    Notes: Summary Twenty patients (5 females, 15 males) with severe heart failure (NYHA IV), due to coronary artery disease in 14, and congestive cardiomyopathy in 6, received an intravenous bolus of the calcium blocker nisoldipine 0.2 mg followed by a continous infusion of 0.2 μg · kg−1 · min−1. Haemodynamic measurements were performed at baseline and after 30 min. The mean arterial pressure fell from 91 to 73 mm Hg, pulmonary capillary wedge pressure from 31 to 26 mm Hg and systemic vascular resistance from 1695 to 1040 dyn · s · cm−5. The cardiac index (2.2 to 2.71 · min−1 · m−2, and stroke volume index (25 to 33 ml · m−2) were markedly increased. There was no reflex tachycardia as the heart rate dropped from 92 to 85 beats · min−1. Plasma renin activity and norepinephrine concentration did not change significantly. The findings indicate that nisoldipine acts as a strong vasodilator and that it has a beneficial acute haemodynamic effect in patients with severe left heart failure irrespective of its aetiology.

    Notes: A position sensitive one-dimensional x-ray detector with a large dynamic range ((approximately-greater-than)214:1) for high photon fluxes with fast image recording sequence (300 ms per frame) has been developed for transvenous coronary angiography with synchrotron radiation. A position resolution of 1 LP/mm and a detection quantum efficiency (DQE) of at least 58% (for 11 000 photons per pixel) has been achieved for 33 keV photons in a Xe-CO2 gas mixture at 20 bars. The use of Kr-CO2 as conversion gas provides a better contrast of the weak iodine signal than Xe-CO2 or Si, respectively, for a fraction of 2% of the second harmonics of the synchrotron beam used. © 1995 American Institute of Physics.

    Notes: During the last years attempts have been underway at several synchrotron radiation laboratories1–4 to employ the abrupt change of absorption at the iodine K edge (33.17 keV) to the imaging of coronary arteries. The subtraction of two images taken with photon energies just above and below the edge strongly surpresses background contrast like bone and soft tissue and allows for amplification of iodinated structure contrast. At HASYLAB, for these studies, a beamline at a bending magnet was only available until 1987. At this beamline the system NIKOS I was tested to obtain information on its essential components like monochromator and detector by in-vivo investigations of dogs.The basic feature which differs from the approaches in other laboratoriesis simultaneous registration of two stationary beams in a two-line detector without any beam switching. Based on the experience with this system during the last year all its components have been improved and changed to dimensions capable of taking a 12-cm-wide image of the human heart. At present the new system NIKOS II is under testing at the new wiggler beamline HARWI. The new monochromator comprises two pairs of a thin Si Laue case crystal and a Ge Bragg crystal, each, with inherent focussing of the beam to a horizontal line focus of about 0.5 mm height and 12 cm width. The advantage of the Laue crystals which are optimized in thickness (30 μm) for maximum reflectivity, is the low heat absorption and the full use of the white beam in the production of two monochromatic beams. The crystals are cooled in a stream of helium. Compared to the NIKOS I version5 the detector was changed in the following points: The two detection lines are enlarged to 125 mm width and the phosphor at the entrance is exchangeable to be either made of powderlike Gd2O2S:Tb or of single crystals like CdWO4 or CaF2:Eu separated by thin walls for optimum spatial resolution. The numerical aperture of the glass fiber optics guiding the light via image intensifiers to the photodiode arrays is higher. The gain of the intensifiers is raised to 1000.Now two photodiode arrays are needed for the simultaneous readout of the two entrance lines. The readout time is halved by reading the odd and even number diodes by two ADCs per array. All these measures will increase the detective quantum efficiency from 0.2 to 0.6. The performance of the new system will be tested again by investigations of dogs before moving to investigations on humans, expected at the end of this year.

    Description / Table of Contents: Zusammenfassung Einleitung: Obwohl die Bedeutung der selektiven Koronarangiographie (SCA) in der Diagnostik der koronaren Herzkrankheit unumstritten ist, ergibt sich aus der Invasivität mit den möglichen assoziierten Komplikationen, evtl. resultierenden Folgekosten und dem in den meisten Fällen notwendigen eintägigen Krankenhausaufenthalt der Bedarf für neue, nichtinvasive Techniken im Bereich der Koronardiagnostik. Mit der intravenösen Koronarangiographie mit Synchrotronstrahlung (ICA) steht ein Verfahren zur Verfügung, welches die nichtinvasive Darstellung der Koronararterien und Stents ohne Artefakte erlaubt. Methodik: 66 Patienten (80% männlich, Alter 62 ± 8.5 Jahre) nach Koronarintervention bzw. CABG-Operation wurden innerhalb von maximal 6 Wochen mittels SCA und ICA untersucht. Nach Applikation eines Kontrastmittels (370 mg Jod/ml, 15 ml/s, 30 ml) durch eine 6 F (7 F)-Schleuse in der V. brachialis wurde in 2 unterschiedlichen Projektionen 6–8 Bilder/Patient akquiriert. Zielgefäße für die Evaluation waren: LAD n = 55, Cfx n = 21, RCA n = 54, CABG n = 52. In 50 Zielgefäßen waren ein oder mehr Stents implantiert. Ergebnisse: 182 Gefäße wurden nach folgenden Kriterien beurteilt: Keine Stenose, 〈 70%, 〉 70% und Okklusion. Die Beurteilung der ICA-Befunde und der SCA erfolgte durch zwei unabhängige Untersucher. Die Ergebnisse der ICA wurden mit denen der SCA verglichen. Wegen schlechter Bildqualität konnten n = 17 Gefäße nicht beurteilt werden. Folgende Sensitivität/Spezifität für die einzelnen Koronargefäße wurden errechnet: LAD 84%/93%, RCA 85%/97%, Cfx 67%/90%, Grafts 85%/97%. Schlußfolgerungen: Es konnte gezeigt werden, daß ICA eine durchführbare und sichere Technik für das ambulante Follow-up von Patienten nach PTCA/Stentimplantation und CABG-OP darstellt. Im Gegensatz zu anderen nichtinvasiven Techniken ist die Beurteilung auch bei Stents und Kalzifikationen möglich. Bei guter Bildqualität lassen sich weitgehend überlagerungsfrei projizierbaren Gefäße (LAD, RCA, Grafts) mit akzeptabler Sensitivität und Spezifität darstellen. Die niedrige Sensitivität bei der Darstellung der Cfx aufgrund von Überlagerung durch den linken Ventrikel muß durch weitere Entwicklungen im Bereich der Bildverarbeitung kompensiert werden.
    Notes: Summary Introduction: Coronary arterty disease still remains the primary cause of death in the western industrialized world. Although the clinical value of selective coronary angiography (SCA) is beyond dispute, the associated risk of an invasive approach, the inherent costs and the necessary hospitalization have lead to the development and investigation of novel non-invasive techniques for coronary imaging. Intravenous coronary angiography (ICA) has been shown to permit non-invasive imaging of the coronary arteries. Methods: In 66 pts (80% male, age 62 (± 8.5 years) after interventional therapy/CABG operation, ICA and a SCA were carried out within a time interval of 〈 6 weeks. After determination of the individual circulation time, contrast media (370 mg iodine/ml, 15 ml/s, 21 ml) was injected via a sheath in the cubital vein while the patient was sitting in an upright position in a specially designed scanning chair. In two differet projections 6–8 images/patient were obtained for further image processing and evaluation. 182 target vessels had to be evaluated (LAD 55, Cfx 21, RCA 54, Grafts 52). In 50 target vessels one or more stents were implanted. Results: 182 target vessels were evaluated according to the following criteria: no stenosis, 〈 70%, 〉 = 70%, occlusion. Evaluation of the ICA and SCA images was performed by two independent investigators. Due to poor image quality, 17 vessels were not evaluated. The ICA findings were compared to that of SCA. For the LAD a sensitivity of 84% (specificity 93%), for the RCA a sensitivity of 85% (specificity 97%), for the Cfx a sensitivity of 67% (specificity 90%), and for grafts a sensitivity of 85% (specificity 97%) was calculated. Conclusion: ICA proved to be a feasible and safe technique for follow-up after coronary intervention/CABG operation on an outpatient basis. Evaluation of stents and severe calcification is possible. A good image quality provided, LAD RCA and grafts can be evaluated with an acceptable sensitivity and specifity. Due to superimpositioning the low sensitivity for the Cfx has to be compensated by further image processing.

    Notes: Durch die Markteinführung von Sildenafilcitrat (Viagra®) im Oktober dieses Jahres kann es vorkommen, daß Notärzte Patienten behandeln, die Viagra® eingenommen haben. Dabei sind folgende Situationen denkbar: 1) Der Patient nimmt als Dauermedikation NO-Donatoren, es kommt zur versehentlichen Kombination mit Sildenafilcitrat und in der Folge zu einem schweren Blutdruckabfall. 2) Relevante Systemerkrankungen wie Diabetes, KHK oder arterielle Hypertension sind wichtige Ursachen der erektilen Dysfunktion. Die mit einer sexuellen Aktivität assoziierte psychologische wie körperliche Belastung kann bei diesen Patienten koronare Ischämien auslösen. Die Gabe von NO-Donatoren (Selbstmedikation, Rettungsdienstpersonal, Hausarzt etc.) führt dann möglicherweise zu bedrohlicher Hypotension. Aus diesem Grunde fand am Rande des Kongresses der European Society of Cardiology in Wien am 25. August 1998 ein Expertenworkshop statt, der auf der Basis des derzeit verfügbaren Datenmaterials vorläufige Therapieempfehlungen zu Sildenafil und NO-Donatoren erarbeitete.
