases a Swanson condylar implant was used as interposition material. The results were very disappointing. Only one patient was satisfied. Nine patients were reoperated upon (mean interval period 43 months, range 3–110) because of pain. In two cases, the cause was dislocation of the Swanson condylar implant. In the other seven cases, prominent bone on the trapezium, ulnar osteophytes, scaphotrapezial osteoarthritis, sclerosis of the trapezium remnant, and impingement were causal factors. In future, the selection of patients for this operation will be performed with even greater care. Despite these poor results, a definitive conclusion about the indication for this procedure can only be made after evaluation of an improved operative procedure.
