    Description / Table of Contents: Zusammenfassung Bei angeborenen Enzymdefekten ist das Vorhandensein von organspezifischen Isoenzymen des betreffenden Enzyms wichtig für die Ausbreitung der Funktionsstörung im Organismus. Die Ergebnisse der Elektrophorese, der Bestimmung der Substrataffinität und der Thermoinaktivierung der GPI in menschlichen Organextrakten lieferten keinen Anhaltspunkt für ein organspezifisches Isoenzymsystem. Die Verteilung der GPI-Aktivität in verschiedenen postmortal und intravital entnommenen Gewebsproben ist relativ gleichförmig mit Ausnahme der Erythrocyten.
    Notes: Summary The occurrence of tissue-specific isoenzymes is important for the distribution of the disease in enzyme deficiencies. In the case of GPI our results of starch gel electrophoresis, substrate affinity measurements and thermoinactivation gave no evidence for the occurrence of tissue-specific isoenzymes of GPI in man. GPI activity was in the same order of magnitude in various postmortal and intravital samples of human tissues, except in erythrocytes.

Unter den immunologischen in vitro Parametern findet sich lediglich ein leichter Abfall der relativen T-Zell-Konzentration, jedoch ohne Beeinträchtigung der T-Zellfunktionen. Eine Leukozytose die während der Infrarot-induzierten Hyperthermie auftritt, wird auf das Hitze-Erythem zurückgeführt. Über Indikation oder Nutzen einer solchen Behandlungs-Maßnahme kann jedoch noch nichts ausgesagt werden.
    Notes: Summary There are no signs of organ lesions in healthy subjects exposed to an 1-hour 40° C whole-body hyperthermia, induced by radiofrequency or infrared light. As to the immunological in vitro parameters, only a slight decrease of the relative T-cell count is seen, T-cell functions however being normal. A leukocytosis appearing during infrared-induced hyperthermia is probably related to the erythema caused by skin heating. About possible indications and clinical values of this treatment, nothing can however be said so far.

    Notes: Summary The significance of proteinuria during febrile infectious diseases is widely underestimated, although the more marked proteinuria probably signalizes a parainfectious nephropathy rather than a functional disorder. This study shows that mild proteinuria of less than 0.65 g/24 h (normal range less than 0.3 g/24 h using the sensitive tannine-FeCl3-technique) might be caused by the elevated body temperature alone. 9 out of 18 volunteers without renal disease undergoing experimental hyperthermia of 40–41° C for 1–2 h did not develop a proteinuria according to quantitative and qualitative (SDS-PAGE) measurements. In 6/18 the amount and composition of urinary proteins changed giving a glomerular type of proteinuria, possibly caused by temperature related transient glomerular alterations. In 3/18 a mild glomerulopathy existed before hyperthermia, as deduced from a glomerular pattern despite a quantitatively physiological proteinuria, leading in all 3 to pathological proteinuria during hyperthermia. In all 18 volunteers alterations reversed to normal within 12 h. Therefore, the degree of proteinuria during febrile diseases should be considered. Proteinuria of less than 0.5–1 g/24 h in adults might be explained by an altered glomerular function alone. Proteinurias exceeding this value, with a slow regressing tendency will indicate glomerular or tubulo-interstitial diseases, caused possibly by immunologic or toxic products resulting from underlying infectious disease.

    Notes: Summary Thrombocytopenia developed in a 21-year-old patient with sarcoidosis when corticosteroid therapy was discontinued. Platelet-associated IgGs were elevated. High-dose cortisone did not affect the thrombocytopenia. However, platelet counts remained normal for more than 1.5 years following intravenous treatment with a single course of human gammaglobulins.

    Description / Table of Contents: Zusammenfassung Bei einem 9jährigen Mädchen deutscher Abstammung fand sich eine hereditäre Glucosephosphat-Isomerase-Defizienz. Die Restaktivität in den Erythrocyten war 15%, in den Thrombocyten 33% und in Leukocyten 35%. Physikalisch-chemische Untersuchungen des defekten Enzyms ergaben eine stark verminderte Thermostabilität mit einer biphasischen Inaktivierungskurve. Bei der Stärkegelelektrophorese fand sich eine Bande mit einer Wanderungsgeschwindigkeit von 88% im Vergleich zur normalen Hauptbande; der isoelektrische Punkt war auf pH 9,60 erniedrigt. Das gelchromatographisch bestimmte Molekulargewicht war mit 92 000 im Normbereich. Das defekte Enzym wurde aus den Erythrocyten der Patientin mit einer Ausbeute von 26% 30 000fach angereichert. Die kinetischen Eigenschaften (Km für G-6-P, Km für F-6-P, Vmax der Vor- und Rückreaktion, pH Optimum der Vor- und Rückreaktion, sowie die Inhibitorkonstante für 2,3-DPG der Rückreaktion und die Ki für 6-PG der Vorund Rückreaktion) wichen nur unwesentlich vom Verhalten des Normalenzyms ab. Stoffwechseluntersuchungen ergaben eine Erhöhung von G-6-P auf 90 nMol/ml Erythrocyten der Patientin. ATP war leicht erhöht. Die Lactatbildung der defizienten Erythrocyten war im Vergleich zu einer Normalzell-population gesteigert und im Vergleich zu einer reticulocytenreichen Kontrolle normal. Eine Alterung der Patientenerythrocyten in vitro ergab eine leicht beschleunigte Inaktivierung der defekten GPI in den Zellen. Die Lactatbildung sank am 5. Tag der Alterung bei Verwendung von Glucose als Energiequelle auf 47% der Kontrollzellen ab. Gleichzeitig war die Hämolyse doppelt so hoch wie bei den Kontrollzellen. Bei Verwendung von Mannose als Energiequelle, wobei der Enzymblock umgangen wird, war am 5. Tag die Lactatbildung nicht eingeschränkt. Daraus ergibt sich die Bedeutung des GPI-Defektes für die Stoffwechsel-beeinträchtigung der Erythrocyten und die Verkürzung ihrer Lebenszeit.
    Notes: Summary Hereditary deficiency of glucose phosphate isomerase (GPI) was observed in a 9-year old gire of German origin. Residual enzyme activity in red cells was 15%, in platelets 33%, and in leukocytes 35%. The patient is suffering from severe non-spherocytic haemolytic anaemia. Splenectomy did not bring about a remarkable clinical improvement. Physico-chemical studies of the deficient enzyme showed a significantly decreased thermostability with a biphasic inactivation curve. Starch gel electrophoresis revealed only one zone of activity; the migration was 88% compared to the normal main zone. The isoelectric point was decreased (pH 9,60), determined by gel chromatography the molecular weight was within the normal range (92 000). The defect enzyme was purified from red cells 30 000-fold with a yield of 26%. The kinetic properties (Km for G-6-P, Km for F-6-P, Vmax of foreward and backward reaction, pH optimum of the foreward and backward reaction, inhibitor constants for 2.3-DPG and 6-PG of the foreward and backward reaction) were similar to those of the normal enzyme. Metabolic studies demonstrated a markedly increased G-6-P concentration of the erythrocytes (90 nMol/ml RBC); ATP was slightly elevated. Lactate production from glucose was increased when compared to normal RBC and was in the normal range when compared to RBC samples with comparable content of reticulocytes. Aging of the deficient RBC was simulated in vitro. The activity of the defect enzyme decreased slightly faster within the patient's red cells than within normal RBC during the experiment. By the 5th day of aging lactate production was only 47% compared to control cells. Simultaneously, haemolysis of 15% was observed in the deficient cells compared to only 8% in the control cell suspension. Using mannose as a source of energy bypassing the metabolic block, the rate of lactate production was still normal at the 5th day of the aging period which reflects the cause and effect relationship between impairment of energy metabolism and GPI deficiency.

    Description / Table of Contents: Zusammenfassung Bei 14 Patienten mit Tumoren in generalisierten Stadien (Morbus Hodgkin IV, Kleinzelliges Bronchial-Carcinom, Fibrosarkom, Akute lymphatische Leukämie) wurde insgesamt 54mal die zytostatische Behandlung in Kombination mit Ganzkörperhyperthermie durchgeführt. Die Temperaturerhöhung wurde durch Mikrowellen einer Frequenz von 27 MHz erzeugt. Die gewünschte Temperatur von 40° C wurde nach 40 min erreicht. Zu diesem Zeitpunkt erfolgte die Zytostatikaapplikation; danach wurde die rectal kontrollierte Temperatur von 40–40,5° C noch für 1 h lang aufrechterhalten. Komplikationen von seiten der Lunge, des Herzens und des Kreislaufs traten nicht auf. Durch Kontrolle organspezifischer bzw. funktionsspezifischer Laborparameter konnten Veränderungen an Leber, Niere, Elektrolythaushalt, Muskulatur und Blutzellen ausgeschlossen werden. Die Laborkontrollen erfolgten vor, während, unmittelbar nach und 24 h nach der Hyperthermiebehandlung. Es ergaben sich keine Hinweise auf eine Verstärkung der zytostatika-typischen Toxizität. Die Ergebnisse werden mit den Ergebnissen anderer Gruppen verglichen. Der therapeutische Effekt des kombinierten Vorgehens ist zum derzeitigen Zeitpunkt noch nicht beurteilbar.
    Notes: Summary 14 patients with tumors in generalised stages (Hodgkin's disease, oat-cell-carcinoma, Fibrosarcoma, acute leukemia) were treated altogether 54 times with cytostatics in combination with hyperthermia. The temperature was induced by microwaves with a frequency of 27 MHz. A temperature of 40° C was reached after 40 min. At this point cytostatics were applicated; afterwards the temperature was maintained for one hour at 40–40,5° C. Cardiac, pulmonary and circulatory complications did not occur. Controls of the laboratory parameters could exclude effects on electrolytes, muscles, blood, liver and kidney. The laboratory controls were made before, during, immediately after and 24 h after hyperthermia. There were no signs for an enhancement of toxicity typical for cytostatics. The observations are compared to the results of other investigators. To date the therapeutic effect of this treatment can not be stated.

    Notes: Summary The case of a 57-year-old patient with a plasmablastic lymphoma is described. The primary diagnosis was an ordinary plasmacytoma, type IgG kappa. The final course of the disease involved almost all of the organs. The clinical aspect was determined by multiple skin infiltrations. The present report has in common with other published cases the fact that multiple secondary skin involvement of immunoglobulin-secreting tumors, regardless of their origin, have a very poor prognosis. In comparison, primary multiple cutaneous immunoglobulin-secreting tumors have a better prognosis.

    Notes: Abstract We treated 500 patients with high-dose chemotherapy and autologous bone marrow or autologous peripheral blood stem cell transplantation. Treated conditions included leukemia, lymphoma, breast cancer, lung cancer, germ-cell carcinoma, and other solid tumors. 10/500 (2%) of patients were treated for a second malignancy diagnosed 12 months to 25 years after their initial neoplasm. Four of these ten patients are in complete remission (CR) of both malignancies at a median follow-up of 29+ months after high-dose chemotherapy and autotransplantation. None of these patients would have been eligible for high-dose chemotherapy and autotransplantation by conventional selection criteria which usually exclude patients with a history of prior malignancies. Conclusion. Conventional exclusion criteria for high-dose chemotherapy and autotransplantation may not adequately reflect the prognosis of patients with second or secondary malignancies treated with this therapeutic modality. High-dose chemotherapy and autologous hematopoietic stem cell transplantation may be of true benfit in selected cases of secondary malignancies.
