    Notes: For the International Fusion Materials Irradiation Facility (IFMIF) scenario, a 140 mA deuterium beam in continuous wave (cw) mode with an atomic yield of above 85% is required. The normalized root-mean-square emittance should be less than 0.2 π mm mrad at the entrance of a RFQ. As part of a conceptual design for IFMIF, a new ion source has been developed and is tested at the Institut für Angewandte Physik in Frankfurt. The ion source is of the volume type with a tungsten cathode driving the discharge. Both cw and pulsed mode are possible and were studied. First experiments were carried out with deuterium. 80 mA deuterons with a D+ fraction of above 90% were extracted [A. Maser et al., Rev. Sci. Instrum. 67, 1054 (1996)]. In order to avoid neutron generation by the d–d reaction, hydrogen was used instead of deuterium later on. As far as the ion source and plasma production processes are concerned, the use of hydrogen instead of deuterium is equivalent because of their similar atomic shells. Recently, a 200 mA proton beam at 55 kV was extracted in cw mode (according to Child–Langmuir, 200 mA H+ corresponds to 140 mA D+) with a H+ fraction of 93%. This article will give a detailed description of the ion source and the essential experimental results. Especially, the influence of important physical parameters (such as discharge current and strength of the filter field) on the H+ fraction was studied. By using different kinds of auxiliary gases, the influence on the H+ fraction and the noise level were investigated, too. © 2000 American Institute of Physics.

    Notes: Background The aetiology of pityriasis alba (PA), a common dermatosis in childhood, is still controversial. The objective of this study was to assess the possible aetiopathogenic factors of this disease in infants.Methods Forty-four patients with PA and 31 healthy children were examined and compared. Personal hygiene habits, sun exposure, presence of Staphylococcus aureus in nasal fossae and presence of major or minor signs of atopy were assessed during anamnesis and physical examination. Susceptibility to ultraviolet (UV) B radiation was measured by the onset of a contact hypersensitivity reaction to diphenylcyclopropenone in individuals sensitized in previously irradiated areas.Results The prevalence of PA was higher in individuals with darker skin, in high phototype categories, as well as in males. The number of daily baths and sun exposure between 10.00 h and 15.00 h were significantly higher in the PA group when compared with controls (P = 0.03 and P = 0.0015, respectively). The presence of atopy signs was more common in pityriasis patients (P = 0.002). Susceptibility to UVB radiation was 29.6% in the PA group vs. 29.0% in the control group; nevertheless, important differences were found after stratification in order to control possible confounding factors. The presence of S. aureus in the nostrils was equal in both groups.Conclusions Our results confirm that PA, in our population, is more prevalent in males and in individuals in higher phototype categories. In those with inadequate personal hygiene and sun exposure habits the disease is more accentuated, demonstrating that the xerosis presenting in individuals with atopic diathesis is an important element in the development of the disease. S. aureus is not an important aetiopathogenic factor in PA. Susceptibility to UVB becomes important when related to the patient's phototype.

    Notes: Background Severe respiratory syncytial virus (RSV) infection in early childhood has been associated with subsequent wheezing and atopy. The aim of this study was to test if severe RSV infection in early life was associated with an increase in type 2 cytokine production and atopy in Gambian children 5 years later.Methods A cohort of children with severe RSV infection during the first year of life (‘cases’, n = 66) and without (‘controls’, n = 122) was followed-up at 5 years of age. Immediate hypersensitivity to common allergens, airway reactivity, serum IgE concentration and the production of IFN-γ, IL-5 and IL-13 by lymphocytes activated in vitro with RSV F-G or control antigens was determined.Results After adjustment for confounders, cases produced significantly higher concentrations of IL-13 in response to RSV F-G and of IL-5 and IL-13 in response to tuberculin. Cases were more likely to have presented with a wheezy lower respiratory tract infection in the first 3 years of life (adjusted odds ratio = 9.9; 95% CI 1.6–61.0), but not thereafter. Cases and controls had similar skin response to allergens, airway reactivity and serum IgE concentrations.Conclusion Severe RSV infection in early life is associated with a higher production of type 2 cytokines in Gambian children at 5 years of age. However this does not appear to result in increased risk of atopy or clinical allergy at that age.

    Description / Table of Contents: Summary Anosognosia for hemiplegia (AHP), i. e., unawareness of motor deficits and associated disorders, has been frequently reported, predominantly following right hemispheric lesions. To a smaller extent, there are case reports of patients who give accounts of a feeling of strangeness concerning the contralesional limbs and sometimes attribute them to other persons. This “positive-variant” of AHP has been labeled “somatoparaphrenia” (SP). We report a case of SP in a 85-year-old woman with infarction of the right posterior cerebral artery and posterior parts of the right thalamus. She showed AHP and described her left side alternatively as her handicapped nephew and a clumsy cat. Misidentification of her daughter also occurred. With respect to the literature the predominant neuroanatomical features involved are lesions including right parietal cortex and/or posterior parts of the thalamus. Theories concerning the pathogenesis of this phenomenon comprise a denial of the illness, a lack of awareness caused by reduced sensory feedback and neglect, a misidentification or disturbance of the active discovery process considered necessary for realizing one's disorder.
    Notes: Zusammenfassung Anosognosie für Hemiplegie (AHP) im Sinne einer mangelnden Bewusstheit einer Hemiparese und assoziierter Störungen ist in Folge überwiegend rechtshemisphärischer Läsionen ein häufigeres Phänomen. Seltener wird kasuistisch über Patienten berichtet, die zusätzlich ein Fremdheitsgefühl kontraläsioneller Körperteile angeben und sie z. B. anderen Personen zuschreiben. Diese “Plusvariante” der AHP wird als Somatoparaphrenie (SP) bezeichnet. Wir schildern den Fall einer 85-jährigen Frau, die nach einem rechtshemisphärischen Infarkt der A. cerebri posterior und posteriorer Anteile des Thalamus eine SP entwickelte. Bei bestehender AHP äußerte sie abwechselnd, ihre linke Körperseite sei ihr körperbehinderter Neffe oder eine ungeschickte Katze. Passager traten auch Fehlidentifikationen der Tochter auf. In der Literatur werden überwiegend Läsionen unter Einschluss des rechten parietalen Kortex und/oder des rechten posterioren Thalamus angegeben. Erklärungstheorien umfassen u. a. psychodynamische Ansätze im Sinne einer Krankheitsverdrängung, ein Wahrnehmungsdefizit durch einen mangelnden sensorischen Feedback und Neglekt, eine Fehlidentifikation, sowie eine Beeinträchtigung eines zur Bewusstwerdung körperlicher Störungen notwendigen aktiven Aufdeckungsprozesses.

Die Ausbildung der Spiralarterien, denen wir im Hinblick auf die hämochoriale Placentation des Menschen eine entscheidende Bedeutung beimessen, läßt sich bei der mit Oestradiol vorbehandelten Kastratin durch 200 mg Progesteron intramuskulär, verteilt auf 10 Injektionen in 10 Tagen, erzielen. Man kann bei der Kastratin mit der gleichen Progesteronmenge die Ausscheidungswerte für Pregnandiol und die biphasische Temperaturkurve des normalen Cyclus gerade erreichen.
