    Notes: Abstract  A 66-year-old male patient was hospitalized for severe heart failure. Within 3 days after admission his platelet count dropped from 148×109/l to 3×109/l. Pseudothrombocytopenia was diagnosed based on the platelet count of 3×109/l in K3-EDTA anticoagulated blood as opposed to 110×109/l in sodium citrate. Platelet reactive antibodies were not detectable. To the best of our knowledge, this is the first report of a documented onset of pseudothrombocytopenia.

    Notes: Summary Lp(a), one of the most atherogenic lipoproteins, is believed to contribute significantly to vascular diseases in non-insulin-dependent diabetic (NIDDM) patients. Contradictive data have been published on these patients concerning plasma concentrations of Lp(a) and their relation to renal function. Since apo(a) fragments appear in urine, we measured urinary apo(a) in 134 NIDDM patients and 100 matched controls and related urinary apo(a) concentrations to plasma Lp(a) levels and kidney function. Plasma Lp(a) values were found to be significantly higher in NIDDM patients. NIDDM patients also secreted significantly more apo(a) into their urine as compared to control subjects. There was no correlation between creatinine clearance or albumin excretion and urinary apo(a) concentrations. Patients with macroalbuminuria exhibited a twofold higher apparent fractional excretion of apo(a) in comparison to patients with normal renal function. Urinary apo(a) values in both patients and control subjects were highly correlated to plasma Lp(a), yet no correlation was found with HbA1 c or serum lipoproteins. It is concluded that urinary apo(a) excretion is correlated to plasma Lp(a) levels but not to creatinine clearance in patients suffering from NIDDM. [Diabetologia (1997) 40: 1455–1460]

    Notes: Abstract Objective: To demonstrate that emergency aortic valve replacement can be successfully performed in patients with critical aortic stenosis and reduced left ventricular function even in cardiogenic shock with associated severe multiple organ failure. Design: Retrospective, consecutive case series. Setting: Multidisciplinary intensive care unit of a tertiary care university hospital. Patients: Five patients admitted to the intensive care unit with critical aortic stenosis (aortic valve area 0.56 ± 0.13 cm2) and greatly reduced left ventricular ejection fraction (20 ± 3 %) in prolonged cardiogenic shock and associated multiple organ failure (Multiple organ failure score 6.8 ± 0.5; Acute Physiology, Age, and Chronic Health Evaluation III score 91 ± 27). Intervention: Emergency aortic valve replacement. Results: All patients survived with full recovery of organ function. At follow-up (18 ± 10 months) all patients were in New York Heart Association functional class I or II with improvement of left ventricular ejection fraction to 48 ± 25 %. Conclusions: This excellent outcome suggests that emergency aortic valve replacement should be strongly considered in patients with critical aortic stenosis even in cardiogenic shock and multiple organ failure.

    Notes: Durch die Markteinführung von Sildenafilcitrat (Viagra®) im Oktober dieses Jahres kann es vorkommen, daß Notärzte Patienten behandeln, die Viagra® eingenommen haben. Dabei sind folgende Situationen denkbar: 1) Der Patient nimmt als Dauermedikation NO-Donatoren, es kommt zur versehentlichen Kombination mit Sildenafilcitrat und in der Folge zu einem schweren Blutdruckabfall. 2) Relevante Systemerkrankungen wie Diabetes, KHK oder arterielle Hypertension sind wichtige Ursachen der erektilen Dysfunktion. Die mit einer sexuellen Aktivität assoziierte psychologische wie körperliche Belastung kann bei diesen Patienten koronare Ischämien auslösen. Die Gabe von NO-Donatoren (Selbstmedikation, Rettungsdienstpersonal, Hausarzt etc.) führt dann möglicherweise zu bedrohlicher Hypotension. Aus diesem Grunde fand am Rande des Kongresses der European Society of Cardiology in Wien am 25. August 1998 ein Expertenworkshop statt, der auf der Basis des derzeit verfügbaren Datenmaterials vorläufige Therapieempfehlungen zu Sildenafil und NO-Donatoren erarbeitete.

    Notes: In aqueous polyethylene glycol/dextran two-phase systems, the hydrophobicity, free volume, surface tension, and interfacial tension of the phases in equilibrium were measured as a function of pH and ionic strength. These parameters were found to change with pH, but the pattern and magnitude cannot explain the unusual partition of charged macromolecules, observed previously. The electrostatic potential difference was determined by a new experimental approach based on the measurement of the pH difference between the phases at equilibrium. In polyethylene glycol/dextran systems containing sodium chloride as ionized species, the electrostatic potential is not constant in the pH range 2 to 11. The partition behavior of charged macromolecules and its dependence on pH can be explained by the combined action of charge and phase potential. This conclusion was tested with poly-L-glutamate, which partitioned as predicted and in a pattern opposite to positively charged macro- molecules. © 1995 John Wiley & Sons, Inc.

    Notes: A physicochemical model for vapor-liquid equilibria and enthalpies in formaldehyde-containing mixtures is revised and extended using recently published NMR spectroscopic data on chemical equilibria in aqueous and methanolic formaldehyde solutions as well as new vapor-liquid equilibrium data for the system formaldehyde-water at temperatures up to 413 K (pressures up to 450 kPa), which were taken in this work with a new apparatus. Furthermore, the model is revised so that it can be extended straight-forwardly to explicitly account for reaction kinetic effects based on results from other recent NMR spectroscopic studies. The model is tested over the entire temperature range (293-413 K), where reliable experimental data on vapor-liquid equilibria are available, and against calorimetric data. Typical relative deviations between experimental data and the correlation are about 5% for vapor-liquid partition coefficients and 2% for the pressure. Deviations between calculated and measured enthalpies of vaporization are generally below 2%.
