    Notes: Abstract Background: In an effort to intensify osteosarcoma therapy, systemic ifosfamide was added pre- and postoperatively to an already aggressive three-drug regimen. In a subgroup of patients, loco-regional treatment intensification was attempted by using the intraarterial route to give cisplatin. Patients and methods: Patients ≤40 years at diagnosis of a localised, de novo high-grade central extremity osteosarcoma were eligible for inclusion into study COSS-86 if registered within three weeks from biopsy. Doxorubicin, high-dose methotrexate, and cisplatin were given to all patients. Patients who fulfilled one or more of three defined high-risk criteria received early systemic treatment intensification by adding ifosfamide as the fourth agent. Preoperatively, these high-risk patients received cisplatin either intraarterially or intravenously. Results: 171 eligible patients were entered, of which 128 were stratified into the high-risk group. When all 171 were analysed by intention-to-treat, actuarial overall and event-free survival rates at ten years were 72% and 66%, respectively. No benefit of intraarterial cisplatin application was detected. Cumulative treatment toxicity was considerable. Conclusions: In a multicenter setting, intensive treatment of osteosarcoma according to protocol COSS-86 led to long-term disease-free survival for two thirds of patients. We saw no benefit of using the intraarterial route to administer cisplatin.

    Notes: Abstract The singularity manifold equation of the Kadomtsev-Petviashvili equation, the so-called Krichever-Novikov equation, has an exact linearization to an overdetermined system of partial differential equations in three independent variables. We study in detail the Cauchy problem for this system as an example for the use of the formal theory of differential equations. A general existence and uniqueness theorem is established. Formal theory is then contrasted with Janet-Riquier theory in the formulation of Reid. Finally, the implications of the results for the Krichever-Novikov equation are outlined.

    Notes: Retroperitoneale Sarkome gehören mit einer Häufigkeit zwischen 0,1 und 0,2% aller diagnostizierten Tumoren zu den seltenen Tumoren. Der nur nach ventral und dorsal gut begrenzte Retroperitonealraum ermöglicht Sarkomen lange Zeit ein fast uneingeschränktes Wachstum. Dementsprechend verläuft die klinische Symptomatik sehr lange unspezifisch und unerkannt. Des öfteren werden diese Tumoren erst durch sekundäre Komplikationen am Gastrointestinaltrakt oder Retroperitoneum diagnostiziert.

    Notes: Innerhalb der letzten Jahre wurden umfassende neue Daten zur Pathogenese des Magenkarzinoms erarbeitet. Zum einen haben molekularbiologische Untersuchungen das Verständnis von Karzinogenese und Tumorprogression stark erweitert, und es liegen erste Hinweise für prognostisch relevante molekulare Marker vor. Andererseits wurde der prognostische Wert bekannter histopathologischer Tumormerkmale in groß angelegten klinisch-pathologischen Studien weiter abgesichert. Dieser Artikel soll einen Überblick über etablierte pathomorphologische sowie einige versprechende tumor- biologische Prognosefaktoren beim Magenkarzinom geben.

    Notes: Abstract The need for contralateral inguinal exploration in children during unilateral inguinal hernia repair remains controversial. We questioned whether an elective preoperative sonographic examination of the contralateral inguinal canal was accurate enough to prevent unnecessary exploration of the asymptotic side. Sonography (US) of the groins was performed randomly in 200 out of 600 patients awaiting unilateral hernia repair over a 1.5-year period during 1990–1991. The patients and their medical records were reviewed in 1994, 3–4 years following surgery. Four hundred infants and children underwent unilateral inguinal hernia repair without preoperative US, based on the clinical diagnosis (group I). Of the remaining 200, 160 (group II)_ had the clinical diagnosis confirmed by US. Forty patients with US evidence of a contralateral hernia or hydrocele (group III) were excluded from the study. At follow-up after 3–4 years, 26 patients in group I presented with a hernia in the opposite groin 2 weeks to 2 years following surgery. The incidence of a subsequent contralateral hernia in patients under 1 year, 1–2 years, and older than 2 years was 7.3%, 9.6%, and 4.5%, respectively. None of the patients in group II presented with a clinically evident hernia during the follow-up period. These findings suggest that US is a readily available, non-invasive, and accurate method of preoperative evaluation of the groins. Our policy of contralateral exploration based on the US findings was shown to be reliable in preventing unnecessary surgical exploration of the unaffected inguinal canal.

    Notes: Abstract  The 20q13 region harboring recently described putative oncogenes is frequently amplified in invasive ductal carcinoma (IDC). The aim of this study was to examine the 20q13 copy number in intraduct hyperplasia (IH), atypical duct hyperplasia (ADH), and ductal carcinoma in situ (DCIS) adjacent to IDC. In 5 patients, comparative genomic hybridization (CGH) after laser microdissection revealed 20q13 amplification in four of five cases of IH, in all of three cases of IH with atypia, all five of DCIS, and all five of IDC. Fluorescence in situ hybridization (FISH) confirmed the amplification at 20q13.2 in IH in the two specimens analyzed. The amplification rate, however, was higher in DCIS and IDC. In phenotypically normal ductal epithelium normal values were found for 20q13 copy number by FISH (n=2) and CGH (n=5). Although the number of cases presented here is small, our results suggest that mutations in the 20q13.2 region in IH may be associated with accelerated proliferation and hyperplasia of the ductal epithelium. Progression to DCIS and ICD is accompanied by a further increase in the 20q13.2 copy number.

    Notes: Abstract Conventional cytogenetic studies revealed gains and structural aberrations of chromosome 1 to be the most consistent chromosomal aberrations in hepatocellular carcinoma (HCC). We investigated touch preparations of eight HCC, five cholangiocellular carcinomas (CCC), five liver cell adenomas (LCA), four focal nodular hyperplasias (FNH) as well as nine specimens of normal liver tissue using fluorescence in situ hybridization (FISH) with centromere specific probes for chromosomes 1 and 8. Polysomies of chromosome 1, especially trisomy 1, were found in five of eight HCC and four of five CCC but in no normal liver tissue or benign tumour. Only three of seven cases of HCC revealed trisomy 8 whereas the five benign liver tumours and all normal liver tissues examined had disomy 8. Our results confirm conventional cytogenetic findings in terms of chromosome 1 aberrations in HCC although they are not specific for these types of malignant liver tumours. Since α-satellite probes were used in our study, only gains or losses including the centromeric regions of the chromosomes 1 and 8 could be detected. Nevertheless, our findings suggest that FISH may help in the differential diagnosis of malignant versus benign neoplasms of the liver.
