    Notes: Abstract. Heat shock genes are found in all organisms, and synthesis of heat shock proteins is induced by various stressors in nearly all the cells forming these organisms. However, a particular situation is noticed for hsp70 genes in mouse embryos at the beginning of their development. First, spontaneous expression of hsp70 is observed at the onset of zygotic genome activity. Second, inducible expression is delayed until morula or early blastocyst stages. A better understanding of both these points depends on a more careful analysis of hsp70 expression in relation to their major regulators, the heat shock factors. In this review, we will see how the development of the preimplanta tion embryo highlights the complexity of heat shock gene regulation involving trans-cis interactions and the cellular and nuclear environment.

    Description / Table of Contents: Summary The rarely occurring bilateral CCAM has a poor prognosis. Case Report: At birth boy E. presented with respiratory distress. His bilateral severe CCAM was misdiagnosed until the typical bullous x-ray pattern developed within the first few days of life. His condition deteriorated despite conservative treatment. Facing respiratory and cardiac decompensation he underwent unilateral pneumonectomy. The diagnosis was histologically confirmed. A refillable balloon was implanted to prevent mediastinal shifting. Boy E. stabilized gradually and came off both CPAP and supplementary oxygen. At age 1 y, during a course of severe pneumonia he died of sudden cardiac arrest. Discussion: According to the literature, bilateral severe CCAM unequivocally leads to death within days or weeks. In contrast, our patient improved gradually after surgery. But, CCAM bears an increased risk of cardial decompensation in infancy which surgery can reduce but not eliminate. For the time being, in cases of severe bilateral CCAM there is no effective treatment modality other than surgery.
    Notes: Zusammenfassung Folgen der seltenen beidseitigen pulmonalen CCAM sind hochgradige Ateminsuffizienz und rechtskardiale Belastung mit Dekompensation typischerweise innerhalb weniger Lebenstage. Fallbericht: Unser Patient fiel als Neugeborenes durch respiratorische Insuffizienz auf. Eine beidseitige CCAM wurde initial als Atemnotsyndrom verkannt. Auch nach zügiger Diagnosestellung drohte unter konservativer Therapie die respiratorische und kardiale Dekompensation, weshalb wir uns zur einseitigen Pneumonektomie mit Einlage eines Ballonplatzhalters entschlossen. Die Histologie bestätigte unsere radiologische Diagnose. Im weiteren Verlauf zeigte sich zunächst eine Stabilisierung; der Patient war zeitweilig ohne Atemhilfe oder supplementären Sauerstoff. Mit 1 Jahr kam es zum Sekundenherztod im Rahmen einer schweren Pneumonie. Entgegen der kurzfristig infausten Literaturprognose berichten wir von einem zunächst guten Behandlungserfolg. Diskussion: In Analogie zur bronchopulmunalen Dysplasie ist ein bis zum 3. Lebensjahr wesentlich erhöhtes Risiko der kardialen Dekompensation anzunehmen. Die Operation kann dieses Risiko nur mindern, nicht ausschließen. In Anbetracht des – wenngleich temporären – Operationserfolgs halten wir therapeutischen Nihilismus bei beidseitiger CCAM für unangebracht.

    Notes: Abstract In vitro folding studies of several proteins revealed the formation, within 2–4 msec, of transient intermediates with a large far-UV ellipticity but no amide proton protection. To solve the contradiction between the secondary structure contents estimated by these two methods, we characterized the isolated C-terminal fragment F2 of the tryptophan synthase β2 subunit. In β2, F2 forms its tertiary interactions with the F1 N-terminal region. Hence, in the absence of F1, isolated F2 should remain at an early folding stage with no long-range interactions. We shall show that isolated F2 folds into, and remains in, a “state” called the pre-molten globule, that indeed corresponds to a 2- to 4-msec intermediate. This condensed, but not compact, “state” corresponds to an array of conformations in rapid equilibrium comprising native as well as nonnative secondary structures. It fits the “new view” on the folding process.

    Notes: Abstract To investigate the frequency and diagnostic significance of Amsler's sign in patients with Fuchs' heterochromic cyclitis (FHC), we performed a diagnostic AC puncture of the eyes of 31 patients with this disease and looked for the same sign in 104 patients with intermediate uveitis and 373 patients with various other types of uveitis who underwent the same procedure. AS was significantly more frequent in the FHC patients than in either of the other two groups of patients, and occurred infrequently in all other types of chronic uveitis. We therefore consider its occurrence to have important differential diagnostic value. We would also emphasize the pathophysiologic significance of AS and suggest that this aspect should be investigated in the future.

    Notes: Abstract Changes in local immunity are important whenconsidering the physiopathology of uveitis. The aim ofthis study was to measure IgG and IgG subclassconcentrations in the serum and the AH of patients withthree different types of uveitis and to determine for eachof them the presence of a local production of IgG in theintra-ocular compartment. This investigation was extendedto IgG subclasses.Serum and AH of 46 patients, including 11 with Fuchsheterochromic cyclitis (FHC), 13 with toxoplasmicchorioretinitis, 11 with herpetic uveitis and 11 with senilecataract (taken as controls) were analyzed by ELISA forIgG and IgG subclasses. Three quotients (r1, IgG/albuminin serum; r2, IgG/albumin in AH; and R, r2/r1) werecalculated in order to estimate the local synthesis (LS) ofIgG and each IgG subclass.In AH of patients with herpetic uveitis, a concomitant andsignificant increase of all IgG subclasses as well asalbumin (with no significant increase of r2 or R) wasobserved. This finding was interpreted as an indirectconsequence of major damage to the blood–aqueousbarrier. In patients with FHC, a significant increase of r2and R involving only the IgG1 subclass was observed,indicating the existence of LS of IgG1 in the majority ofthese patients. In the AH of patients with toxoplasmicchorioretinitis, no significant modification of IgG subclassor albumin concentrations was observed when comparedto controls.In conclusion, it would seem interesting to considermeasurement of IgG and IgG subclasses and calculationof the coefficients r1, r2 and R for a better evaluation ofthe local immunological processes observed in differenttypes of uveitis.
