    Notes: Summary In the rhesus monkey, cat and rat, pial arteries give off branches which run vertically through all three layers of the cerebellar cortex. The large cortical arteries are surrounded by a perivascular space in the molecular layer. Their wall consists of several layers of smooth-muscle cells and the luminal endothelium. As the arteries reach the deeper layers of the cerebellar cortex, the number of smooth-muscle cells is reduced. In the rat, sometimes no smooth-muscle cells are detectable in the preterminal arterial vessels. If these deep arteries branch off by dichotomy of terminal vessels there occurs a gradual or complete loss of myocytes in all three species. In the cat, where cortical arteries give off branches at rightangles, there is a sphincter-like accumulation of smooth-muscle cells at the opening to the smaller branch. The postterminal vessels and veins in all species exhibit the smae mural structure found in capillaries. The wall consists only of an endothelium and occasional pericytes embedded in the basal lamina. Even the large veins which run to the pial veins show this simple mural structure.

These paired vessels, consisting of one artery and one vein, are enveloped by a common glial sheath. In some Virchow-Robin's spaces two arteries and two veins can be observed. The physiological significance of this morphological feature is briefly discussed.

    Notes: Abstract  We treated 500 patients with high-dose chemotherapy (HDC) and autologous bone marrow (ABMT) or autologous peripheral blood stem cell transplantation (PBSCT). Treated conditions included leukemia, lymphomas, breast cancer, lung cancer, germ-cell carcinomas, and other solid tumors. In order to assess relapse of primary malignancy or occurrence of new neoplasms, routine screening after ABMT or PBPCT was performed at regular and close intervals. With a total follow-up of 1358 person-years and a median follow-up of 34 months (range 9–91), 10/500 (2%) patients developed second malignancies after PBSCT or ABMT; i.e., one new cancer occurred every 136 person-years. All malignancies were detected at routine follow-up examinations; and 7/10 diagnoses were made in an asymptomatic phase; 6/10 neoplasms were amenable to complete surgical resection, five of which remain in CR at a median of 23+ months after autotransplantation. We conclude that regular and close follow-up examination of patients after autologous hematopoietic stem cell transplantation may be beneficial, since successful treatment of second malignancies is possible in selected cases after early detection.

    Notes: Abstract  Dendritic cells (DCs) are professional antigen-presenting cells (APCs) specialized to internalize, process, and present antigen. They have the capacity to stimulate the primary immune response of resting T-cells. We generated DCs from the adherent cell fraction of peripheral blood, as well as from purified CD34+ cells from CML patients. Characterizing DCs from ten CML patients by flow cytometry, we found that these cells are highly positive for HLA-DR, CD1a, CD23, and CD80 and negative for CD14, CD15, and CD16. The yield of DCs ranged from 19.5 to 68%. In addition, we used a functional test of FITC-dextran uptake to verify that early DCs take up large particles (0.5–3 μm) by macropinocytosis while monocytes do not. FITC-dextran uptake was detected by flow cytometry, showing that DCs had accumulated these fluorescent particles. Electron-microscopic analysis showed no major morphological differences between normal and CML-derived DCs. Furthermore, cultured DCs were isolated by FAC sorting for CD1a and HLA-DR expression. In these highly purified cells the Ph chromosome was detected by interphase fluorescence in situ hybridization (FISH) and by fluorescence immunophenotyping and interphase cytogenetics as a tool for the investigation of neoplasms (FICTION); 30–85% of DCs generated were Ph-chromosome positive. It might therefore be possible not only to prime T-cells with bcr/abl-specific synthetic peptides, but also to stimulate T cells directly with Ph-positive DCs. Use of DCs might serve as a novel therapeutic approach in CML patients, due to their ability to induce highly specific T-cell responses in an autologous system.

    Notes: Abstract  Treatment-related (Tr) AML and MDS after chemotherapy, radiotherapy, or the combination of both have been well characterized. However, tr-CML seems to differ from these better-known entities in frequency, clinical course, and prognosis. Tr-CML cannot be distinguished from de novo CML cytogenetically, and, in contrast to tr-AML and tr-MDS, typical chromosomal aberrations related to tr-CML have not been described. Treatment-related CML is a late effect of cytotoxic or immunosuppressive therapy which might be increasingly recognized due to a higher number of patients treated with intensive therapy regimens. We review here the available data on incidence of tr-CML as well as the affected individual's characteristics with regard to different treatment options in malignant and nonmalignant diseases.

    Notes: Abstract  We used synthetic RNA transcripts to prove the cleavage capability of ribozymes targeted against bcl-2-related RNAs. No cleavage occurred when control oligonucliotides were used. To assess the functional role of the specific ribozymes in chronic myelogenous leukemia (CML) cell lines we cultured K562, BV173, and Daudi cells for 48 h after lipofection with 10 μM oligonucleotide. An increase in apoptotic cells, dependent on ribozyme specificity, was shown in BV173 cells.This finding was underlined by the typical morphological changes, but there is no correlation with regard to the level of bcl-2 protein expressed. Though bcl-2 appears to interfere with cell death in myeloid cells, bcl-2-targeted ribozymes do not induce programmed cell death (PCD) by reducing bcl-2 protein levels, but rather by a presently unknown mechanism.

    Notes: Abstract We first show how to formalize environments in which situations can be changed only by action, and in which a situation should be brought about that is acceptable. This leads us to the concept of a problem constellation that consists of an action system, a reaction system, a judgement system, and an initial situation. We then present a cybernetical system which we call Robbimat. It is capable of testing problem constellations, of modifying them according to the results of the tests, and of judging the final constellation. We designed Robbimat in order to have favourable problem constellations automatically generated. As an example we discuss a simple game. It constitutes a win-loss-environment the structure of which can be deduced from the payoff function. We show how Robbimat analyses such an environment, and utilizing its structure classifies the initial situation.

    Notes: Abstract Background: We conducted a phase I–II trial to assess the activity of standard-dose (SDC) and high-dose chemotherapy (HDC) with etoposide, ifosfamide, cis/carboplatin, and epirubicin (VIP-E, VIC-E) in 107 patients with limited-stage (LS, stage I–IIIB) and extensive stage (ES, stage IV) non-small-cell lung cancer (NSCLC). Patients and methods: Updated results of a previously published trial are presented. Results: Response rates and survival after VIP-E were comparable to those of other standard-dose combination chemotherapies in NSCLC. Treatment-related mortality (TRM) in SDC was 3% in LS-NSCLC, and 8% in ES-NSCLC. TRM was 4% in patients selected for HDC by response rate and performance score. Five-year survival in LS-NSCLC was 12% after SDC, and 18% after HDC; it was 0% for both treatment protocols in ES-NSCLC. Conclusions: The activity of VIP-E SDC and VIC-E HDC is not superior to that of established protocols in the treatment of NSCLC. In view of the toxicity and TRM associated with this protocol, less aggressive regimens should be preferred for most patients. Whether selected patients with chemosensitive disease could benefit from VIP-E SDC and/or VIC-E HDC in an adjuvant or neo-adjuvant setting could not be determined within the scope of this study.
