    Notes: Trotz der weit verbreiteten Karzinomvorsorge stellt das Zervixkarzinom in den USA und den westlichen Industrienationen immer noch den vierthäufigsten Tumor bei Frauen dar. Die Anzahl von Neuerkrankungen beträgt ca. 14 000 neue Fälle pro Jahr. In unterentwickelten Ländern ist das Zervixkarzinom weit verbreitet und oft der häufigste Tumor [18]. Neben dem primär chirurgischen Vorgehen stellt die Strahlentherapie die wesentliche kurative Behandlungsmodalität dar, wobei die 5-Jahres-Überlebensraten nach Therapie bei 90% für das Stadium I und 7% für das Stadium IV liegen (FIGO Annual Report). Es erscheint daher sinnvoll, nach therapeutischen Konzeptionen zu suchen, um durch neue Kombinationen der Bestrahlung mit einer System- therapie die gegenwärtigen Ergebnisse weiter zu verbessern.

    Notes: Die Transplantat-Gegen-Wirt-Reaktion oder Graft-Versus-Host-Disease (GVHD) als Ausdruck der Immunreaktion zwischen Spenderlymphozyten und als „fremd” erkanntem Empfängerorganismus ist eine typische Komplikation z. B. nach allogener Knochenmarktransplantation [39]. Als verwandtes Krankheitsbild ist die seltene sogenannte Transfusions-assoziierte-Graft-Versus-Host-Erkrankung (TAGVHD) beschrieben. Voraussetzung für das Auftreten einer TAGVHD ist die Übertragung von teilungsfähigen allogenen Spender-Lymphozyten durch eine Transfusion in den Empfänger, welcher auf Grund seiner Immunitätslage nicht in der Lage ist, diese fremden Lymphozyten zu eliminieren. Die klinische Symptomatik ähnelt dem Bild einer GVHD mit den typischen Veränderungen an Haut, Darm und Leber [36, 37]. In Abhängigkeit von der Grunderkrankung und dem Allgemeinzustand ist die Letalität der TAGVHD trotz der modernen Möglichkeiten der Immunsuppression hoch und wird in der Literatur zwischen 75% und 90% angegeben [24]. Während lange Zeit vermutet wurde, daß das Krankheitsbild nur im Rahmen einer Hochdosistherapie nach peripherer Stammzelltransplantation oder auch nach allogener Knochenmarktransplantation (KMT) auftritt, ist heute bekannt, daß generell alle Blutempfänger mit mittelgradiger oder schwerer Immunsuppression sowie auch Empfänger von nur partiell HLA-indentischem Blutprodukten durch diese Komplikationen gefährdet sind. Hierzu zählen z. B. auch Organempfänger von Herz- oder Nierentransplantaten, Patienten mit malignen Lymphomen (auch nach konventioneller Chemotherapie), Frühgeborene oder Empfänger von Transfusionen unter Verwandten. Aufgrund der unbefriedigenden therapeutischen Möglichkeiten kommt der Prävention der TAGVHD in der Klinik eine besondere Bedeutung zu. Angestrebt wird vor der Transfusion eine möglichst komplette ▸Depletion teilungsfähiger Lymphozyten in der Transfusion, wofür in der Klinik die ex-vivo-Bestrahlung der Blutprodukte mit ionisierenden Strahlung Anwendung findet [16].

    Description / Table of Contents: Summary Although carcinoma screening is now widely available, cervical carcinoma is still the fourth most common tumour among women in the USA and in the western industrialized nations. There are about 14,000 new cases every year. In underdeveloped countries carcinoma of the cervix is widespread and often the most frequent tumour[18]. Besides primarily surgical treatment radiotherapy is the main curative treatment modality, the 5-year survival rates after therapy being around 90% for stage I disease and 7% for stage IV (FIGO Annual Report). It therefore seems desirable to look for therapeutic concepts, with the aim of improving the present results further by means of new combinations of irradiation with a systemic therapy.

    Notes: Abstract We present infrared imaging and photometry of the bright, giant comet C/1995 O1 (Hale-Bopp). The comet was observed in an extended infrared and optical observing campaign in 1996–1997. The infrared morphology of the comet was observed to change from the 6 to 8 jet “porcupine” structure in 1996 to the “pinwheel” structure seen in 1997; this has implications for the position of the rotational angular momentum vector. Long term light curves taken at 11.3 μm indicate a dust production rate that varies with heliocentric distance as ∶ r−1.4. Short term light curves taken at perihelion indicate a rotational periodicity of 11.3 hours and a projected dust outflow speed of ∶ 0.4 km s−1. The spectral energy distribution of the dust on October 31, 1996 is well modeled by a mixture of 70% silicaceous and 30% carbonaceous non-porous grains, with a small particle dominated size distribution like that seen for comet P/Halley (McDonnell et al., 1991), an overall dust production rate of 2 × 105 kg s−1, a dust-to-gas ratio of ∶5, and an albedo of 39%.

    Notes: Summary We retrospectively analysed 13 patients (pts.) treated at the University of Tübingen from 1985 to 1993 to evaluate the results of radiation therapy (XRT) given as an adjuvant to totally or subtotally resected meningiomas. The overall survival was 38% at five years with a probability of relapse of 50% at this time. Reclassification of the tumours according to the new WHO-classification of brain tumours [14] revealed 10 grade-II-tumours (atypical meningioma) and 3 grade-III-tumours (anaplastic meningioma). Radiotherapy failed in all 3 pts. with macroscopically incomplete resection (Simpson's grade IV), who died with relapse between 4 and 51 months after radiotherapy. 5 out of 10 pts. with grade-II-tumours relapsed. All 3 pts. with grade-III-tumours died with relapse between 6 and 21 months after XRT. Morbidity was seen in 2 pts. after irradiation with 60 GY (ICRU dose specification). Complete surgical exstirpation offers the best possibility of tumour control. Grade-III-tumours should be irradiated whatever the extent of the primary surgery was. Our results might indicate a possible indication for XRT in pts. with atypical grade-II-tumours especially when radical surgery must be in doubt. Prospective multicentre trials are warranted to prove the prognostic value of the new WHO-classification for atypical and anaplastic meningiomas and to define the ultimate role of radiotherapy in this setting.

    Notes: Summary The clinical importance of selection of different antihypertensive drugs for the treatment of diabetic patients is still unclear. Thus we performed a randomised, controlled study in 105 hypertensive non-insulin-dependent diabetic (NIDDM) patients with microalbuminuria over 1 year. Patients received either the angiotensin converting enzyme (ACE) inhibitor ramipril (2.5–5.0 mg/day; in addition 24 % of patients also received felodipine) or the beta blocking agent atenolol (50–100 mg/day; in addition 24 % of patients also received hydrochlorothiazide). Blood pressure, metabolic control, lipid levels and albumin excretion rate were studied during the follow-up. After 1 year an almost identical fall (p 〈 0.001) in blood pressure was observed with ramipril (170/100 vs 150/85 mmHg, median) and atenolol (180/100 vs 150/80 mmHg, median). With ramipril a reduction of total cholesterol (6.3 vs 5.9 mmol/l), of LDL cholesterol (3.8 vs 3.6 mmol/l) and HDL cholesterol (1.3 vs 1.2 mmol/l) was found, whereas triglycerides slightly increased (1.8 vs 2.0 mmol/l). With atenolol a similar reduction of total cholesterol (6.3 vs 5.9 mmol/l), LDL cholesterol (3.8 vs 3.7 mmol/l) and HDL cholesterol (1.4 vs 1.2 mmol/l) and an increase of triglycerides (1.4 vs 1.7 mmol/l) was noted. Metabolic control of the patients was maintained with both ramipril and atenolol treatment. With ramipril treatment urinary albumin creatinine ratio (14.4 vs 13.8 mg/mmol) and creatinine clearance (82 vs 84 ml/min) were constant, but with atenolol an increase of albumin creatinine ratio (13.9 vs 19 mg/mmol, p 〈 0.001) and a slight decrease of creatinine clearance (80 vs 66 ml/min, p 〈 0.05, not significant after Bonferroni correction) was observed. In conclusion: 1-year treatment of NIDDM patients with ramipril or atenolol does not influence metabolic control, the changes in serum lipids were similar. Despite almost identical blood pressure reduction in both groups the albumin creatinine ratio was constant under ramipril, but increased under atenolol treatment. [Diabetologia (1996) 39: 1611–1616]

    Notes: Abstract The shift in the nuclear magnetic resonance (NMR) line of 71Ga isotopes in GaAs crystals is investigated in the temperature range 160–360 K. It is observed that the position of the resonance line is temperature-independent. The value of the shift relative to an isolated nucleus is 440 and 420 ppm for the three samples investigated. The roles of various contributions to the NMR line shift are discussed.

    Notes: Abstract  A quantitative fingerprinting of automotive lubricants with respect to zinc dialkyl dithiophosphates (ZDP), major anti-wear/antioxidant additives, is presented. ZDPs in lubricant solutions are converted into the corresponding methyl and p-nitrobenzylic esters, respectively. After removal of the lubricant matrix the methyl esters are submitted to gas chromatography (GC). Mass spectrometric detection (MS) and comparison with reference methyl esters enable the characterisation of practical ZDP mixtures with respect to alkyl chain length and isomery of the single components. Overall recovery rates are higher than 90% and phosphorus-selective detection (NPD) allows a quantitative determination down to 0.1 pg/μl. The p-nitrobenzylic esters may be analysed by HPLC. Identification and quantification is performed by on-line HPLC-UV and HPLC-MS (APCI) with a determination limit of 20 pg/μl. The ZDP quantification via the methyl esters is applied to seven lubricants from the German market. The method is applicable to used oils allowing the monitoring of ZDP consumption during engine operation.
