    Notes: Abstract Objective: To assess the possibility of any clinically relevant pharmacokinetic interactions between tiagabine, a novel antiepileptic drug, and digoxin. Methods: Potential pharmacokinetic interactions between tiagabine and digoxin were investigated in an open-label, two-period cross-over study in healthy male volunteers. Thirteen volunteers, aged between 18 and 43 years, were randomised to receive digoxin (0.5 mg twice a day for 1 day, then 0.25 mg once a day for 8 days) either alone or co-administered with tiagabine (4 mg three times daily for 9 days). Following a 7-day wash-out period, volunteers crossed over to the other dosing regimen. Peak serum concentration, time to maximum serum concentration, area under the serum concentration–time curve from zero to 24 h and steady state serum concentration were calculated for digoxin and compared between treatment groups. Results: No statistically significant differences between treatment groups were observed for any of the derived digoxin pharmacokinetic parameters. The most common adverse events reported during digoxin alone and in combination with tiagabine were somnolence and headache; an overall greater frequency of adverse events was reported during combined treatment. Adverse events were generally mild in nature; no serious adverse events were reported. Conclusions: At the doses administered, there is no evidence of a pharmacokinetic interaction between digoxin and tiagabine in healthy male volunteers.

    Notes: Concerning the soft tissue reaction to stainless steel (316L) fibre mesh structures, it was found that mesh materials composed of small diameter fibres (2–8 μm) were lined by several layers of macrophages and giant cells. In addition, the majority of these mesh implants were filled with inflammatory cells. In contrast, mesh materials with a larger fibre diameter (12–22 μm) evoked less or no inflammatory reaction. An explanation for this observation could be the occurrence of shear displacements at the soft tissue-implant interface. In addition, corrosion phenomena as a result of the increased surface area of small fibre diameter meshes can be responsible for a greater toxic ion release. in this study the corrosion rates and the influence of shear stress on the corrosion rates of different stainless steel 316L fibre mesh structures were compared under in vitro laboratory conditions. It appeared that the effect of mechanical stress on 316L corrosion was a significant increase in the concentration of corrosion products in the loaded situation. Furthermore, the effect of fibre diameter on the release of corrosion products was a significant difference in ion release between mesh materials made of fibres with a diameter of 4 μm compared to meshes with a diameter of 12 and 22 μm. Therefore it was concluded that the chemical properties of fibre mesh implant material, together with the ability to release toxic ions, are an important determinant in the final tissue behaviour.

    Notes: Abstract A calciumphosphate cement, consisting mainly of tricalciumphosphate (85% α-TCP and 15% β-TCP), was inserted in 16 surgical defects created in the tibia of goats. X-ray diffraction (XRD) and energy dispersive spectroscopy (EDS) showed that after 3 months of implantation the α-TCP was transformed to hydroxyapatite (HA). Histological evaluation revealed that the presence of cement stimulated the ingrowth of bone compared with unfilled cavities. Active resorption and remodelling of cement particles was observed. The cement did not evoke an inflammatory reaction. At 6 months after implantation no further changes in the composition of the cement occurred. All remaining material was surrounded by mature bone.

    Notes: The bone response to different calcium phosphate (Ca-P) coated implants was evaluated in a goat animal model. Two types of plasma spray coatings were applied to a commercially pure titanium (cpTi) tapered, conical screw-design implant (BioComp®); hydroxyapatite (HA-PS) and a dual coating, consisting of FA and HA (FA/HA-PS). In addition an amorphous RF magnetron sputter coating (Ca-P-a) and uncoated implants were investigated. Forty-eight implants were inserted in the maxilla of 12 adult female goats. After implantation periods of 3 and 6 months, the bone implant interface was evaluated histologically and histomorphometrically. After both implantation periods all plasma spray coated implants were maintained. On the other hand three Ca-P-a and two cpTi implants were lost. Histological examination revealed a better bone response to both plasma spray coated implants. Histomorphometrical evaluation confirmed this finding. At 3 and 6 months significantly higher percentages of bone contact (p〈0.001, ANOVA) were measured for both plasma spray coated implants than for the cpTi and Ca-P-a implants, while no significant difference (p〈0.05) existed between both implantation periods. Degradation of both plasma spray coatings was observed. Supported by the results, it is concluded that, although Ca-P coatings can improve the performance of dental implants, the presence of a Ca-P coating is not the only important factor for bone healing around implants placed in low density trabecular bone.

    Notes: Abstract The tissue reaction to a percutaneous access device, applicable as a carrier for an implantable glucose sensor, was evaluated in goats. Titanium fibre mesh structures were used for anchorage of the device in superficial as well as deeper soft-tissue locations. The percutaneous part was subcutaneously anchored with a fibre mesh sheet. The distal part was placed intraperitoneally and anchored in deeper soft-tissue layers using a fibre mesh cuff. All implants showed good healing with the surrounding tissue. Histological evaluation showed that the subcutaneous fibre mesh sheets and peritoneal fibre mesh cuffs were filled with immature connective tissue, generally free of inflammation. Problems concerning disconnection of the silicone catheter from the titanium holding element and filling of part of the peritoneal fibre mesh cuff with silicone glue have to be overcome by more appropriate preclinical testing and improved implant design. Our results demonstrate that titanium fibre mesh structures can be used effectively for soft-tissue anchorage of percutaneous access devices. A sufficient ingrowth of connective tissue was obtained in superficial as well as in deeper soft-tissue layers. The access device could have application as a carrier for an implantable glucose sensor for glucose monitoring in different tissue compartments.

    Notes: Abstract Radio-frequency magnetron sputtering deposition was used to produce calcium phosphate sputter coatings with three different thicknesses (0.1, 1 and 4 μm) on titanium discs. Half of the as-sputtered coatings were subjected to an additional heat treatment for 2 h at 500°C. X-ray diffraction demonstrated that annealing at 500°C changed the amorphous 1 and 4 μm sputtered coatings into an amorphous–crystalline structure, while the amorphous 0.1 μm changed in a crystalline apatite structure. Further, scanning electron microscopy (SEM) inspection demonstrated that annealing of the 1 and 4 μm coatings resulted in the appearance of some cracks. The dissolution behaviour of these Ca–P coatings was determined in a simulated body fluid. It was found that after incubation for 4 weeks the dissolution was determined by the crystallinity of the deposited coating. SEM and Fourier transform infrared evaluation showed that all the heat-treated sputter coating appeared to be stable under the test conditions and a Ca–P precipitate was always deposited on the coating surface. On the other hand, the amorphous 0.1 and 1 μm coatings dissolved completely within 4 weeks, while the amorphous 4 μm coating showed only signs of surface dissolution. Scratch testing demonstrated that there is a linear correlation between the critical load, L c, and the coating thickness. A heat treatment for the CaP-4 coating resulted in an additional decrease in the critical load. On the basis of these findings, we conclude that already a 0.1 μm heat-treated Ca-P sputter coating is of sufficient thickness to show in-vitro adequate bioactive and adhesive properties.

    Notes: Abstract Four different implant materials were installed into the mandibular corner of goats to investigate the trabecular bone response in a mainly unloaded model. The implants were installed using a standardized technique and were left in situ for 12 weeks. One goat had to be sacrificed after surgery because of a broken rib; the other animals healed uneventfully. After sacrifice of the animals, the bone response to the uncoated and the three different Ca–P implants was evaluated histologically and histomorphometrically. Four sections of each implant were evaluated; two were located in the cortical and two in the trabecular bone. Of the 44 retrieved implants, 20 implants appeared to be installed partially in the mandibular canal, as clearly visible on the X-rays. These implants were not used in the histomorphometrical measurements. Histological evaluation showed that the trabecular and cortical bone reactions were similar; there was no significant difference in the percentage of bone contact nor in the amount of bone in contact with the implants. In conclusion this study showed that the mandibular corner is an unsatisfactory model for the installation of implants because of anatomical restrictions. Also, the experiment remained inconclusive about the influence of loading conditions on bone behaviour. Nevertheless, the histological results confirmed the bioactive properties of Ca–P coatings.

    Notes: A rat bone marrow cell culture was used to evaluate the osteogenic potential of amorphous and crystalline thin calcium phosphate (Ca/P) coatings. The coatings were deposited on titanium discs using a radiofrequency magnetron sputter procedure. Amorphous and crystalline plasma spray Ca/P coated and noncoated titanium discs served as reference material. The cellular behavior was analyzed with quantitative (attachment and proliferation rates) and qualitative (scanning electron microscopy) techniques. No significant differences were found in cell attachment and proliferation rates between the various materials. Scanning electron microscopy showed extracellular matrix formation after 18 days of culture on amorphous plasma-sprayed and the two types of magnetron sputtered coatings. Furthermore, no severe degradation of the magnetron sputtered coatings was observed. They even appeared to induce apatite formation. On basis of the results, we conclude that magnetron sputtering appears to be a promising method to manufacture bioactive ceramic coatings. © 1995 John Wiley & Sons, Inc.

    Notes: It has been suggested that during wound healing microtextured surfaces can alter events at the interface between implant surface surface and surrounding tissues. To investigate this phenomenon, smooth and microtextured silicone rubber implants were implanted subcutaneously in rabbits for 3, 7, 42, and 84 days. The textured implants possessed parallel surface microgrooves and ridges with a width of 2.0, 5.0, and 10.0 μm. All grooves had a depth of approximately 0.5 μm. SEM observation showed fibroblasts, erythrocytes, lymphocytes, macrophages, fibrin, and collagen on all implant surfaces after 3 and 7 days. After 42 and 84 days only little collagen, a small number of fibroblasts, but no inflammatory cells were seen on the implant surfaces. The fibroblasts were not oriented along the surface grooves on all textured surfaces. Three-dimensional reconstruction of CLSM images and LM images showed no significant differences between the thickness of the capsules surrounding the smooth and those surrounding the microgrooved implants. In contrast, LM did show a significantly lower number of inflammatory cells and a significantly higher number of blood vessels in the capsules surrounding the microgrooved implants. Differences between the 2.0, 5.0, and 10.0 μm grooved implants were not detected. Our results concerning the capsule thickness suggest that the depth of our grooves was not sufficient to facilitate mechanical interlocking, but the cause for the observed differences in inflammatory response and number of blood vessels remains unclear. © 1997 John Wiley & Sons, Inc.     Notes: This study evaluated the effect of rapid heating with infrared radiation on the physico-chemical and morphological properties of radio frequent (RF) magnetron-sputtered calcium phosphate (Ca-P) coatings. About 2.5 μm thick Ca-P coatings were deposited on titanium disks and cylinders. These specimens were left untreated or were heat treated by infrared radiation at 300, 400, 500, 600, and 700°C for 4, 7, 11, 17, and 24 s. Subsequently, the specimens were immersed in simulated body fluid (SBF) for 1 day, 1 week, and 5 weeks. X-ray diffraction measurements showed that heating at 500°C or higher resulted in an increase of coating crystallinity. In addition, FT-IR measurements revealed the appearance of OH peaks in the spectra of samples treated at 500-700°C. Electron probe microanalysis showed that after 5 weeks of immersion about 40-50% of the coatings heat treated at 500 and 600°C was maintained. The coatings heat treated at 700°C showed no dissolution at all. On the other hand, as-coated and 300°C treated films were dissolved within 1 day. Scanning electron microscopy of the samples showed that directly after heat treatment no apparent cracks were present in the coatings. On the basis of these findings, we conclude that rapid heating with infrared radiation around 600°C is the best heat treatment for RF magnetron-sputtered coatings. © 1997 John Wiley & Sons, Inc.
